Aims: The aims of this study were to assess the capability of DTI of uterine zones in relation to the menstrual cycle and ascertain the normal apparent diffusion coefficient and fractional anisotropy values at 3T magnetic resonance imaging (MRI). Study Design: Prospective clinical study. Methods: A total of 13 young reproductive and 12 postmenopausal healthy volunteers were included in the study. MRI examination included sagittal T2-weighted and singleshot echo planar imaging DTI obtained under free breathing. Fractional anisotropy (FA) values of the endometrium, junctional zone, and myometrium were determined. Results: The median (minimum-maximum) FA of the endometrium, myometrium, and junctional zone of the reproductive group were 0.31 (0.260-0.465), 0.42 (0.302-0.664), and 0.58 (0.420-0.745), respectively, in the proliferative phase and 0.26 (0.180-0.413), 0.48 (0.357-0.656), and 0.59 (0.490-0.675)], respectively, in the secretory phase. In the postmenopausal group, the FA values of the endometrium, myometrium, and junctional zone were 0.275 (0.136-0.425), 0.255 (0.191-0.553), and 0.27 (0.129-0.397), respectively. Apparent diffusion coefficient (ADC) values of the endometrium, myometrium, and junctional zone of the reproductive group were 1.25±0.254 (0.970-1.463), 1.67 (1.213-1.854), and 1.23 (0.853-1.301), respectively, in the proliferative phase and 1.32±0.283 (1.165-1.706), 1.55 (1.360-1.791), and 1.17 (1.163-1.705), respectively, in the secretory phase. In the postmenopausal group, the ADC values of the endometrium, myometrium, and junctional zone were measured as 1.100±0.192 (0.850-1.302), 1.14 (0.864-1.283), and 1.09 (0.912-1.291). The FA values of the endometrium and myometrium were lower in the secretory phase of the reproductive group, while ADC values were higher. However, both the FA and ADC values were lower in the postmenopausal group. Conclusion: The present study showed that uterine DTI is feasible when used quantitatively. While FA values tend to decrease, ADC values increase significantly in all zones in the secretory phase except the junctional zone. Zonal FA and ADC values of postmenopausal women are lower in comparison to those in young women.

Furthermore, it can affect sciatic nerve as a rare presentation causing cyclic sciatica, chronic pelvic pain, paresis, and paresthesia. Early diagnosis and treatment is very important to minimize the nerve damage caused by endometrial focus on sciatic nerve. Diagnosis of sciatica due to endometriosis can be made by magnetic resonance neurography (MRN), thereby identifying abnormalities of the sciatic nerve in patients with unexplained sciatica. MRN is a sensitive tool for evaluating both the lumbosacral plexus and sciatic nerve. We described a 38-year-old female patient with cyclic sciatica for 4 years. She had an endometrial focus compressing sciatic nerve at left sciatic foramen level that was diagnosed by MRN. In this case report, we aimed to underline the importance of appropriate diagnostic tools in the patients with extraspinal sciatica.

