Clinical profile of patients with abdominal tuberculosis in rural North India

Background: Tuberculosis is a very serious health hazard in India. The nonspecific clinical features of abdominal tuberculosis (TB) have made its diagnosis difficult, which can lead to the poor outcome in patients who are not able to receive early treatment especially from rural areas. Aim of our study was to determine the clinical profile including various clinico-pathological manifestations, demographic profile, spectrum of clinical presentation, etiology, pathological and radiological investigations of patients with abdominal TB.Methods: A total of 30 patients with abdominal TB were analysed. All diagnosed patients received anti-tubercular treatment and were followed up at 1 to 3 months of ATT.Results: The disease was common in both sexes. Most common symptoms were abdominal pain (93%), weight loss (83%), anorexia (68%). Background: Tuberculosis is a major health hazard in India. Because of its diverse manifestation ,difcult diagnosis, widespread complication ,prolonged morbidity and increased mortality the study of this disease becomes even more important .The objectives of the study were to study age and sex distribution of abdominal tuberculosis ,to study the association of abdominal tuberculosis with pulmonary tuberculosis ,various clinical presentation of abdominal tuberculosis ,to study surgical management of abdominal tuberculosis. Methods: The study was carried out in 30 patients of abdominal tuberculosis of diferent age group and sex admitted in department of surgery , Smt SCL hospital , Saraspur , Ahmedabad during period of April 2018 to march 2019.This is the prospective study in which patients are present with acute abdomen like intestinal obstruction ,peritonitis ,RIF lump taken ,further investigated and laparotomies were performed and AKT started after proven by biopsy reports. RESULTS: In my study most common age group afected is between 2 nd to 4 th decade and gender wise male are more afected then female., Most common symptom of presentation is abdominal pain mostly at RIF and periumbilical region associated with anorexia and vomiting and abdominal distension .Out of 30 patients 19 patients had pulmonary tuberculosis and patient had completed course of AKT. CONCLUSION: In all 30 patients present as acute abdomen. Patients present with peritonitis were explored in emergency and other patients initially conservatively managed after all radiological investigations on patients were explored ,Biopsy reports were collected and after confrmation of tuberculosis AKT (ISONIAZID,RIFAMPICIN,PYRAZINEMIDE,ETHAMBUTOL and STRPTOMYCIN,LEVOFLOXACIN.) started .Good clinic pathological workup in patients of abdominal tuberculosis results in earlier diagnosis and timely management of this curable disease.

A high index of suspicion is an important factor in early diagnosis. Abdominal involvement may occur in the GI tract, peritoneum, lymph nodes or solid viscera. Early diagnosis and initiation of anti-TB therapy and surgical treatment are essential to prevent morbidity and mortality. A retrospective study was conducted on 60 patients with abdominal TB from January 2016 to May 2017 in Basaveshwar Teaching and General Hospital, Kalaburagi. Clinical presentation of abdominal tuberculosis in Indian scenario: A retrospective study

Tuberculosis (TB) is global public health emergency with 24% of all cases being reported from India. TB of the abdomen is often vague and non-specific in presentation and a great mimic of other diseases. With the objective of characterizing the demographic, clinical and treatment features of abdominal TB, all patients, without an age restriction with diagnosis of abdominal TB in the years of 2011 and 2012 were included. Median age was found to be 35. Abdominal pain (68.9%), fever (44.4%), weight loss (44.4%) were most frequent symptoms in the study group. The highest proportion of patients had peritoneal TB (46.3%). We found that 69% of the patients had hyponatremia at their first presentation. The study showed male predominance and showed no relation with HIV co-infection. Background: Current study aims to determine the frequency of abdominal tuberculosis and to evaluate the current surgical treatment strategies and itsoutcome. Methods: This prospective observational case series study was conducted in Medical Teaching Institute (MTI) surgical department of the Lady Reading Hospital Peshawar from January 2015 to January 2016. In this study a total of 40 patients of abdominal tuberculosis were admitted in surgical department through emergency and outpatient department. Background: The objective of this study is to evaluate diagnosed cases of abdominal tuberculosis (TB) in terms of incidence, manifestation in different age groups, clinical presentations, treatment received (conservative or surgical), lesions found intraoperatively, and its outcome on follow-ups. This condition is regarded as a great mimicker of other abdominal pathology. A high index of suspicion is an important factor in early diagnosis. Abdominal involvement may occur in the gastrointestinal tract, peritoneum, lymphnodes or solid viscera. Various investigative methods have been used to aid in the diagnosis of abdominal tuberculosis. Early diagnosis and initiation of antituberculous therapy and surgical treatment are essential to prevent morbidity and mortality. Most of the patients respond very well to standard antitubercular therapy and surgery is required in some cases. Imaging plays an important role in diagnosis of abdominal tuberculosis because early recognition of this condition is important. Abdominal tuberculosis constitutes a chief public health setback, presents a diagnostic challenge requiring a high index of clinical suspicion. Young age at presentation, delayed presentation, poverty & high morbidity & mortality are among the hallmarks of the disease in this region. These challenges need to be addressed in order to deliver optimal care for these patients. Early diagnosis, early antituberculous therapy & surgical treatment of the related complications are important for survival.

Prevalence of Intestinal Tuberculosis in Cases of Acute Abdomen at VMMC Safdarjung Hospital, New Delhi

Objective: To determine the prevalence of Intestinal Tuberculosis in cases of Acute abdomen. Design & Duration: A randomized prospective study from August 2019 to July 2020. Setting: Department of Surgery, VMMC & Safdarjung Hospital, New Delhi. Patients: A total number of 200 patients with Acute Abdomen, who presented as intestinal obstruction or peritonitis. Methodology: Detailed data of each patient including presentation, operative findings, procedure performed, post operative outcome and histopathology was entered on a specially designed proforma, compiled and analyzed. Results: Among the 200 patients with acute abdomen, 32 (16%) had Intestinal Tuberculosis on the basis of operative findings and histopatholoigcal reports.
