Comparison of DTPA and MAG3 renal scintigraphies in terms of differential renal function based on DMSA renal scintigraphy

Objective: Renal scintigraphy has privileges in imaging methods in terms of providing functional information. Technetium-99m dimercaptosuccinic acid (Tc-99m DMSA) used in imaging of renal parenchyma is the most reliable radiopharmaceutical in the calculation of differential renal function (DRF). In this study, it was aimed to compare the agents of dynamic renal scintigraphy such as Technetium-99m diethylenetriamine pentaacetic acid (Tc-99m DTPA) and Technetium99m mercaptoacethyltriglycine (Tc-99m MAG3) based on Tc-99m DMSA in terms of DRF. Methodology: Tc-99m DTPA and Tc-99m DMSA scintigraphies were performed in Group A; Tc99m MAG3 and Tc-99m DMSA scintigraphies were performed in Group B. Comparison of 99mTc-DMSA, 99mTc-DTPA and 99mTc-MAG3 Renal Scintigraphy in the Calculation of Relative Renal Function

Amaç: Bu çalışmanın amacı Tc-99m dimerkaptosüksinikasit (99m Tc-DMSA) böbrek kortikal sintigrafisi ile Tc-99m dietilentriaminpentaasetik asit (99mTc-DTPA) ve Tc-99m merkaptoasetiltriglisin (99m Tc-MAG3) ile dinamik böbrek sintigrafilerini rölatif böbrek fonksiyonu hesaplanması (RBF) yönünden karşılaştırmaktır. Gereç ve Yöntem: Bu retrospektif çalışmaya; bölümümüzde çeşitli tanılar ile böbrek kortikal sintigrafisi (99m Tc-DMSA ile) ve dinamik böbrek sintigrafisi (99m Tc-DTPA veya 99m Tc-MAG3 ile) uygulanmış 47 hasta dahil edildi. 99m Tc-DMSA, 99m Tc-DTPA ve 99m Tc-MAG3 sintigrafilerinden elde edilen RBF değerleri ile klinik ve demografik veriler istatistiksel olarak analiz edildi. Bulgular: RBF değerlendirme açısından 99m Tc-DMSA statik böbrek sintigrafisi ile dinamik böbrek sintigrafileri (99m Tc-MAG3 veya 99m Tc-DTPA ile) arasında yüksek düzeyde korelasyon bulundu (r=0,981, p<0,001; r=0,918, p=<0,001). 99m Tc-DMSA ve 99m Tc-DTPA ile ölçülen RBF (%) arasındaki Bland Altman grafiği, 3,30'luk ortalama fark gösterirken, 99m Tc-DMSA ve 99m Tc-MAG3'ün Bland Altman grafiği 0,08'lik ortalama fark gösterdi. Sonuç: Sonuç olarak bu çalışma RBF değerlendirme açısından 99m Tc-DMSA böbrek sintigrafisi ile dinamik böbrek sintigrafileri (99m Tc-MAG3 veya 99m Tc-DTPA ile) arasında yüksek düzeyde uyum olduğunu göstermiştir. Amaca uygun tek yöntem kullanımı ile hastanın maruz kaldığı radyasyon, zaman kaybı ve ekonomik kayıplar en aza indirilmiş olur.

Can Tc 99m DTPA be Used in Adult Patients in Evaluation of Relative Renal Function Measurement as the Reference Tc 99 DMSA Method ?

Objective: In the literature, there are many reports comparing relative renal function calculated with Tc 99m DTPA and Tc 99m DMSA in adults and children. However, there is no consensus about the results. As there is indeterminacy in the reliability of Tc 99m DTPA for the calculation of the relative renal functions, we retrospectively designed a study to compare the relative renal functions measured with Tc 99m DMSA and Tc 99m DTPA in adult patients with renal diseases Material and Methods: We retrospectively analyzed the data of 144 patients who applied to Nuclear Medicine Department of three hospitals between 2009 and 2010 and who had both dynamic and static renal imaging. Renal dynamic scintigraphies were compared to the relative function measured using Tc 99m DMSA static scintigraphy. Comparison of relative renal function measurements using dynamic and static renal scintigraphies was performed using Pearson correlation test. The comparison results were expressed with Bland-Altman analysis. Results: The study was conducted with 144 patients and 288 kidneys. Fifty six of patients were male. Mean age was 39.9±15.2 years. Thirty four patients had hydronephrosis, 28 pyelonephritis, 53 renal calculi, 3 chronic renal failure, 2 acute renal failure, 1 benign renal neoplasia, 15 renal atrophy, 8 ureteropelvic junction stenosis. Relative renal function was calculated in Tc 99m DMSA and 99m Tc-DTPA studies. The mean relative renal functions measured with Tc 99m DTPA was 52.54±23.09% and 47.25±23.09, with Tc 99m DMSA 52.85±21.80% and 47.07±21.77% for right and left kidneys, respectively. In bivariate correlation analysis (Pearson) a significant positive correlation was found between the relative renal functions calculated with Tc 99m DTPA and Tc 99m DMSA (r =0.937, p< 0.001). In Bland-Altman plots, the mean difference between two methods was 0.3 and the correlation limits were between 16.2 to-15.5. Conclusion: As a result, we concluded that Tc 99m DTPA is also a good method for the relative renal function evaluation when compared to Tc 99m DMSA scan. Although Tc 99m DMSA is the most reliable method for the calculation of relative renal function, Tc 99m DTPA can be another choice for the calculation of relative renal function without a complementary DMSA scan particularly in patients who require renogram curve and GFR calculations.

