Pengembalian berkas rekam medis rawat jalan di Rumah Sakit Tingkat III Baladhika Husada Jember masih mengalami keterlambatan. Pada triwulan pertama tahun 2019 total berkas rekam medis rawat jalan yang terlambat sebanyak 25,32% berkas dari 25,32% berkas. Rekam medis sangat berguna dalam memberi pelayanan kepada pasien karena berisikan bukti-bukti tertulis dalam bentuk kertas atas pelayanan tindakan yang diberi oleh dokter dan tenaga kesehatan lainnya. Oleh karena itu, rekam medis harus disimpan pada tempat penyimpanan yang tepat agar dapat tersedia kembali dengan waktu sesuai dengan Standar Pelayanan Minimal yaitu ≤ 10 menit. Penelitian ini dilaksanakan di RSUD Kota Depok, Jawa Barat. Penelitian ini bertujuan untuk mengetahui Lama Waktu ketersediaan rekam medis rawat jalan di RSUD Kota Depok. Penelitian ini menggunakan metode deskriptif dengan pendekatan kuantitatif dengan melakukan wawancara dan observasi. Populasi pada penelitian ini adalah kunjungan pasien rawat jalan selama 1 bulan pada bulan Maret. Sampel pada penelitian ini sebanyak 106 permintaan rekam medis rawat jalan dengan menggunakan teknik incidental sampling. Hasil penelitian didapatkan ≤ 10 menit sebesar 51 (48%), dan pengambilan tidak depat waktu rekam medis >10 menit sebesar 55 (52%) dengan dibagi ketegori waktu pengambilan 11-20 menit sebesar 25 (23,6%), 21-30 menit sebesar 8 (7,6%), dan > 30 menit sebesar 22 (20,8%). Dengan rata-rata waktu ketersediaan 17,42 menit. Hal ini menunjukan keterlambatan dalam ketersediaan rekam medis rawat jalan yang harusnya ≤ 10 menit.

Tinjauan Waktu Penyediaan Berkas Rekam Medik Rawat Jalan Rumah Sakit X di Tangerang Selatan

Penelitian ini melihat gambaran waktu penyediaan berkas rekam medis dan faktor-faktor penyebab keterlambatan dalam penyediaannya di rumah sakit X Tangerang Selatan.Penelitian ini bersifat deskriptif dengan pendekatan kualitatif. Data yang digunakan adalah 242 berkas rekam medis rawat jalan yang diukur waktu penyediaannya. Berkas tersebut diperoleh selama 6 hari yaitu mulai hari Senin-Sabtu dari pukul 09.30-12.00. Data dikelompokkan menjadi kelompok waktu penyediaan ≤ 10 menit dan kelompok waktu penyediaan >10 menit. Kemudian dilakukan wawancara mendalam dengan kepala Instalasi Rekam Medik untuk mendapatkan gambaran penyebab penyediaan berkas rekam medis >10 menit.Ditemukan sebanyak 144 atau 60% dari berkas rekam medis dengan waktu penyediaan ≤ 10 menit sedangkan 98 atau 40% berkas rekam medis memerlukan waktu penyediaan > 10 menit. Faktor-faktor penyebab waktu penyediaan berkas rekam medis >10 menit adalah kesalahan dalam penyimpanan berkas rekam medis, Tidak berfungsinya mesin pencetak transaksi dan tidak tersedianya tracer.Hasil penelitian ini dapat menjadi bahan evaluasi bagi Rumah Sakit X untuk memperbaiki pelayanan penyediaan rekam medis pasien rawat jalan.

This study was conducted to determine the time to provide patient medical records for long outpatient services at the Adhyaksa General Hospital. Its specific objectives are to identify policies and standard operating procedures (SOP) for providing old patient medical records, to know the standard time needed for providing outpatient medical records, and to identify factors that cause inaccuracies in the provision of medical records. From the results of research conducted in the medical record unit, it was found that there was no policy and SOP for the provision of old patient medical records which included the standard time for providing medical records for long-patient outpatients. The factors that cause inaccuracy in the provision time of outpatient medical records are the absence of medical records on the shelf, storage space has not been centralized and the medical records have not returned from the outpatient and inpatient units.
