In my headcanon, Mheetu is Nala's half-brother. When they were young, Scar and Sarafina were betrothed, but the arrangement eventually fell apart. Once Scar took over as king, he needed a queen and, more importantly, an heir. Sarafina simply seemed like the most convenient choice due to their prior betrothal. Sarafina begrudgingly agreed after Scar threatened Nala's safety.

So, Mheetu was born from the unhappy arrangement between Scar and Sarafina. For a while, it seemed that Mheetu would be the perfect heir, and Scar doted on him. However, it quickly became apparent that Mheetu was too gentle and kind-hearted to follow in Scar's footsteps. The influence of Sarafina, Sarabi, and the other lionesses prevented him from going down the same dark path as his father. However, this ultimately proved to be the young cub's undoing. Realizing Mheetu was no longer useful to his cause, Scar made the would-be prince disappear. There were suspicions, of course, but no body was ever found...
