Photodynamic diagnostics of nonmuscle invasive bladder cancer

The review is devoted to the application of photodynamic diagnosis of non-muscle invasive bladder cancer. The recent data on the mechanism of action of photosensitizers, the method of performing fluorescence cystoscopy are presented, and the results of clinical studies of the application of photodynamic diagnostics in practical medicine are presented. It has been shown that photodynamic diagnostics significantly increases the efficiency of detecting bladder cancer in comparison with standard cystoscopy. The application of this method is especially valuable in cases of carcinoma in situ and multifocal growth of urothelial tumors. Improvement in diagnostics makes it possible to increase the radicality of surgical treatment and to increase the duration of the relapse-free period.

Photodynamic therapy of intradermal metastatic breast cancer (literature review)

В последние годы во всем мире наблюдается рост заболеваемости раком молочной железы (РМЖ), причем в 20% случаев при РМЖ происходит развитие внутрикожных метастазов. Возможности хирургического и лучевого лечения внутрикожных метастазов РМЖ достаточно ограничены, а эффективность полихимиотерапии с применением стандартных схем не превышает 22-27%, при этом срок ремиссии, как правило, составляет лишь 2-3 мес. Фотодинамическая терапия (ФДТ) является перспективным методом лечения внутрикожных метастазов РМЖ. Опыт применения ФДТ при данной нозологии достаточно ограничен, но полученные результаты демонстрируют его высокую эффективность и безопасность. Так, несколько российских исследований посвящены оценке эффективности ФДТ внутрикожных метастазов РМЖ с фотосенсибилизатором хлоринового ряда фотолон. По данным авторов, лечебный эффект был достигнут у 85-97% пациенток (доля пациенток с полным и частичным эффектом составляла 73-85%). Исследования эффективности ФДТ у пациенток с такой же нозологией с использованием фотосенсибилизатора фотосенс демонстрируют несколько меньшую эффективность -лечебный эффект был достигнут в 81,8% наблюдений, при этом доля пациенток с полным и частичным эффектом составляла только около 50%. За рубежом проведен ряд исследований на моделях метастазирующего РМЖ с использованием новых фотосенсибилизаторов (например, синопорфирина натрия) и новых комбинированных схем ФДТ (например, адъювантная ФДТ с 5-фторурацилом или капецитабином). Полученные результаты демонстрируют перспективность новых подходов: ФДТ с синопорфирином натрия ингибировала рост как самой опухоли, так и ее метастазов; применение адъювантных схем привело к повышению дифференцировки опухолевых клеток у животной модели, прекращению роста опухоли, а также метастатических очагов.

Palliative surgical treatment of malignant tumours in the pancreatobiliary area complicated by obstructive jaundice using local and systemic photodynamic therapy

The paper analyses the tactics of managing patients with malignant neoplasms in the pancreatobiliary area complicated by obstructive jaundice in two nosologically and clinically comparable groups of patients. The aim of the research. To evaluate the effectiveness of complex palliative treatment with the use of photodynamic therapy in patients with malignant neoplasms in the pancreatobiliary area complicated by obstructive jaundice. Efficiency of the Combined Photodynamic Therapy with Cisplatin in the Treatment of B16 Melanoma in C57BL/6 Mice

Введение. Фотодинамическая терапия — один из современных методов лечения опухолевых и предопухолевых заболеваний. Данный метод имеет ряд ограничений, одним из направлений преодоления которых может быть его комбинирование с химиотерапевтическими противоопухолевыми средствами. Цель исследования. Introduction. The paper provides a review of domestic and foreign literature on photodynamic therapy, which is mainly used by oncologists in the treatment of adult patients and is little known to pediatric surgeons. The aim of this work is to describe the history of the formation, principles and mechanisms of photodynamic therapy, the main groups of photo sensitizers, areas of clinical application and prospects for wider use in pediatric surgery. Effects of continuous wave and fractionated diode laser on human fibroblast cancer and dermal normal cells by zinc phthalocyanine in photodynamic therapy: A comparative study

In this experimental study, cancer and normal cells behavior during an in vitro photodynamic therapy (PDT) under exposure of continuous wave (CW) and fractionated mode of laser with different irradiation power and time intervals was compared and investigated. At the first, human fibroblast cancer cell line (SW 872) and human dermal normal (HFFF2) cell line were incubated with different concentrations of zinc phthalocyanine (ZnPc), as a PDT drug. The cells, then, were irradiated with a 675nm diode laser and the cell viability was evaluated using MTT assay. Under optimized conditions, the viability of the cancer cells was eventually reduced to 3.23% and 13.17%, and that of normal cells was decreased to 20.83% and 36.23% using CW and fractionated diode lasers, respectively. Photodynamic Therapy (PDT) is a minimally invasive approach that has shown promising results in management of oral, head and neck lesions. PDT can be used alone or in combination with other conventional treatments (surgery, chemotherapy, radiotherapy). Oral Lichen Planus (OLP) is a mucosal and cutaneous chronic disease characterized by an autoimmune insult of basal keratinocytes. We aim to evaluate the feasibility of topical toluidine blue-mediated PDT for the treatment of oral cavity multifocal homogeneous white lesions by oral lichen planus without dysplastic features.

Photodynamic therapy of penile leukoplakia: a case report

Purpose: to describe a clinical case of successful application of photodynamic therapy (PDT) in a patient with leukoplakia of the glans penis skin.Material and methods. For PDT, chlorin-type photosensitizer “Photolon” was used. The photosensitizer (lyophilized powder) was diluted in 200.0 ml of 0.9 % sodium chloride solution, administered intravenously at dosage 1.5 mg/kg of body weight. After 2.5 hours, intravenous laser blood irradiation was performed with “Lakhta-Milon” laser device (output power – 100 mW, exposure – 30 minutes). Before PDT, fluorescence diagnostics in ultraviolet light with wavelength 395–405 nm was performed. PDT technique was single-field irradiation with output power 1800 mW (1.8 W), irradiation energy density – 150 J/cm2. Power density of laser light was 200 mW/cm2. Exposure time – 14 min.Results.
