HealthBegins is a national mission-driven design & implementation partner helping Medicaid-serving clients move upstream and advance health equity. Founded by physicians and public health experts, HealthBegins believes that healthcare is better when it routinely works inside and outside its walls to improve the ‘upstream’ social and structural conditions that make people sick, especially people who belong to historically marginalized communities. HealthBegins has emerged as a leading network for healthcare-based “Upstreamists”, professionals with the responsibility and skills to improve the social and structural drivers of health equity through practice and system transformation. HealthBegins supports this transformation through strategic consulting, training, education and technical assistance solutions, and tools including the Upstream Capability Assessment℠, the Upstream Quality Improvement℠ toolkit, and Community Health Detailing℠. Inspired by a popular book and TED Talk by its founder Dr. Rishi Manchanda, HealthBegins has activated thousands of providers, educators, payers and community partners around the world who improve care and the social and structural factors that make people sick in the first place. These results from the CMMI/CMS Accountable Health Communites model are remarkable. The model, which screened over 1.1 million people for health-related social needs, reduced total health care expenditures between 3-6% and improved quality of care and health equity among Medicaid and Medicare beneficiaries. HealthBegins was proud to partner with Centers for Medicare & Medicaid Services and Mathematica to support all the frontline leaders and organizations who participated in this groundbreaking model. Social Needs Investment Lab | Catalyzing Impactful Investments for Social Change socialneedsinvestmentlab.com - Our HealthBegins team presenting on best practices for rural providers with other Collaborative Planning and Implementation (CPI) facilitators at the meeting of Providing Access and Transforming Health (PATH) initiative— a five-year, $1.85 billion initiative that helps community-based organizations, hospitals, county agencies, and tribes participate in the Medi-Cal delivery system.