Study Design: Prospective clinical study. Methods: A total of 13 young reproductive and 12 postmenopausal healthy volunteers were included in the study. MRI examination included sagittal T2-weighted and singleshot echo planar imaging DTI obtained under free breathing. Fractional anisotropy (FA) values of the endometrium, junctional zone, and myometrium were determined. Results: The median (minimum-maximum) FA of the endometrium, myometrium, and junctional zone of the reproductive group were 0.31 (0.260-0.465), 0.42 (0.302-0.664), and 0.58 (0.420-0.745), respectively, in the proliferative phase and 0.26 (0.180-0.413), 0.48 (0.357-0.656), and 0.59 (0.490-0.675)], respectively, in the secretory phase. In the postmenopausal group, the FA values of the endometrium, myometrium, and junctional zone were 0.275 (0.136-0.425), 0.255 (0.191-0.553), and 0.27 (0.129-0.397), respectively. Apparent diffusion coefficient (ADC) values of the endometrium, myometrium, and junctional zone of the reproductive group were 1.25±0.254 (0.970-1.463), 1.67 (1.213-1.854), and 1.23 (0.853-1.301), respectively, in the proliferative phase and 1.32±0.283 (1.165-1.706), 1.55 (1.360-1.791), and 1.17 (1.163-1.705), respectively, in the secretory phase. The apparent diffusion coefficient (ADC) values of the malignant and normal tissues were correlated. A total of 23 patients with 14 urinary bladder carcinomas and 9 prostate carcinomas, and 50 healthy controls with normal ultrasonographic urinary bladder and prostate gland imaging findings were enrolled in the study. The ADC values were reported as the mean +/- standard deviation. Student's t test was performed to compare the ADC values of the normal and pathological tissues. Diffusion-weighted imaging (DWI) was performed with b factors of 0, 500, and 1000 s/mm(2), and the ADC values of the normal tissues and lesions were calculated. C arpal tunnel syndrome (CTS) is the most common peripheral neuropathy of an upper extremity, and... more C arpal tunnel syndrome (CTS) is the most common peripheral neuropathy of an upper extremity, and it is caused by an entrapment of the median nerve at the level of the carpal tunnel (1). The carpal tunnel is formed by the carpal bones, which form a concave arch, and the transverse carpal ligament, which lies on the arch. The ligament is also fused to the pisiform, the hook of the hamate at the ulnar side, the tubercle of the navicular bone, the crest of the trapezium, and the radial styloid (2). CTS is a common disorder with an estimated 8% annual incidence rate, and an accurate diagnosis can be achieved by considering the patient's electrodiagnostic features and symptom characteristics. Electrodiagnostic testing is considered the single most accurate method of diagnosis, but a high rate of false negatives and positives are also well documented. If electrodiagnostic findings are not available, then the patient's characteristic symptoms and physical examination may provide the best diagnostic information (3). Fat-saturated, T2-weighted magnetic resonance imaging (MRI) can reveal morphological changes in CTS patients, such as nerve enlargement, nerve flattening, increased nerve signal intensity, and bowing of the flexor retinaculum at the level of the hamate hook. However, the sensitivity and specificity of MRI findings are low; therefore, conventional MRI may provide insufficient diagnostic data for clinically assessing CTS (1). Diffusion tensor imaging (DTI) is an advanced MRI technique that is used to measure the diffusion of water in tissue (4). In white matter, water diffusion is anisotropic due to its impermeable boundaries, and this anisotropy can be measured with DTI; therefore, its main clinical application is white-matter tract visualization, but recently, it has been shown that DTI can be useful for imaging non-neural tissue, such as peripheral nerves (5-8), muscle (9), and prostate (10). There are a few preliminary reports in the literature on the application of DTI for diagnosis of CTS, but they include a limited number of subjects. Nevertheless, these reports found decreased fractional anisotropy (FA) and increased radial diffusivity at the median nerve within the carpal tunnel (8, 11, 12). Here, we investigated DTI's diagnostic utility in a large group of CTS patients using two quantitative measures: FA and the apparent diffusion coefficient (ADC). In the postmenopausal group, the ADC values of the endometrium, myometrium, and junctional zone were measured as 1.100±0.192 (0.850-1.302), 1.14 (0.864-1.283), and 1.09 (0.912-1.291). The FA values of the endometrium and myometrium were lower in the secretory phase of the reproductive group, while ADC values were higher. However, both the FA and ADC values were lower in the postmenopausal group. Conclusion: The present study showed that uterine DTI is feasible when used quantitatively. While FA values tend to decrease, ADC values increase significantly in all zones in the secretory phase except the junctional zone. Zonal FA and ADC values of postmenopausal women are lower in comparison to those in young women.

Furthermore, it can affect sciatic nerve as a rare presentation causing cyclic sciatica, chronic pelvic pain, paresis, and paresthesia. Early diagnosis and treatment is very important to minimize the nerve damage caused by endometrial focus on sciatic nerve. Diagnosis of sciatica due to endometriosis can be made by magnetic resonance neurography (MRN), thereby identifying abnormalities of the sciatic nerve in patients with unexplained sciatica. MRN is a sensitive tool for evaluating both the lumbosacral plexus and sciatic nerve. We described a 38-year-old female patient with cyclic sciatica for 4 years. She had an endometrial focus compressing sciatic nerve at left sciatic foramen level that was diagnosed by MRN. In this case report, we aimed to underline the importance of appropriate diagnostic tools in the patients with extraspinal sciatica.

