Care-at-home programs have proven to benefit patients and providers, however, scaling these models often brings significant challenges for logistics and technical support. However, a team dedicated to over-the-threshold support and education can reduce the operational burden on providers and improve the patient experience, paving the way for scaling care-at-home across the continuum. In just four years, Atrium Health's care-at-home program has served over 9,400 patients, saving more than 33,000 brick-and-mortar bed days. The care program boasts a readmission rate of only 0.8%, much lower than that of their brick-and-mortar facilities, with a mortality rate below 1%. Their integrated care-at-home approach has not only exceeded expectations but also provided exceptional patient experiences.
