The executive physical examination has been advocated in the United States for almost 100 years. A MEDLINE search of the literature found very few studies that document the potential impact of a worksite physical examination program on medical and disability costs. Bank One has performed executive physical examinations at its corporate headquarters' medical department since 1983. Approximately 65% of eligible executives voluntarily participate in the program annually. Medical claims and short term disability data were available for a total of 1773 executives who were eligible for a physical examination for a consecutive 3-year period. For three consecutive years after the initial physical examination, the Bank paid a total of 5361formedicalclaimsforperiodichealthexaminationparticipants(PHE)incontrastto5361 for medical claims for periodic health examination participants (PHE) in contrast to 5361formedicalclaimsforperiodichealthexaminationparticipants(PHE)incontrastto6426 paid for medical claims for non-periodic health examination participants (NPHE). PHE participants experienced an average 0.93 (or 2.78 for 3 years) short-term disability days absent per year in comparison with an average of 1.34 (or 4.02 for 3 years) short-term disability days absent for NPHE. The net return on investment for a worksite-based executive health examination which cost approximately 400perexecutivewhosetotalcompensation(salaryandbenefits)isatleast400 per executive whose total compensation (salary and benefits) is at least 400perexecutivewhosetotalcompensation(salaryandbenefits)isatleast125,000 is estimated to be 2.3:1, which compares favorably with other preventive health programs. Cognitive Screening in Asia: Recognizing the Role of the Patient-Family Unit
Dementia and Geriatric Cognitive Disorders, 2010
Background/Aims: Cognitive screening programmes may improve awareness and help at-risk subjects receive earlier medical attention. Cognitive profiles of subjects who attend cognitive screening by personal choice (self-referred) compared to those where the referral was initiated by family members (family-referred) were compared. Methods: A cross-sectional survey of community subjects attending a cognitive screening initiative. Performance on the MMSE, Frontal Assessment Battery (FAB), Elderly Cognitive Assessment Questionnaire (ECAQ) and Even Briefer Assessment Scale for Depression was evaluated. Results: A total of 342 subjects with a mean age of 59.2 ± 9.0 years were screened. Overrepresentation of Chinese and Indian subjects and underrepresentation of Malay subjects was noted. The prevalence of cognitive impairment ranged from 7.0 to 9.6% depending on the screening instrument used. Of the 342 subjects, 267 were self-referred, while 75 subjects were family-referred. Family-referred...

Annual adult health screening: barriers, challenges and effectiveness
International Journal Of Community Medicine And Public Health
Although it has been well-established that screening for early detection and intervention practices has been associated with favorable outcomes, there have been many concerns about the potential of these approaches to cause harm to the patients. In addition, evidence also shows some associated barriers and challenges to these approaches that need to be considered by healthcare authorities when planning for such approaches. The present literature review discusses annual adult health screening programs' effectiveness, barriers, and challenges. There is no doubt that these screening programs can help identify undiagnosed disorders among many patients with various conditions, like diabetes mellitus, hypertension, and different cancers. Moreover, the effectiveness of these screening programs can be best highlighted by the premarital screening program in Saudi Arabia, which targets high-risk couples for developing certain conditions and infections. However, it should be noted that the...
Periodic health checkup (PHC) has been recognized as a key instrument in the practice of preventive healthcare since the early 1900s and has been supported for maintaining health and preventing diseases, however, it has never been shown to be economical. Periodic Health Checkups (PHC), a primary level of care provided by healthcare facilities to prevent and manage several chronic diseases, PHCs comprise history-taking, clinical examination, and laboratory investigations in which even asymptomatic people of any age can participate. Disease outcomes and activities connected to health are correlated with socioeconomic level, Low socioeconomic level people are more susceptible to harmful living conditions, obesity, cancer, cardiovascular disease, and other health problems, all of which have a detrimental impact on their quality of life. Moreover, because to their limited exposure to hospitals for basic healthcare services, socioeconomic inequality makes it difficult for such people to b...

health workers are unable to deliver services effectively without appropriate physical capitals. [1] The primary health care is managed by the rural primary health centers (PHCs), where only primary health care is provided. Apart from treatment, provision of health care is the most important event here. The first-level intervention starts from PHC. Patient from the rural setup with any type of ailments approaches the PHC and seeks advice or treatment depending on the nature and seriousness of the impaired health condition of the individual. [2] The Planning Commission of India has released a document on universal health coverage in which health service entitlements are proposed for different disease categories. [3] The goal of universal health coverage is to ensure that all people obtain the health services they need without suffering financial hardship when paying for them. Background: Skilled health workers are unable to deliver services effectively without appropriate physical capitals. Objective: The current study is pursued with the objectives (1) to study the competence level of medical officers in providing the treatment and care of two specific noncommunicable diseases and emergency conditions. (2) To score primary health centers (PHCs) on the basis of vulnerability by using Vulnerability Index calculator. (3) To know about the availability of the medicines to treat diseases in question, important instruments, and vehicle. Materials and Methods: The current study is a cross-sectional study involving PHC medical officers (MBBS). There are total 47 PHCs in Anand district in Gujarat. It was decided to include 50% PHCs from each block out of total 47 PHCs. The PHCs were selected by systematic random sampling with sampling interval of 2 and ni calculated for each block separate. Σni is 25. Results: We found that the median knowledge scores for hypertension and diabetes among PHC medical officers were 4.00 and 4.50 out of 10. Mean Vulnerability Index was 7.36, which is overall coming in moderate vulnerability. A total of 40% medical officers were able to identify correctly at least 2 symptoms of hypertension. A total of 88% medical officers were able to handle cases of snake bite and bee stings. In all the PHCs, emergency lifesaving drugs were available. Conclusions: There should be proper training of medical officers in the treatment of noncommunicable diseases. Logistics that are not available should be made available.
