Check the requirements for each college to which you are applying. Some will only require the guidance counselor recommendation, but others will require two additional recommendations from teachers. It is preferable that those teachers taught you in an academic subject in 11th or 12th grade.
I always suggest reaching out at least a month before the deadline. Be polite and respectful. Start by thanking them for their support in the past and then explain why you value their recommendation. It’s important to make the request feel natural and appreciative, not like an obligation.
Don't be afraid to ask the teacher directly if they feel like they are able to write a positive recommendation, and give them an out if that answer is no. Perhaps say, "I'd love you to write one of my college recommendations if you feel like you could write me a positive one".
Help your recommender help you. Give them all the materials they need, a resume, a brief summary of your goals and even a list of programs you're applying to.
Teachers and guidance counselors will have a general idea of your school related clubs, sports, and activities.
