Excessive gingival display during smiling ("gummy smile") is an esthetic issue that affects a considerable part of the population. The hyperactivity of the elevator muscle of the upper lip is one of the main causes of a gummy smile, and several techniques have been proposed for its treatment. The aim of this report is to describe a modification of the lip repositioning technique to achieve stable and significant outcomes through a more conservative procedure. Two patients complaining about a gummy smile were treated with the proposed technique and presented, after a 6-month follow-up, significant improvement in the amount of gingival exposure and esthetic satisfaction. Case Report Surgical Lip Repositioning: A Compelling Procedure to reduce Gingival Display
Excessive gingival display or 'Gummy smile' is an aesthetic concern of an individual, affecting the overall personality. It can be predictably managed by Orthognathic surgery and Orthodontic treatment. The purpose of present article is to report an alternative and comparatively less invasive surgical procedure 'Lip repositioning' to treat 'gummy smile'. This case report discusses a 26 years Indian male patient presented with Excessive gingival display. On examination, dynamic smile was extending from maxillary right first molar to maxillary left first molar, with 4 to 5mm of excessive gingival display with normal maxillary anterior anatomic proportions. A comprehensive treatment plan was made which includes Scaling and Root planing followed by Lip positioning procedure to correct excessive gingival display. The purpose of presenting this case is to describe an alternative treatment modality for treatment of excessive gingival display.
A Cosmetic Technique Called Lip Repositioning in Patient of Excessive Gingival Display

Excessive gingival display can be managed by a variety of treatment modalities, depending on the specific diagnosis. A 29-year old woman was referred to the Unit of Oral Surgery of the University of Chieti-Pescara by her dentist for a consultation regarding a gummy smile. A partial-thickness dissection was made. The epithelium and connective tissue was excised. Tissue tags were removed. The mucosal flap was advanced and sutured at the mucogingival junction using 5-0 polypropylene sutures and 4.0 chromic gut sutures. No periodontal dressing was placed. Postoperative instructions included recommendations for limited facial movements, no brushing around the surgical site for 14 days, and placing ice packs over the upper lip. This treatment modality was effective, producing esthetically acceptable smiles in these patients. This case report demonstrates the successful management of excessive gingival display with a lip-repositioning procedure.

Lip repositioning surgery is a largely unknown and underutilized treatment modality for excessive gingival display. It involves precise resection of maxillary mucosal tissues with reattachment of the lip in a more coronal position. This limits lip elevation on smiling and increases lip fullness. While this is an elective treatment, no reported cases have yet offered patients the ability to preview the outcome in a reversible manner. This case series presents seven patients who were successfully managed with trial, and then definitive, lip repositioning.
Clinical advantages of improving the excessive gingival display (EGD) by surgical repositioning of the upper lip, 2022
Objectives The objective of this study is to minimize gingival display by surgical repositioning of the upper lip and to suggest this technique as an alternative treatment modality to orthognatic surgery for treatment of excessive gingival display. Materials and methods Forty-eight patients were selected with gingival display of more than 2 mm during maximal smiling. All patients underwent surgical repositioning of the upper lip, aimed at limiting elevator muscle activity to treat excessive gum exposure. Patients were regathered in 1 week for the follow-up postoperative symptoms according to VAS scale to evaluate pain and swelling. Reference values were given to patients. Results The study has indicated good results and stability especially to patients with skeletal class I classification along with medium and thick biotype of attached gingiva without hypermobile upper lip. Conclusion Surgical repositioning of the upper lip is an effective way to improve a patient's gingival smile caused by degree I and II VME in combination with HUL as an alternative treatment modality to orthognathic surgery. This method is less invasive and cost-effective, causes minimal postoperative complications, and provides faster recovery. Clinical relevance Excessive gingival display (EGD) with various etiologies requires several proper treatment modalities. The proposed modified method of lip repositioning to reduce the degree of gingival display is less aggressive, reliable, and causes fewer postoperative complications compared to orthognathic surgery.
In the era of aesthetic dentistry a high smile line is a challenge well known to any aesthetically minded dentist. The exposure of more than three mm of the gum during the smile is known as gingival or gummy smile. This condition is known as excessive gingival display (EGD). EGD can be due to altered passive eruption, Hypermobile upper lip, Short upper lip, Vertical maxillary excess. Lip repositioning surgery (LRS) is a new aesthetic surgical procedure for managing excessive gingival display. It involves precise resection of maxillary mucosal tissues with suturing of the lip in a more coronal position. With the help of reversible surgical trial patient and doctor can preview final results. Thus LRS turns out to be new conservative approach for managing EGD. This case report presents decision making for management of excessive gingival display and surgical technique of Lip Repositioning Surgery for Management of Excessive Gingival Display.
Modified Lip Repositioning: A Conservative Approach to Treat the Gummy Smile: A Case Report

Background: Unesthetic gummy smile or "Excessive Gingival Display" is a frequent finding among the population that can occur because of various intraoral or extraoral etiologies. This clinical report describes a successful surgical coverage obtained by modified lip repositioning, thus surgically treating gummy smile. The procedure restricts the muscle pull of the elevator lip muscles by shortening the vestibule, thus reducing the gingival display while smiling. The technique is performed by removing two strips of mucosa from maxillary buccal vestibule on both the sides leaving the frenum untouched and creating a partial thickness flap between mucogingival junction and upper lip musculature, and suturing the lip mucosa with mucogingival junction, thereby reducing gingival display. This technique is different from the conventional surgical lip repositioning as labial frenum is left untouched over here as it helps in maintaining the lip symmetry therefore reduces postoperative morbidity.

The aim of present case report is to discuss surgical lip repositioning technique for the management of excessive gingival display related with vertical maxillary excess and increase mobility of the upper lip. By restricting the muscle pull by elevator lip muscles, this procedure reduces excessive gingival display during smiling. For patient, this procedure is safe, less invasive with minimal side effects and an alternative to orthognathic surgery in the correction of gummy smile.
