Cackle's a bit complex. While inside the confines of Pop's jack in the box, he's harmless. He just wants to sleep, and does so as long as he's inside. When Pop decides it's time for him to wake up, things change. As Pop cranks the box, the chain on Cackle's collar is wound around it, pulling Cackle toward a wall of pins. As he is pierced by the many pins, he is violently awoken, creating the monster you see before you. Always silent thanks to his mouth being stitched closed, he is only able to grunt and growl. In this state, he is prone to violent and unpredictable bouts of rage. When angered enough, he will transform into his second form.