Aims: The aims of this study were to assess the capability of DTI of uterine zones in relation to the menstrual cycle and ascertain the normal apparent diffusion coefficient and fractional anisotropy values at 3T magnetic resonance imaging (MRI). Study Design: Prospective clinical study. Methods: A total of 13 young reproductive and 12 postmenopausal healthy volunteers were included in the study. MRI examination included sagittal T2-weighted and singleshot echo planar imaging DTI obtained under free breathing. Fractional anisotropy (FA) values of the endometrium, junctional zone, and myometrium were determined. Results: The median (minimum-maximum) FA of the endometrium, myometrium, and junctional zone of the reproductive group were 0.31 (0.260-0.465), 0.42 (0.302-0.664), and 0.58 (0.420-0.745), respectively, in the proliferative phase and 0.26 (0.180-0.413), 0.48 (0.357-0.656), and 0.59 (0.490-0.675)], respectively, in the secretory phase. In the postmenopausal group, the FA values of the endometrium, myometrium, and junctional zone were 0.275 (0.136-0.425), 0.255 (0.191-0.553), and 0.27 (0.129-0.397), respectively. Apparent diffusion coefficient (ADC) values of the endometrium, myometrium, and junctional zone of the reproductive group were 1.25±0.254 (0.970-1.463), 1.67 (1.213-1.854), and 1.23 (0.853-1.301), respectively, in the proliferative phase and 1.32±0.283 (1.165-1.706), 1.55 (1.360-1.791), and 1.17 (1.163-1.705), respectively, in the secretory phase. In the postmenopausal group, the ADC values of the endometrium, myometrium, and junctional zone were measured as 1.100±0.192 (0.850-1.302), 1.14 (0.864-1.283), and 1.09 (0.912-1.291). C arpal tunnel syndrome (CTS) is the most common peripheral neuropathy of an upper extremity, and... more C arpal tunnel syndrome (CTS) is the most common peripheral neuropathy of an upper extremity, and it is caused by an entrapment of the median nerve at the level of the carpal tunnel (1). The carpal tunnel is formed by the carpal bones, which form a concave arch, and the transverse carpal ligament, which lies on the arch. The ligament is also fused to the pisiform, the hook of the hamate at the ulnar side, the tubercle of the navicular bone, the crest of the trapezium, and the radial styloid (2). CTS is a common disorder with an estimated 8% annual incidence rate, and an accurate diagnosis can be achieved by considering the patient's electrodiagnostic features and symptom characteristics. Electrodiagnostic testing is considered the single most accurate method of diagnosis, but a high rate of false negatives and positives are also well documented. If electrodiagnostic findings are not available, then the patient's characteristic symptoms and physical examination may provide the best diagnostic information (3). Fat-saturated, T2-weighted magnetic resonance imaging (MRI) can reveal morphological changes in CTS patients, such as nerve enlargement, nerve flattening, increased nerve signal intensity, and bowing of the flexor retinaculum at the level of the hamate hook. However, the sensitivity and specificity of MRI findings are low; therefore, conventional MRI may provide insufficient diagnostic data for clinically assessing CTS (1). Diffusion tensor imaging (DTI) is an advanced MRI technique that is used to measure the diffusion of water in tissue (4). In white matter, water diffusion is anisotropic due to its impermeable boundaries, and this anisotropy can be measured with DTI; therefore, its main clinical application is white-matter tract visualization, but recently, it has been shown that DTI can be useful for imaging non-neural tissue, such as peripheral nerves (5-8), muscle (9), and prostate (10). There are a few preliminary reports in the literature on the application of DTI for diagnosis of CTS, but they include a limited number of subjects. Nevertheless, these reports found decreased fractional anisotropy (FA) and increased radial diffusivity at the median nerve within the carpal tunnel (8, 11, 12). Here, we investigated DTI's diagnostic utility in a large group of CTS patients using two quantitative measures: FA and the apparent diffusion coefficient (ADC). We also measured the correlation between CTS severity and FA.
