If anyone else feels like closing some of these issues, go ahead (no need to report back, I'll find it out later). My rather bureaucratic approach is just to avoid a possible trigger-happiness on my part :)
What you are doing is "formal process." It only becomes "bureaucracy" when you try to make others do the same. ;-)
