The deaths of the seven workers of the World Central Kitchen organization in an Israeli drone strike on Monday was caused by a series of mistakes, including a lack of coordination between the different levels of the Israel Defense Forces, the mistaken identification of a man believed to be armed in a targeted vehicle, and the firing of two additional drone missiles against army protocol.