Relative renal functions for children using 99mTC-DTPA and 99mTC-DMSA

Radionuclides are widely used to study the relative renal functions (RRFs) for kidney patients. This work reports the determination of the RRFs using the technetium-99m dimercaptosuccinic acid (99mTc-DMSA) and technetium-99m diethylenetriamine-pentaacetic acid (99mTc-DTPA) for children having kidney diseases. The two methods are then compared to find the choice of scintigraphy for a particular type of disease. A total of 65 child kidney patients of ages 5 months - 16 years were the representative samples of the study. The mean RRFs calculated with 99mTc-DTPA was found as and respectively, for the left and right kidneys. Objective: In dynamic renal scans, 99m Tc-diethylenetriamine pentaacetic acid (DTPA) is copiously used and it yields information about the renal blood-flow and the excretory capacity. 99m Tc-dimercaptosuccinic acid (DMSA) is used for static renal imaging and can likewise uncover the renal cortical structure. This work was intended to evaluate whether DMSA can be declared as a radiopharmaceutical in dynamic renal scans or not. It also aimed at comparing the outcomes procured from DMSA and with DTPA examinations. Materials and Methods: A comparison of the information gained from the renograms of 47 subjects (normal subjects and subjects with abnormal renal function having: obstructive nephropathy, bilateral nephropathy, hydronephrosis, reduced or non-function kidney, and/or atrophic kidney) who had 99m Tc-DMSA and 99m Tc-DTPA dynamic scintigraphies utilizing the same protocol. Results: A strong positive correlation had come in view on the evaluation of the left kidney, right kidney, and total glomerular filtration rate (GFR) from 99m Tc-DTPA and 99m Tc-DMSA renograms. In addition, the estimation of the time of the peak height from 99m Tc-DTPA and 99m Tc-DMSA renograms for the right and left kidneys demonstrated as non-significant varieties and a strong positive correlation for both the normal subjects and subjects with kidney abnormalities. Conclusion: In the light of the outcomes gained from the present investigation, it could be prescribed that 99m Tc-DMSA can favorably replace 99m Tc-DTPA in dynamic renograms in the case of the renal conditions that had been examined in the present work. As scheduled, this will be cost-effective and time saving and will decrease the radiation dose administered to the subjects. Also, the same qualities of results are accomplished with both radiopharmaceuticals in normal subjects and subjects with abnormal renal function.

EFFECT OF REGION OF INTEREST SELECTION AND UPTAKE MEASUREMENT ON GLOMERULAR FILTRATION RATE MEASURED BY 99mTc-DTPA SCINTIGRAPHY IN DOGS

Determinations of different methods of measurement of uptake of ""Tc-DTPA using scintigraphy of glomerular filtration rate (GFR) were made from 29 studies on 10 healthy beagle dogs. GFR was measured by calculating the percentage dose uptake (integral method) and rate of uptake (slope method) of 99mTc-DTPA using manual kidney regions of interest (ROI) and automatic kidney and background ROIs at different time periods of the uptake phase. These results were compared using linear regression analysis to the GFR obtained from 99mTc-DTPA plasma clearance using multiple blood samples. The best correlation coefficient between percentage DTPA uptake and GFR by DTPA clearance (r = 0.84, P < 0.001) was derived from time intervals between 30s-120s with a perirenal background ROI at 1 or 2 pixels out from the kidney ROI using automatic kidney ROI at 20% threshold. With the slope method, the best correlation coefficient (r = 0.85, P < 0.001) was obtained from time intervals between 30s-peak with the background ROI at 2 pixels out from the kidney ROI using automatic ROI at 35% threshold. The offset was higher, and the correlation varied more with different ROIs and the method was unreliable at time intervals extending beyond the peak radioactivity. Manual kidney ROIs with automatic background ROIs had slightly lower correlations. With DTPA renography both integral and slope uptake method with automatic kidney and background ROIs are accurate methods to estimate the GFR, but that the integral method is much more stable to variations in ROI size and the duration of the uptake phase of the renogram.
