Demegestone, sold under the brand name Lutionex, is a progestin medication which was previously used to treat luteal insufficiency but is now no longer marketed. It is taken by mouth. Demegestone is a progestin, or a synthetic progestogen, and hence is an agonist of the progesterone receptor, the biological target of progestogens like progesterone. It has no androgenic activity. Demegestone was first described in 1966 and was introduced for medical use in France in 1974. It has only been marketed in France, and has since been discontinued in this country. Dopo somministrazione orale si osserva un picco di concentrazione plasmatica entro 2-3 ore. Il farmaco subisce trasformazione metabolica nel fegato, principalmente per idrossilazione in posizione 1, 2, 11 e 21; i suoi metaboliti sono stati isolati nella bile. È utilizzato per il trattamento di stati di insufficienza di progesterone, dismenorrea, irregolarità mestruali, sindrome premestruale, emorragie funzionali, menorragie dei fibromi uterini, endometriosi. Associato ad un estrogeno può essere impiegato come anticoncezionale. La dose abituale è di 0,5–2 mg al giorno dal 16º al 25º giorno del ciclo mestruale. Può causare alterazioni del ciclo mestruale, amenorrea, perdite ematiche uterine, seborrea, aggravamento dell'insufficienza venosa agli arti inferiori, disturbi gastrointestinali, ittero colestatico e prurito. Si sconsiglia l'uso in caso di insufficienza epatica e durante i primi tre mesi di gravidanza, nonché nei pazienti con anamnesi di tromboflebite o con manifestazioni in atto a carico del circolo venoso, soprattutto degli arti inferiori.
