Role of Microbiome in Stomach Cancer Development

Objective of the Review: to analyse new information on the role of microbiome in stomach cancer development. Key Points. Study of microbiome in stomach cancer patients is one of the actively developing areas of modern medicine. Study results are very ambiguous and sisputable. Sequencing of 16S ribosomal ribonucleic acid made it possible to study microorganisms that colonise the stomach, which could not be studied with other methods. It allowed identifying a number of gastric bacteria and study their role in pathology. Conclusion. Most likely there will appear new information confirming the role of dysbiosis in gastric carcinogenesis. Keywords: microbiome, dysbiosis, stomach cancer, gastritis, Helicobacter pylori.

RELEVANCE In patients with colorectal cancer (CRC), the normal small intestine, located outside the pathological focus, undergoes changes that may be the cause of digestive dysfunction after radical surgery to remove the tumor.The assessment of metabolic and microstructural changes in the ileum mucosa in patients with colorectal cancer is necessary to correct the algorithms of postoperative therapy and enteral nutrition. The development of the tumor process involves the involvement of various functional elements of innate and adaptive immunity. A special role in the reorganization of tissues, changes and evolution of the properties of cancer cells is given to cytokines, the study of the role of which in these processes with various types of neoplasms and, in particular, with nonmetastatic gastric cancer is an urgent task. A total of 152 stool samples from 76 patients with symptoms of gastrointestinal diseases have been analyzed using metagenomic shotgun sequencing technology to assess the effect of Helicobacter pylori eradication therapy on Bifidobacterium, Lactobacillus, Escherichia, and Clostridium genera. The relative abundance of bacteria representing these genera in the intestinal microflora of patients before and after antibiotic therapy has been evaluated. It has been shown that the therapy did not have any critical effect in the majority of cases on the number of Lactobacillus, Escherichia and Clostridium genera in the microbial community. Their abundance varied within 0.5% in 76.5%, 51.3%, and 55.2% of patients, respectively. The Bifidobacterium genus has been found to be more susceptible to antibiotics (their number decreased significantly in 60.5% of cases). However, 9.2% of patients have shown the opposite effect. Bacterial ecosystem of the gut plays a fundamental role in the normal functioning of the metabolic and immune systems. Functional bowel disease and obesity are highly prevalent in the population and place a heavy burden on healthcare system. Both comorbidity and multimorbidity are considered to be common for obesity and intestinal functional disorders. Changes in the microbiota can be both the cause and consequence of each disease: intestinal functional disorder changes the composition of the microbiota, resulting in obesity, and vice versa. Вместе с тем не вызывает сомнений тесная связь между микробиотой различных эпитопов нашего организма и функционированием как отдельных его органов и систем, так и всего организма в целом. Накопленный объем достоверных данных о функционировании микробиоценозов организма человека позволяет рассматривать их как ключевые точки влияния на здоровье человека не только в плане лечения имеющихся заболеваний, но и в плане первичной и вторичной профилактики широкого спектра инфекционных, метаболических, иммуноопосредованных и иных заболеваний. В ближайшем будущем предстоит определить показания, найти эффективные пробиотические штаммы, определить оптимальные разовые и суточные дозы, продолжительность приема, а также критерии эффективности проводимых мероприятий.
Direct and Indirect Methods for Studying Human Gut Microbiota

Aim: To review the main methods of intestinal microbiota studying.Key points. Currently, molecular genetic methods are used mainly for basic research and do not have a unified protocol for data analysis, which makes it difficult to implement them in clinical practice. Measurement of short chain fatty acids (SCFA) concentrations in plasma provides the data, which can serve as an indirect biomarker of the colonic microbiota composition. However, currently available evidence is insufficient to relate the obtained values (SCFA levels and ratio) to a particular disease with a high degree of certainty. Trimethylamine N-oxide (TMAO) levels in the blood plasma and urine can also reflect the presence of specific bacterial clusters containing genes Cut, CntA/CntB and YeaW/YeaX. Therefore, further studies are required to reveal possible correlations between certain disorders and such parameters as the composition of gut microbiota, dietary patterns and TMAO concentration.
