Knowledge and Utilization of Oral Rehydration Therapy (ORT) among Mothers in Enugu, Nigeria: A Health Facility Survey

Objectives A health-facility survey was done to determine the knowledge and utilization of oral rehydration therapy (ORT) among 400 mothers resident in Enugu. Methods A systematic sample from a list of mothers attending the immunization clinic of the Institute of Child Health (ICH) of the University of Nigeria Teaching Hospital, Enugu was interviewed using a pre-tested structured questionnaire. Results A pre-survey awareness of ORT was noted among 97.5% of the mothers. The beneficial effects of ORT were seen by 57.3% of the mothers as replenishment of lost energy and 37.5% as fluid and salt replacement. Although 77.0% of the respondents could not describe correctly the preparation of salt-sugar solution (SSS)/oral rehydration salt solution (ORS), a high ORT utilization rate of 74.6% was recorded. ORT as a modality of managing childhood diarrhoea remained totally acceptable to 89.5% of the mothers. Conclusion Many Mothers, although they accept and use ORT/SSS, could not accurately describe the methods of preparation of ORS/SSS. It is recommended that maternal perception of the role of ORT should be utilized by health personnel to promote its use, while routine training of caregivers on the preparation of SSS/ORS should be intensified during health education sessions. Knowledge and Attitude of Mothers Regarding Oral Rehydration Solution in Sulaimani

Background: Dehydration resulting from diarrhea is a significant cause of death of young children in developing countries. Oral rehydration solution (ORS) is useful to replace fluid and electrolyte loss. Objective: The main objective is to assess the knowledge, attitude and practice of mothers towards the use of oral rehydration solution in the treatment of acute diarrhea in children. Patients and Methods: This observational descriptive study was conducted at the pediatric medical emergency department of the Children's hospital and the institute of child health in Sulaimani city for a period of eight months from July 2012 till March 2013. The criteria of inclusion were mothers of children with diarrhea. A total of 200 mothers fulfilling the inclusion criteria were enrolled over the study period. Mothers were interviewed and information collected. A pre designed questionnaire was filled for each case by the investigator which included a detailed history highlighting their demographic data, presenting complaints , treatment given at home, mothers knowledge about ORS and drugs, maternal education, water sources, and socioeconomic history was taken and recorded carefully. All the data entered in SPSS and results were analyzed in percentages. Results: A total 200 mothers who visited the Pediatric Teaching Hospital in Sulaimani were interviewed. The median age of children group was 2 years, and minimum was (4 months)and maximum age was 8 years, of these 25(12.5%) were below 1 year, and 160 cases (80%) were between 1 to 5 years, and 15 cases (7.5%) more than 5 years. The most frequent age group was between 1 to 5 years, male were 101 (50.5%) and female were 99 female (49.5%), 58(29%) of mothers education were illiterate, and 105 (52.5%) of mothers education was low, and 37(18.5%) of mothers education was high. Among all the patients 172(86%) were from Sulaimani and 28(14%) from outside Sulaimani. Among all the patients the source of water used was well 103(51.5%), and 63(31.5%) was piped, and 34(17%) was tanks. Out of 200 mothers 199 (99.5%) heard about ORS and 1(0.5%) did not hear about it. Of these mothers 183(91.5%) used ORS. and 17(8.5%) did not use it. Among the mothers that usedORS, 7(3.5%) used it by themselves and 42(21%) were advised by family member and 85(42.5%) were advised by physician and 49(24.5%) were advised by health worker. The way of giving ORS wasby cup and spoonin 139(69.5%), and 44(22%) gaveORS by bottle. Of them 109(54.5%) amount of ORS given adequate amount and 74(37%) gave inadequately. And of them 107(53.5%) prepared ORS correctly and 76(38%) prepared the solution incorrectly. Among all mothers; 172(86%) used drugs and 28(14%) did not use drugs. Among those who use drugs 21(10.5%) used it by themselves and 151(75.5%) got advise by family physician. Conclusion: Although most of the mothers knew about ORS and used it at times of need, many had inadequate knowledge on the importance and appropriate use of ORS.

