Lipoma in the parotid gland: a clinical case presentation

Lipoma of the parotid gland is a rare benign mesenchymal tumor. It is seldom considered in the differential diagnosis of the salivary glands pathology. Lipoma represents approximately 1% of the parotid gland tumors. We report a clinical case of a 54-year-old male patient presented with 5-year history of painless, slowgrowing mass in the left parotid gland. The management of choice was surgical removal of the lateral lobe of the parotid gland with facial nerve preservation. No tumor recurrence, facial nerve palsy or pain was noted postoperatively.
Lipoma in Superficial and Deep Lobes of Parotid Gland: A Case Report

Introduction: Lipomas among the most common benign neoplasms and rarely observed in parotid glands. We present a new case of lipoma in the superficial and deep lobes of the right parotid gland. Case report: A 52-year-old woman with painless and progressive inflation in the right preauricular region was referred to us. Computed Tomography scanning showed a hypodense area 5.2×4 cm in dimension in the right parotid gland region, and the facial nerve was fully exposed. The patient underwent parotidectomy, during which extensive removal of the mass was done. The pathology report cited a yellow-colored fatty tissue mass, 5×4×2 in dimension. In the microscopic report, lipoma of the parotid gland was seen.Conclusion: Determination of the exact tumor location is very important in the surgical approach in such cases. To our knowledge, this case seems to be an extremely rare case of lipoma in the superficial and deep lobes of the parotid. [GMJ. 2012;1(2):88-90]

Lipoma is a benign, rare, mesenchymal tumor found in the head and neck region, especially in the parotid gland. It thus requires a careful and precise examination to establish a diagnosis. A surgical procedure of the parotid gland is challenging due to the associated risk factor of facial nerve injury. We report a rare case of head and neck region lipoma between the superficial and deep lobe of the parotid gland. A 44-year-old female patient was presented with the chief complaint of a painless lump on the left front ear to the left cheek for about 1 year. There were no complaints of tooth pain before the lump appeared, and there were no lumps in other regions. A fine-needle aspiration biopsy, ultrasonography, and magnetic resonance imaging were all performed to establish the preoperative diagnosis and to plan the correct surgical approach. Lipomas are common benign soft tissue neoplasms, however they are found seldom within the deep lobe of the parotid gland. Surgical intervention in parotid mass is difficult owing to the proximity of the facial nerve and so information of anatomy and surgical procedures are essential. A sixty-four-year-old female presented with a left cheek mass from one-year period that was painless and didn’t grow in size with chewing. A high-frequency sonography visualized a hypoechoic mass measuring 3.2× 2.6 × 2 cm in left parotid region with well-defined margins and heterogeneity for the presence of fibrous septae. Magnetic resonance imaging confirmed, on T1and T2-weighted images, a well-described hyper intense lesion within the left parotid gland that was hypo intense on STIR images. A firstclass needle aspiration cytology (FNAC) became conclusive for the presence of “homogeneous proliferation of mature adipocytes,” compatible with lipoma. A case of lipoma of the parotid gland in a 61-year-old woman is reported. Lipoma is one of the most common mesenchymal soft tissue tumours, but rarely presents in the parotid gland. There is no reliable method of preoperative discriminatory diagnosis, because this lesion exhibits no unique clinical features. Lipomas are common mesenchymal tumours; however, their occurrence in the head and neck region is relatively rare and account for only 15%-25% of all lipomas. Lipomas of the parotid region are even more uncommon, representing only about 1.0%-1.3% of all parotid tumours, and are often not included in the initial differential diagnosis of benign parotid tumours. Usually, a soft, non-tender, well-circumscribed tumour of the parotid gland in an elderly man is considered a Warthin's tumour unless proven otherwise by histopathological examination. A rare case of a large lipoma of the parotid gland, which had been present for 15 years, is reported in a 63-year old male patient. The literature on clinical presentation, method of investigations and management of parotid lipoma is briefly reviewed.

We report 3 cases of parotid lipomas whose diagnosis was suspected by tomodensitometry and confirmed by histology. Treatment was a superficial parotidectomy for two patients and enucleation for one patient. Parotid lipomas are located mainly in the superficial lobe. The diagnosis is based on imaging, but only histology can confirm. The treatment of superficial lipomas is based on superficial lobe parotidectomy but sometimes enucleation could be enough. In all cases, the recurrence rate is very low.

Lipoma is a common benign tumor affecting the soft tissues arising in every location where fat is normally present. About 15% of lipomas occur in head and neck region. The lipoma of parotid gland is a rare entity, comprising 0.6%-4.4% of all parotid tumors. The etiology remains unclear. The case described a lipoma of the superficial lobe of the parotid gland in a 46-year-old male.

Lipomas are common benign soft tissue neoplasm, but they seldom occur in head and neck regions compared to other sites. Fine needle aspiration cytology (FNAC) accompanied with Computed tomography (CT) scan and Magnetic Resonance Imaging (MRI) determines its diagnosis preoperatively. In symptomatic cases, surgery remains the treatment of choice but requires an experienced surgeon because of possible damage to the facial nerve and its branches. The authors have discussed a rare case of lipoma, involving the deep and superficial lobe of parotid of a 42-year-old female patient with emphasis on its characteristics, preoperative diagnostic methods and its surgery. This case report shows MRI's to be potent diagnostic tool for Lipoma of the parotid, by means of which a surgical procedure could be planned while maintaining function and aesthetics.

Lipomas are common benign soft tissue tumours, they are found in every body part where the fat is normally present but they are rarely found in the head and neck region. There are few reports of parotid lipoma and till date only one case report of bilobe lipoma has been reported earlier. They compromise 0.6%-4.4% of all parotid tumour. A 60 year old male presented with a large swelling in left parotid region extending from left pre-auricular region to submandibular region from 6 years. Examination of the facial nerve was normal. Ultrasound of the region suggested of lipoma. Fine needle aspiration cytology was done which supported the ultrasounds diagnosis. Patient underwent tumour excision with facial nerve preservation. Lipoma of size 10 x 8 cm which is the largest reported bilobe parotid lipoma reported till date. Literature review has suggested lipomas of parotid region are rare hence should be kept in one of the differential diagnosis. Surgical intervention is challenging because of the probability of injuring the facial nerve, thus knowledge of the anatomy and meticulous surgical technique is essential.
