The NPMA-33 WDI inspection report is only used to report activity or damage from wood destroying insects that, for the purposes of this report, include termites, carpenter ants, carpenter bees, and reinfesting wood boring beetles.

This form is submitted for proposed (new) construction cases when prevention of subterranean termite infestation is specified by the builder or required by the lender, the architect, FHA or VA. This form is to be completed by the builder. This guarantee is issued by the builder to the buyer.
NPMA-99B
New Construction Subterranean Termite Soil Treatment Record
The HUD NPMA-99B form is a termite-related document. It is submitted for the informational purpose to the builder on new (proposed) construction when soil treatment for the prevention of subterranean termite infestation is specified by the builder, architect or required by the lender, FHA, or VA.