Knowledge and use of oral rehydration solution in the home management of diarrhea among mothers of under fives in Jos, Plateau State

Background: Diarrheal disease is one of the leading causes of pediatric morbidity and mortality worldwide, with an estimated 1.8 million childhood deaths annually, accounting for 17% of childhood deaths. Death can occur following severe dehydration if body fluids and electrolytes are not replenished either through the use of Oral Rehydration Salt (ORS) solution or intravenous fluid. Hence, this study was conducted to determine the level of use of ORS among mothers of under fives. Methodology: This was a cross sectional study conducted among 158 mothers of under-fives attending the FHC between November 2014 and April 2015 using quantitative method of data collection. Epi info statistical software version 3.5.4 was used for data analysis with a 95% confidence interval used in this study and a p-value of ≤ 0.05 was considered statistically significant. Result: The mean age of the respondents in this study was 29 ±5 years. Good knowledge of ORS was found among 94 (59.5%) of the respondent with a corresponding 96 (60.8%) engaging in good use of ORS in the management of diarrhoea disease for their under five children. Use of ORS in management of diarrhoea disease was significantly influenced by level of knowledge of ORS, age of the mothers, availability of prep-packed ORS and tertiary education. Conclusion: This study revealed a fairly high level of knowledge and practice of ORT in the home management of diarrhoea amongst mothers of under–fives.

India is constantly making efforts to improve its health indicators. But the gender ratio is still the matter of concern. Although the sex ratio at birth has slightly improved from 892 in 2000-02 to 909 in 2011-13.Still we need to make genuine contribution in improving the same. Another priority area of intervention for the Government is to reduce deaths and disabilities faced by women as a result of unsafe abortions. Unsafe abortion is the third largest cause of maternal deaths in India and contributes to eight percent of all maternal deaths despite being preventable. Each nearly 10 women die on account of unsafe abortions. The reasons for the unsafe abortion are multiple and deep rooted.

A STUDY TO ASSESS THE KNOWLEDGE AND PRACTICE OF MOTHERS ABOUT ORAL REHYDRATION THERAPY IN FANDA AND BHAISAKHEDI VILLAGES OF BHOPAL

ABSTRACT: BACKGROUND: Diarrhoea is a major cause of illness and death among young children in India. Most cases of childhood diarrhea can be treated at home by increased fluid intake and continued feeding during diarrheal episodes. In accordance with WHO guidelines, the Government of India promotes oral rehydration therapy, including the use of oral rehydration salt (ORS) packets or a recommended home-made solution (RHS) made from salt, sugar, and water. METHOD: This was a cross sectional study conducted among mothers having children less than 5 years to know about their knowledge attitude and practices related to ORS usage during diarrheal diseases. RESULTS: Most of the mothers i.e. 52% got information about ORS from Anganwadi, 54% of the mothers told that ORS usage during diarrhea decreases water loss, 52% of the mothers were not having knowledge about homemade ORS, 77% of the mothers directly consulted doctors when their children had diarrheal episodes hence they did not use ORS as a first remedy for management. CONCLUSION: This knowledge gap needs to be improved by the measures that are directed towards disseminating the knowledge, changing the negative attitudes and encouraging the better practices about usage of ORS in the mothers for the effective management of diarhhoea. Knowledge and Practice of Oral Rehydration Solution in Diarrhoea of the Mothers Attending ORT Corner of Rajshahi Medical College Hospital

