Chronic pruritus is considered to be a common health problem in the elderly population, and negatively impacts the quality of life in the affected individuals. Senile pruritus can be a result of multiple conditions, including aging process (xerosis, immunologic changes), cutaneous conditions (seborrheic dermatitis, contact dermatitis, psoriasis, scabies), systemic conditions (chronic kidney disease, thyroid disfunction or malignancy), neuropathic and psychogenic causes. Senile pruritus is more like a diagnosis of exclusion, difficult to treat because polimedication has to be avoided in eldery patients. Senile pruritus and its impact on quality of life could foster comorbidities and patients must be provided with follow-up, support and education to enable better compliance and monitoring.

Material and methods: we studied patients with erysipelas hospitalized in the Dermatological Clinic between 2001 and 2010. Results: the investigated 140 cases represented 8% of all skin in-fections hospitalized. Distribution of annual admissions has shown a gradual reduction in the number of admissions in the second half of the decade. A higher frequency was found in the second half of the year. Most patients were from rural areas (55%). The female gender pre-dominated (69.3%).With few exceptions, the patients were adults averaging 59 years. The most common location was the lower leg (91.4%). Most clinical forms were mild and medi-um. Septic complications were absent. Recurrent erysipelas occurred in 5% of cases. Comorbidity included: chronic venous insufficiency (14.2%), hypertension (10%), obesity (8%), diabetes mellitus (5.7%) or chronic hepatitis (3.5%). Conclusions: the "benign" ap-pearance of our cases is contrary to the trend rate of severe cases of cutaneous streptococcal infections in some western areas. From comparing laboratory tests results we observed their frequent mismatch. The ESR and the increase in percentage of polymorphonuclear neutro-phils were more significant than the total number of leukocytes. Etiological therapy with penicillin G at a dose of 4 million UI/day was effective. The analysis allows the establish-ment of demographic and clinical features of erysipelas during the period and given territory.

Aim: to gather data regarding the incidence of syphilis over the last 5-6 years in North-eastern Romania. Material and methods: Retrospective study on the main forms of syphilis (primary syphilis, secondary syphilis, latent syphilis, congenital syphilis, neurosyphilis) which included a total of 304 patients admitted and treated to the Clinic of Dermatovenerology of the Iasi County “Sf. Spiridon” Emergency Hospital between 2013-2018. Results: The incidence of the different clinical forms of syphilis was as follows: primary syphilis 1%, secondary syphilis 16%, recent latent syphilis 37%, late latent syphilis 22%, early congenital syphilis 3%, late congenital syphilis 11%, results on agreement with literature data. Conclusions: Our study demonstrates a linear decrease in the incidence of syphilis over the last 5-6 years in Northeastern Romania, but it still ranks first among EU countries. Further research is needed to understand the social determinants that lead to the spread of sexually transmitted diseases in Romania.

R... more Psoriasis is a chronic inflammatory disease of the skin, relatively frequent in the population. Recent studies have shown that psoriasis can have a great impact on quality of life, even in patients with limited disease. Although the cause of this disease remains unknown, research suggests that psoriasis is a complex disease caused by the interaction of many genes, the immune system and environmental factors. In principle, any disease can coexist with psoriasis. Material and method: We conducted a retrospectiv study based on observation files of hospitalized patients with psoriasis admitted in the Dermatology Clinic Iaşi during January 2004 - December 2008. Results: Current data suggests that smoking and obesity increase the risk of developing psoriasis. Psoriasis may be an independent risk factor for developing diabetes, atherosclerosis and myocardial infarction. Therefore the patients with psoriasis die 3-4 years earlier than the rest of the population. Patients should be advised to stop excessive alcohol consumption and smoking. Conclusion: Further studies are needed to clarify the link between severity of skin lesions, treatments and the evolution of associated diseases.

Psoriasis is a common cause of disturbance of the nail morphology. Nail disorders was reported in 10-80% of patients with psoriasis and may be associated with all clinical forms of disease. It has been suggested that the nail dystrophy in patients with psoriasis produce the lose of the natural protective barrier and increase vulnerability to fungal infections. But most authors report that in patients with psoriasis the prevalence of onychomycosis is no higher than in the general population. We conducted an observational analytical retrospective study of case report forms of patients with nail psoriasis hospitalized in Iasi Dermatology Clinic, 2004-2008. 360 patients were hospitalized with psoriasis. Of these 32 patients had nail disorders.

