Effect of Management Structure on Quality Service Delivery of Mission Hospitals in Meru County, Kenya

Purpose: The purpose of the study was to examine the relationship between management structure and quality service delivery in mission hospitals in Meru County Methodology: The study adopted a descriptive research design and primary data was collected from respondents while secondary data was retrieved from published sources. The author used questionnaires as the data collection instrument. The target population was one hundred twenty-eight (128) respondents from the mission hospitals in Meru County of Kenya. A pilot study was conducted at Chuka mission hospital in Tharaka Nithi County to establish the reliability of the research instruments. Effectiveness of Hospital Management Committees in Busia County, Kenya

The Kenya Ministry of Health restructured the health sector in 2009 with the goal of promoting and improving the health status of all citizens. It established the Hospital Management Services Fund to be managed at the facility level by the Hospital Management Committees (HMCs). Since the establishment of the HMCs at the county level following the devolution of health services in 2016, no evaluation has been done to assess their performance in Busia County. We assessed the criteria, selection process, orientation, induction, and the performance of the HMCs in Busia and considered their implications for Kenya and similar contexts in Africa. Data were collected by purposive sampling of all HMC members in six level 4 and one level 5 hospitals through Focus Group Discussions and key informant interviews. Documents collected both at the facility and county levels were analysed and used appropriately. All seven hospitals evaluated did not fully comply with the National Guidelines. Three (43%) of the hospitals nominated persons with educational levels below the required O-level certification. Another 3 (43%) contravened the Constitution of Kenya (2010) by excluding persons with disabilities and minorities. The study identified systemic commissions and omissions in the recruitment process, leading to anxiety and frustrations by members of HMCs, staff and the catchment population of the respective hospitals. In conclusion, the nominated HMC members have no legitimacy to assume their roles and responsibilities as they have not been gazetted, appointed, oriented, and facilitated.

Effect of Consultative Management on the Performance of Kenyatta National Hospital in Nairobi City County, Kenya

The country's current economic and social situations have had a negative impact on the delivery of health care services. Employee empowerment is one of the key values, which strives to have the right people with the necessary talents and attitude, as well as financial sustainability. The Kenyatta National Hospital, however, continues to suffer from inefficiency, low service quality, and consumer dissatisfaction. Therefore, this study sought to investigate the influence on consultative management on performance of Kenyatta National Hospital in Nairobi City County, Kenya. This study employed a descriptive research design. Kenyatta National Hospital will serve as the unit of analysis, while 911 Kenyatta National Hospital personnel from ten departments served as the unit of observation. A stratified sample approach was used to divide respondents into departments to ensure that all instances are represented. A simple random selection process was used to choose the respondents. A total of 278 persons, or 30.5 percent of the total population, was sampled. To collect primary data, a questionnaire was employed. There was 28 Kenyatta National Hospital respondents in a pilot study who were not included in the final study. The researcher conducted a content validity test to check that the surveys are written in simple English and that the questions are understandable. The Cronbach alpha test was used to determine reliability. To evaluate quantitative data, descriptive statistics such as mean and standard deviation were used. The data that was examined was presented using tables and figures. Inferential statistics were also done using correlation analysis and multiple regression analysis because there were various explanatory variables in this study. The study found that consultative management had a positive and significant influence on Kenyatta National Hospital's performance in Nairobi City County, Kenya. The study concluded that the hospital had a very effective of implementation of consultative management strategy which improved their performance through openness/respect, sharing of information and suggestion plans. The study recommended that the hospital should be aware of its business environment. This entails conducting a critical analysis of both the internal and external business environments.

Effective management of hospitals and other healthcare institutions all emphasize a broadly accepted mission of measured performance, continuous quality improvement and responsiveness to the needs of patients, physicians, employees and community stakeholders. It is against this fact that this study aimed to investigate the factors hindering health professionals on management of Moi Teaching and Referral Hospital (MTRH), Eldoret in Kenya. The study adopted the contingency theory to guide the study and employed a case study research design. The study targeted 43 doctors who were in management positions. Performances of public health facilities in Kenya have been underperforming for several years. This made the role of managing public health facilities in the country to be transferred to county governments from the year 2013. The purpose of this study will be to examine the influence of resource provision on performance of selected hospitals in Kenya. The target population for the study involved 340 employees selected from various departments in health facilities in Ainamoi Sub County, Kericho County Kenya. Management Practices and Performance of Public hospitals in Uganda

The Daily Monitor publications ran serialized articles showing the awful state of government hospitals across the country. While the Ministry of Health insists that the problem is not as bad as it is depicted, the level of service delivery in public hospitals has come under serious public scrutiny espousing the cause for concern about policy, practice and research. There should be glaring gaps in management practices as a possible explanation. In this study, we investigated impact of management decision making, structure, processes, communication and management style on hospital performance. The study has emphasized good management as the determinant of better performance of hospitals in the Ugandan context. Findings of this study challenges policy makers to strengthen management processes in addition to mobilizing financial, human and capital resources for hospitals. The study extends the debate on application of management theory with practice in the health sector in the Ugandan context.

Service delivery in Kenyan district hospitals – what can we learn from literature on mid-level managers

Background: There is a growing emphasis on the need to tackle inadequate human resources for health (HRH) as an essential part of strengthening health systems; but the focus is mostly on macro-level issues, such as training, recruitment, skill mix and distribution. Few attempts have been made to understand the capability of health workers, their motivation and other structural and organizational aspects of systems that influence workforce performance. We have examined literature on the roles of mid-level managers to help us understand how they might influence service delivery quality in Kenyan hospitals. In the Kenyan hospital settings, these are roles that head of departments who are also clinical or nursing service providers might play.

Human Resource Management Practices and Performance of Devolved Healthcare Facilities in Nairobi City County, Kenya

The overall objective of this study was to investigate the effect of HRMP on the performance of delegated health facilities in Nairobi County, Kenya. The study was based on three theories, namely the theory of goal-setting, the theory of human capital and the theory of resource-based vision. Researchers world over have concentrated their efforts on challenges affecting the public health sector. Many of their findings show that these problems include high vacancy rates and attrition. They highlight that these results from poor conditions of work and funding. This is the same case in Kenya. The purpose of this study was to investigate the role of performance management system on service delivery. It was to specifically examine the role of performance target setting on service delivery in public hospitals in Kenya. The study was conducted at Kakamega county General Hospital, Kenya. The researcher adopted a descriptive case study design. Three hundred (300) permanent health workers were targeted. Stratified random sampling technique was used to categorize the population into respective stratum consisting of Doctors, Nurses, and Clinical Officers, Laboratory Technologists /Technicians, dentists and Pharmacists. Simple random sampling and purposive sampling techniques were employed to pick the respondents who participated in the study. A closed-ended questionnaire was the main instrument used to collect data. Instrument Validity and reliability was found through SPSS to be to be 0.82 and 0.97 respectively.

Staff Competence, Management Support and Service Delivery in Public Health Sector: A Case Study of Mbagathi District Hospital, Nairobi City County

The study investigated the influence of staff competences and management support on the quality of service delivery in public hospitals. The target population for this study comprised of 273 employees and 10,000 patients in Mbagathi district hospitals within Nairobi County. Simple random sampling was used to select 398 employees and patients while census was done on the 8 management staff. This study used primary data, both quantitative and qualitative. Primary data was obtained from the original sources using a structured questionnaire and captured through a 5-point type Likert scale.