This cross sectional type of descriptive study was carried out among the mothers, who attended ORT corner of Rajshahi Medical College Hospital for the treatment of diarrhoea of their children to assess their knowledge and practice of oral rehydration solution in diarrhoea. The sample size was 385, which was selected purposively. Data were collected from the respondents by face to face interview according to a partially structured questionnaire. Data were analysed by using SPSS software programme and interpretations were done using appropriate statistical tests like Chi Square Test. Ethical issues were considered throughout the study. Majority (62.5%) of the mothers were in the age group of 21 – 30 years and most (71%) of them were housewives. Most (95.12%) of the mothers knew the preparation of ORS correctly but only 42.82% of them used ORS properly during diarrhoea of their children. Is knowledge and practice of oral rehydration therapy suboptimal? Assessment at Federal Medical Center, Asaba, South-South Nigeria

Background: Oral rehydration therapy (ORT) is a core component of the childhood survival strategies to reduce child mortality and morbidity due to diarrhoeal disease with its fatal dehydrating complication of acute kidney injury. This strategy is indispensable to the attainment of the International Society of Nephrology's (ISN) aim to eliminate preventable deaths from acute kidney injury (AKI) by year 2025. Diarrheal disease is the second most common cause of morbidity and mortality in children at our centre. This interventional study assessed the knowledge and the practice of ORT among caregivers, educated and trained them on the management of diarrhoeal diseases and practice of ORT. Oral rehydration salt (ORS) and zinc tablets were also distributed. Methods: An interviewer-administered questionnaire was used following informed consent. Consented care givers of wards attendees of FMC Asaba were assessed on their knowledge and their practice of ORT, educated on childhood diarrheal diseases, trained on practice of ORT and had ORS and zinc tablets distributed to them. These assessments were done on the spot and 6 weeks after the health talk, from July 2015 to December 2015. Results: There were 266 respondents and 231 were mothers. The immediate impact of the health talk on the knowledge and the practice of ORT was laudable, p≤0.0001 and educational attainment of the respondent influenced the immediate post health talk knowledge of ORT, p=0.009. The age of the respondent predicted the long term impact of health talk on practice of ORT, p=0.020. Conclusions: Knowledge and practice of ORT are not optimal but can be improved by regular education.

KNOWLEDGE AND ACCEPTANCE OF ORAL REHYDRATION THERAPY AMONG MOTHERS IN THE MANAGEMENT OF DIARRHEA IN CHILDREN 0-5 YEARS

Oral rehydration therapy (ORT) is the preferred treatment for fluid and electrolyte losses due to diarrhea in children with mild to moderate dehydration. This study aimed to assess the knowledge, and acceptance of ORT in the management of diarrhea in children 0-5 years at Seven Say Adventist Hospital (SDAH, Ife). A structured questionnaire was adopted for this study design and data was collected using a self-structured questionnaire and equally from files analyzed manually. The total population of this study was 80 participants; 44 (55%) of the respondents had heard of ORT before this study, while 36 (45%) had not. Out of 48 people that used ORT at home, 10 died half (16, 50 %) of the children that were not given ORT at home died. The major reasons for non-acceptance of ORT were because most (50%) had no knowledge of ORT while other reasons includes unavailability of mother and financial difficulties. There was a noticeable difference in the morbidity . those that used ORT at home denoted lesser hospital stay, fewer cases of severe malnutrition and weight loss. With the above results Intensive ORT training should be organized for community/village mothers and health works. Thus, ORS should be primarily used for oral rehydration while appropriate home available fluids might be used as supplement or only when ORS is not available.

Use of Commercially Available Oral Rehydration Solutions in Lima, Peru

Caregivers' practices concerning oral rehydration of young children during diarrheal illness were investigated in a periurban community of low socioeconomic level in Lima, Peru. Data of 330 caregivers of children aged 6-36 months were analyzed; 72.7% of all caregivers would give commercially available oral rehydration solutions (ORSs). However, only 58.6% of those caregivers with children that had experienced diarrhea during the previous week stated that they had used commercially available ORSs, a significantly lower percentage. The main reason for not using commercially available ORSs was that caregivers did not know about them. Of all recipes caregivers provided for homemade ORS, none contained the recommended concentrations of sugar and salt. Educating caregivers about availability, benefits, and use of commercially available ORSs as well as correct preparation of homemade ORS is urgently needed.
