The dissertation aims to analyse whether it is possible to have municipal autonomy in the management public health services as well as in the decision-making process that establishes policy priorities, given the institutional features of Brazilian public health system. Two features of this system were considered as the most relevant for this study regarding the limitation of autonomy: the centralization of resources in the federal government and the private sector historical influence in the area. The two main aims of the dissertation are: to analyse municipal autonomy on resources management; to analyse the municipal autonomy on facing the influence of the private sector. On one hand, the municipal autonomy of the resources management is limited by the importance of federal government in the financing of public health, in Brazil. On the other hand, the private sector resists to allow public regulation over services they provide.
