Personalized nutrition diagnostics at the point-of- need

Micronutrient deficiency is widespread and negatively impacts morbidity, mortality, and quality of life globally. Ongoing advancements in nutritional biomarker discovery are enabling objective and accurate assessment of an individual's micronutrient and broader nutritional status. The vast majority of such assessment however still needs to be conducted in traditional centralized laboratory facilities which are not readily accessible in terms of cost and time in both the developed and developing countries. Lab-on-a-chip (LOC) technologies are enabling an increasing number of biochemical reactions at the point-of-need (PON) settings, and can significantly improve the current predicament in nutrition diagnostics by allowing rapid evaluation of one's nutritional status and providing an easy feedback mechanism for tracking changes in diet or supplementation. We believe that nutrition diagnostics represents a particularly appealing opportunity over other PON applications for two reasons: (1) healthy ranges for many micronutrients are well defined which allows for an unbiased diagnosis, and (2) many deficiencies can be reversed through changes in diet or supplementation before they become severe. In this paper, we provide background on nutritional biomarkers used in nutrition diagnostics and review the emerging technologies that exploit them at the point-of-need.

The First 1,000 Days: Trends towards Biosensing in Assessing Micronutrient Deficiencies

:Micronutrients provide the essential building blocks for brain development, healthy growth and a strong immune system in children. Malnutrition in form of micronutrient deficiencies develop gradually over time and their negative effects are not seen until irreversible damage may have occurred. The World Health Organization (WHO) supports the 2016 Global Nutrition Report (GNR), emphasizing the enormous importance of investing in the critical first 1,000 days nutritional requirement as it directly affects the attainment of 12 out of the 17 United Nations (UN) Sustainable Development Goals (SDGs). Up till now, in many countries, early detection of malnutrition is carried out by specific, majorly blood-based tests in specialized laboratories by trained personnel. This review expatiates on diagnostic trends towards early detection of micronutrient malnutrition highlighting the significant role of Engineering in this cause. Focusing on the children within the 1000day critical window, suggestions on modalities for continual tracking required to prevent malnutrition using biosensors are also advanced in this review.

Precision nutrition involves several data collection methods and tools that aim to better inform nutritional recommendations and improve dietary intake, nutritional status, and health outcomes. While the benefits of collecting precise data and designing well-informed interventions are vast, it is presently unclear whether precision nutrition is a relevant approach for tackling nutrition challenges facing populations in low- and middle-income countries (LMIC), considering infrastructure, affordability, and accessibility of approaches. A digital dietary assessment tool may help identify malnutrition and nutritional deficiencies in hospitalized patients

Malnutrition is common among hospitalized patients and associated with longer hospital stays, higher rates of rehospitalization, and increased mortality. Validated questionnaires of varying sensitivity and specificity to help identify patients at risk of malnutrition have been developed, but none has been broadly adopted. Tools to identify patients at risk for malnutrition should be quick, inexpensive, easy to administer and use, not require specialized nutrition knowledge, and provide results which can be entered into an electronic medical record; ideally, the tool should be deployed within 24 hours of admission and repeated if warranted. The serious problem of hospital undernutrition is still being underestimated, despite its impact on clinical evolution and costs. The screening methods developed so far are not useful for daily clinical practice due to their low effectiveness/cost ratio. We present an screening tool for CONtrolling NUTritional status (CONUT) that allows an automatic daily assessment of nutritional status of all inpatients that undergo routine analysis. The system is based on a computer application that compiles daily all useful patient information available in hospital databases, through the internal network. Evaluation of Micronutrient Sensors for Food Matrices in Resource-Limited Settings: A Systematic Narrative Review

In resource-limited settings, mass food fortification is a common strategy to ensure the population consumes appropriate quantities of essential micronutrients. Food and government organizations in these settings, however, lack tools to monitor the quality and compliance of fortified products and their efficacy to enhance nutrient status. The World Health Organization has developed general guidelines known as ASSURED (Affordable, Sensitive, Specific, User-friendly, Rapid and Robust, Equipment-free, and Deliverable to end-users) to aid the development of useful diagnostic tools for these settings. These guidelines assume performance aspects such as sufficient accuracy, reliability, and validity.