It is in fact a form of acute myeloid leukemia notable for highly aggressive behavior with cutaneous, lymph node and bone marrow involvement. This entity is derived from plasmocytoid dendritic cells and has a predilection for extranodal sites, especially the skin. Elderly male patients are the most affected and the prognostic is poor. The first case was reported in 1994 and sice then, single cases and a few small series have been published. This article presents the case of a previously healthy 56-years-old man, who presented himself to a skin eruption consisting in multiple, large dermal ulcerated tumors, located on the trunk and scalp. The lesions were painless and grew in size rapidly. Physical examination was normal except for the skin lesions. Histological examination of a biopsy specimen and immunohistochemical studies (positive for next markers: CD4, CD 45, CD56, CD68, Ki 67) revealed the rare diagnostic-blastic plasmacytoid dendritic cell neoplasm.

Lesions develop in the pilosebaceous units, which are especially distributed on the cheeks, chin, forehead and back. Severe acne could have a chronic evolution and in majority of cases it is followed by post-inflamatory hyperpigmentation and unaesthetic scars. Although acne does not affect the general health, the disease has a major negative psycho-emotional impact caused by unaesthetic acne lesion on the face or the scars which are truly hard to treat. Actually, the management of acne represents a challenge for the doctors and from the perspective of self-medication which become a trend hard to deal with. Many patients because of desire to have spectacular results in a short time, apply without prescription medical treatments and skincare products. This approach often can lead to sensitization of the skin and also to low compliance with the dermatologist's medical treatment. Through this article we propose to highlight the importance of psychological counseling of our acne patients, as well to inform about the risks of self-medication.

The disease can develop during childhood, but it predominantly affects adults of both sexes. The triggering factors or those that favor the condition's progression are heterogeneous and difficult to fully identify. Alopecia has various clinical forms with development towards full recovery or with residual scars. Even though the clinical inflammatory process that affects the hair follicle is not life threatening or painful, alopecia has a strong negative echo at the psycho-emotional level, since hair is a key element of self image, of self esteem, of the socialization manner. Most alopecia patients have appropriated distorted patterns in relation to thoughts, emotions and behaviors connected to their self image and, in time, they can develop depressive or anxiety disorders induced by this unaesthetic matter. The changing of these patterns through psychological counseling along with dermatological treatment must represent a priority in the management strategy of the patient with alopecia.

Mental health is not a destination, but a process. It Is about "how you drive, not where you go".... more Mental health is not a destination, but a process. It Is about "how you drive, not where you go". In a broad sense, anorexia is an eating disorder, which consists in the voluntary decrease in food intake, which can reach most extreme cases with the cessation of feeding. In the context of this mental illness analysed by this material, it can be concluded that the implications are from small to most serious, even reaching catastrophes, irreversible trauma and even causing the death of the victim. This paper tries to highlight the medico-legal and social risks of this condition, even if it is a rare but constantly growing mental disorder due to the challenges that adolescents are currently going through. From the analysis of the practical cases briefly exposed in the chapter dedicated to this analysis, we have exemplified with two cases examined at European legislative level.

Understanding this concept is essential in order to improve patient care. Numerous quality of life assessment tools have been developed, some generic and some specific. DLQI (Dermatology Life Quality Index) is the most common tool of its kind in dermatology and it's 32/Bulletin of Integrative Psychiatry New Series March 2022 Year XXVIII No. 1 (92) easy to implement. We performed a study including 147 patients diagnosed with various skin disorders accompanied by mucosal involvement, admitted in "St. Spiridon" Emergency Clinical Hospital's Dermatovenereology Clinic during 01.01.2019-31.12.2020, aiming to measure the quality of life by applying the DLQI questionnaire and calculating its score (initial DLQI and DLQI in follow-up). As a result, the DLQI score showed a statistically significant improvement in follow-up (p <0.001). The proportion of patients with low scores (DLQI <10) increased from 31.3% to 51.7% and those with scores >20 (severe impairment of quality of life) decreased from 27.2% to 15%, with an ascending trend in male gender. Of these patients, 69.4% were diagnosed in the Dermatology Clinic. Regarding the diseases that influenced the evolution of the DLQI score, pemphigus (p=0.005), psoriasis (p=0.010) and syphilis (p=0.042) generated statistically significant improvements. The development of tools for quantifying and assessing the quality of life facilitates the identification of the level of physical and mental impairment of patients, allowing the establishment of a personalized therapy and facilitating the doctor-patient communication.

