Our Mission is to Improve the Health and Well-Being of the Communities we serve, through the sharing of vital information between Trusted Partners. SacValley MedShare (SVMS) is a Qualified Health Information Organization (QHIO) established in 2013 as a 501(c)3 not‐for‐profit organization to serve the needs of our patients, local populations, and the providers involved in their medical care. At SacValley MedShare we are transforming data from movable to usable is the alchemy of insights, turning raw potential into the gold of informed decision-making, ultimately forging a path towards health equity and improved outcomes.

- SacValley MedShare has partnered with the California WIC Association to enhance the management of WIC referrals through our health information exchange (HIE). This groundbreaking collaboration is set to improve outcomes for women and children throughout Northern California. California WIC association provides free healthy foods, breastfeeding support, nutrition education and referrals to health and community resources for your family. With this program, we aim to: Streamline the WIC referral process using advanced health information exchange technology. Increase enrollment among eligible participants. Develop a scalable model that can be replicated in other regions. We’re excited to see how this initiative will automate workflows, enable real-time data sharing, and improve care coordination. These advancements will help ensure timely access to critical WIC services and lead to better health outcomes. - California's implementation of the Data Exchange Framework (DxF) is a monumental step forward in modernizing healthcare. By creating a unified approach to health information exchange, the DxF ensures that patients and providers have access to the right information at the right time, ultimately improving outcomes and streamlining care delivery. At SACVALLEY MEDSHARE, we are proud to support the DxF's vision of seamless interoperability and equitable access to data. This framework is setting the stage for innovation, collaboration, and a future where health information exchange empowers both providers and communities. Together, we are building a connected healthcare system that serves everyone.
California is making impressive progress implementing the DxF! So far: 📝 98% of required health plans have signed 🏥83% of required General Acute Care Hospitals have signed 🧑⚕️83% of required Skilled Nursing Facilities have signed - Starting January 1, 2025, millions of Californians will have more financial support during critical family moments! Thanks to SB 951, workers earning up to ~ $60,000 annually will receive 90% of their regular income when they need to take Paid Family Leave or Disability Insurance. This means time to bond with a newborn, care for a loved one, or recover from illness—without the stress of losing income. #SB951 #PaidFamilyLeave #CaliforniaWorkers
