Evaluation of sodium cromoglycate B.P. (INTAL) in the prophylaxis of bronchial asthma in Asians

A double-blind cross-over trial was undertaken to determine the prophylactic value of sodium cromoglycate B.P. in patients of Asian stock with bronchial asthma. Statistical analysis of the results showed the drug was superior to placebo in reducing:-(a) the number of acute asthmatic attacks; (b) the bronchodilator requirement The majority of the patients at the end of trial period preferred the drug to the placebo. Slight improvement in the Forced Expiratory Volume ill one second was obtained at the end of the drug period.

Thirty-nine asthmatic subjects, aged 5-50 and each with a history of exercise-induced asthma, were classified according to their skin response to prick tests using nineteen common antigens. Ten had negative skin tests, four responded only to D. farinae and twenty-five had multiple positive responses. Each patient then carried out three exercise tests on a treadmill, each test on a separate day. A control test was followed, in random order, by an exercise test after administration of disodium cromoglycate or of a placebo.

The bronchial hyperreactivity, measured as the responsiveness to histamine, was studied in 14 utopic patients before, during, and after 4 wk of treatment with sodium eromoglycate (SIX) and placebo in a double-blind, randomized, crossover study. The patients were not exposed to relevant allergens during the study. The variations in provocation concentrutions corresponding to 20% decrease in FEV, (PCJ were small during both placebo and active drug treatment. After SCG treatment, PC2, increased (less responsiveness) in nine of the 14 patients, especial!\ in those with low PCzO values. The difterence between placebo and uctive drug treutment was not statistically sign&ant. Although SCG has a mediator-inhibiting effect, this study gave no support for the assumption that inhibition of mediator release leads to a reduction of the bronchial hyperreactivity in atopic asthmatic subjects who are not exposed to relevant allergen.,. Nedocromil sodium and sodium cromoglycate in patients aged over 50 years with asthma

We conducted a double-blind parallel-group study to compare the efficacy of inhaled nedocromil sodium and inhaled sodium cromogylcate in patients aged 50 years or over with asthma. Seventy-seven patients were randomized, 38 to receive nedocromil sodium (4 mg q.d.s.) and 39 to receive sodium cromoglycate (10 mg q.d.s.) over a 16-week period. Eight patients withdrew from the study. Both patient groups showed a reduction in inhaled bronchodilator usage during the treatment period. No statistically significant differences were seen in diary card PEF recordings or symptom scores. Both clinic PEF and FEV1 were significantly greater (P < 0.05) in the sodium cromoglycate group at week 12. Patients considered the treatment to be very or moderately effective in 59% of the nedocromil sodium group and 50% of the sodium cromoglycate group. Sodium cromoglycate and carbachol-induced bronchoprovocation in asthmatic children

Ten children with mild bronchial asthma underwent bronchial challenge with carbachol on three consecutive days; using a double-blind technique, they were given inhaled saline placebo on the first day and placebo or sodium cromoglycate (SCG) on the second and third days 15 min prior to the bronchial provocation test with carbachol. We did not find any significant difference between the baseline and placebo values, but we did obtain a statistically significant difference when comparing baseline with post-SCG and placebo with post-SCG values. These results demonstrate a protective effect of SCG on carbachol-induced bronchospasm in asthmatic children. Pediatr Pulmonol 1987;3:161-165.
