Effects of nonnutritive sucking stimulation on the age at introduction of oral feeding in preterm newborns

OBJECTIVE: To analyze the effects of nonnutritive sucking stimulation (NNS) on the age of introduction of oral feedings in preterm newborns (PTNB). METHODS: 95 PTNB with gestational age (GA) at birth < 33 weeks, born at the Nursery Annex to the Maternity of the Clinical Hospital of the University of São Paulo School of Medicine, São Paulo, Brazil, were studied. They were distributed into the following groups: Group 1 (G1), control group, without a specific incentive for oral feedings; Group 2 (G2), stimulation of NNS with an orthodontic pacifier (Nuk®) for premature infants; and Group 3 (G3), stimulation of NNS with a gloved finger. RESULTS: The GA varied from 26 to 32.7 weeks (30.5±1.6), the corrected GA at the moment of enrollment in the study varied from 27.4 to 33 weeks (31.6 ±1.3), and the average birthweight was 1,390g, without statistic differences among groups. The sensory-motor-oral stimulation effects on nutritive sucking of preterm infants during bottle feeding

Objetivo: verificar o desempenho da sucção nutritiva, na mamadeira, em recém-nascidos pré-termo submetidos à estimulação sensório-motora-oral. Métodos: pesquisa do tipo ensaio clínico controlado de intervenção. 20 recém-nascidos pré-termo foram distribuídos em grupo estimulado e grupo controle. Estes grupos foram submetidos à avaliação fonoaudiológica em dois diferentes momentos: na liberação da alimentação por via oral; e quando atingiam alimentação plena por via oral num período de 24 horas. As avaliações foram filmadas, e os resultados foram analisados pela pesquisadora. Para análise dos dados utilizou-se o Teste exato de Fisher e o Teste "T" de Student do software estatístico STATA 10.0. Considerou-se p < 0,05. Resultados: recém-nascidos do grupo estimulado, na segunda avaliação, apresentaram resultados com diferença estatística significante quanto a força de sucção forte (p=0,003), presença dos três reflexos adaptativos (p=0,001), coordenação da sucção/deglutição/ respiração (p=0,003), no tempo total de sucções (p=0,01) e na frequência de sucção (p<0,01). O número total de sucções apresentou diferença estatística significante (p<0,05) entre as avaliações do grupo controle. Quanto ao ritmo e a relação entre o volume de leite prescrito e ingerido não apresentou resultados com diferença estatística significante. Conclusão: a estimulação sensório-motora oral parece favorecer o desempenho nas funções de sucção nutritiva de recém-nascidos pré-termo.

[Enteral feeding in infants born preterm: update of the National Consensus]

In the present update of the recommendation of Enteral Feeding in Preterm Infants from the Portuguese Neonatal Society, special attention is paid to nutritional requirements of preterm infant, particular aspects of enteral nutrition during the stay in the neonatal unit, including when to initiate enteral feeding, how to advance, mode of feeding, specificities of preterm human milk, its fortification, nutrition in special conditions and nutrition after discharge. Information on the composition of special formulas, fortifier and protein supplement for human milk commercialized in Portugal is included.

Introduction of inappropriate complementary feeding in the first year of life and associated factors in children with low socioeconomic status

The study aimed to identify factors associated with the introduction of inappropriate complementary feeding in the first year of life in children living in municipalities (counties) with low socioeconomic statusl. This was a cross-sectional multicenter study in 1,567 children 12 to 59 months of age in 48 municipalities participating in the Brazil Without Poverty plan in the South of Brazil. A structured questionnaire was applied to the children's parents to obtain socio-demographic information and the age at which inappropriate complementary foods were introduced for the first time in complementary feeding. Prevalence of introduction of sugar before four months of age was 35.5% (n = 497; 95%CI: 33.1-38.0). The prevalence rates for the introduction of cookies/crackers, creamy yogurt, and jelly before six months of age were 20.4% (n = 287; 95%CI: 18.3-22.3), 24.8% (n = 349; 95%CI: 22.4-27.1), and 13.8% (n = 192; 95%CI: 12.0-15.7), respectively. Early weaning: implications to oral motor development

Objective: this article aims at reviewing the relationship between early weaning and its consequences to oral motor development, focusing on the consequences to occlusion, breathing and children's oral motor aspects. Sources: a literature review based on Medline database from the early 60's up to 2001 was performed taking into consideration the following topics: pediatrics, dentistry and speech language pathology. Summary of the findings: based on this review of literature, we could verified that early weaning may lead to a proper oral motor development rupture, which may cause negative consequences to swallowing, breathing and speaking activities as well as malocclusion, oral breathing and oral motor disorders. Conclusions: in addition to several benefits of breastfeeding, it contributes to a proper oral motor development and also avoids speech-language disorders, regarding oral motor system.

Complementary feeding practices to children during their first year of life

This study aimed to investigate complementary feeding practices during children's first year of life in Botucatu, SP, Brazil. Practices were described according to the age range and the breastfeeding (BF) practice. Data were collected during a multi-vaccination campaign through the interview of 1,238 individuals who accompanied children younger than one year old being vaccinated. Differences associated with the BF situation were identified by the chi-square test and Fisher's exact test. The early introduction of complementary food was observed, which led to the low frequency of exclusive breastfeeding (36.9% <4 months). Children younger than 4 months old consumed tea (30.7%); children between 4 and 6 months old consumed fruits (54.1%), soups (39.9%) and home-made food (19.2%).