This chronic itch could be a presenting sign of malignancy; therefore, paraneoplastic pruritus has also been associated with neuroendocrine tumors (NETs). This article focuses on a patient presenting with chronic pruritus for the past 12 months and who received numerous treatment schemes with very poor clinical improvement, that presented in the hospital for worsening of the chronic pruritus associated with skin rash and significant weight loss (approximately 6 kg over a 2-month period). The laboratory tests showed iron deficiency anemia, eosinophilia and negative tumor markers. In order to investigate the hypoanabolic and anemic syndromes, upper gastrointestinal endoscopy and colonoscopy, which showed no lesions or tumors, were employed. Skin biopsy was performed and antihistaminic and local steroid treatment was initiated. The patient's status worsened within a week and the patient was started on systemic steroid treatment with poor results. Computer tomography was performed to identify any tumor(s) located either in the pelvis or abdomen. A lesion was found in the terminal ileum, identified as a hypervascularized associating bulky lymphadenopathy. The patient was transferred to the surgical ward where right hemicolectomy with manual ileotransverse anastomosis L-L was performed. The histopathological result confirmed NET G2. The patient clinically improved, the skin lesions resolved and the itchiness disappeared. The general status improved significantly. NET G2 diagnosing was possible due to the atypic paraneoplastic sign: chronic pruritus. This case study highlights the association between itch and malignancy and presents an atypical way of NET presentation when all tumor markers remain negative.

Once called “venereal diseases,” STDs (also known as sexually transmitted infections or STIs), are among the most common infectious diseases today in the USA. Although prevention and education efforts have curbed the rise of certain STDs in the USA, the diseases continue to affect sexually active people in the USA as well as other countries.

Thus, patients with any type of psoriasis require an assessment of the severity of the disease, the impact of the disease on physical, psychological and social condition, diagnosis of the existence of psoriatic arthritis or other comorbidities. Overall assessment of the patient, assessment of the affected body surface, nail damage, affected areas with high impact and difficult to treat, or any systemic disorder such as fever, malaise, which are common in unstable forms of psoriasis such as erythroderma or generalized pustular psoriasis [1]. Psoriasis is a chronic, multisystemic disease with many comorbidities, which is often difficult to treat, and some cases may be refractory to treatment. Total eradication of plaques is difficult to achieve, and the remission time is short. Recurrence is inevitable and is often preceded by poor adherence to topical therapy. Forms of gouttate psoriasis may resolve spontaneously or may progress to chronic plaque psoriasis. Erythrodermic psoriasis and generalized ARTICLE INFO ABSTRACT Background: The epidemiological profile of psoriasis may show the great psychoemotional impact through hard-to-treat body locations. Methods: We performed a retrospective study of 527 patients with psoriasis in the Dermatological Clinic of the Emergency County Clinical Hospital "Saint Spiridonov", Iasi in order to establish the frequency of major forms of psoriasis and describe the characteristics such as age group and special impairment distribution. Results: The frequency of scalp psoriasis was significantly higher in patients aged ≤18 years (51.61% compared to 32.73% aged 19-49 years, Z=2.01, p=0.02; compared to 31, 66% aged between 50-69 years, Z=2.22, p=0.01 or compared to 29.17% aged ≥ 70 years, Z=2.18, p=0.01). Facial psoriasis was significantly higher in patients aged ≤18 years (12.9% compared to 4.24% aged 19-49 years, Z=1.92, p=0.027, compared to 1.16 % aged between 19-49 years, Z=4.02, p <0.00001 and compared to 1.39% aged ≥ 70 years, Z=2.49, p=0.006). It was also observed that the frequency of genital area involvement was significantly high (38.1%, Z = 1.77, p = 0.038). Conclusion: The burden of disease may be due to high frequency of hard-totreat body locations such as the facial and scalp regions at early age and genital area involvement has a negative impact on their quality of life.

Recently, interest has grown regarding the application of ozonated oil for vascular leg ulcer treatment. The efficacy of an innovative spray formulation of ozonated oil and !-bisabolol combination in the topical treatment of chronic venous leg ulcers was evaluated compared with standard epithelialization cream. DESIGN: A randomized controlled trial was conducted. SETTING: A total of 29 patients older than 18 years with chronic venous leg ulcers for less than 2 years' duration were assessed. PATIENTS: Patients were randomized to receive daily application of both ozonated oil and !-bisabolol or the control cream (vitamin A, vitamin E, talc, and zinc oxide) for 30 days. MAIN OUTCOME MEASURES: Patients were evaluated on 4 different visits: at days 0, 7, 14, and 30. At each visit, the wound surfaces were measured. Wound area ratio and the speed of ulcer healing were calculated. MAIN RESULTS: At the end of treatment, the proportion of patients with complete ulcer healing was higher with ozonated oil and !-bisabolol formulation (25% vs 0%). Furthermore, the changes in ulcer surface area were significant for ozonated oil and !-bisabolol formulation only (P G .05), in particular, observing a significant and progressive reduction of the wound surface by 34%, 59%, and 73%, after 7, 14, and 30 days of treatment, respectively. CONCLUSION: The innovative spray formulation of ozonated oil and !-bisabolol combination shows promise as an important new therapeutic option in the adjuvant treatment of venous ulcers.
