Die Nabelschnurumschlingung (NSU) ist eine häufige Form der Nabelschnurkomplikation. Sie bezeichnet die ein- oder mehrfache Umschlingung eines Körperteils oder mehrerer Körperteile (Hals, Arme, Beine oder Rumpf) durch die Nabelschnur. Eine Nabelschnurumschlingung wird bei etwa 20 % der Geburten beobachtet. Eine lange Nabelschnur sowie eine zu große Fruchtwassermenge (Polyhydramnion) begünstigen die Entstehung einer Nabelschnurumschlingung. Bei einer schlaffen Umschlingung entsteht keine Gefährdung für die Sauerstoffversorgung des Feten. Eine straffe Umschlingung, insbesondere des Halses, kann jedoch zu einer erheblichen Verringerung der Durchblutung führen. In solchen Fällen ist meist im CTG ein Absinken der fetalen Herzfrequenz (Dezelerationen) zu registrieren. Die NSU ist nur selten Ursache für einen intrauterinen Fruchttod. In der Pränatalperiode kann man die Nabelschnurumschlingung mit der Doppler-Sonographie nachweisen. Eine diagnostische oder therapeutische Konsequenz leitet sich daraus jedoch nur bei CTG-Auffälligkeiten ab. (de)
A nuchal cord is when the umbilical cord becomes wrapped around the fetus's neck. Symptoms present in the baby shortly after birth from a prior nuchal cord may include duskiness of face, facial petechia, and bleeding in the whites of the eye. Complications can include meconium, respiratory distress, anemia, and stillbirth. Multiple wraps are associated with greater risk. The diagnosis may be suspected if there is a decrease in the baby's heart rate during delivery. Nuchal cords are typically checked for by running the finger over the baby's neck once the head has delivered. Ultrasound may pick up the condition before labor. If detected during delivery, management includes trying to unwrap the cord or if this is not possible clamping and cutting the cord. Delivery can typically take place as normal and outcomes are generally good. Rarely long term brain damage or cerebral palsy may occur. Nuchal cords occur in about a quarter of deliveries. The condition has been described at least as early as 300 BC by Hippocrates. (en)

Entre as possíveis complicações estão a presença de mecónio, stress respiratório, anemia e morte fetal. O risco é maior quanto maior o número de emaranhamentos do cordão. O diagnóstico pode ser suspeitado quando se verifica uma diminuição do ritmo cardíaco do bebé durante o parto. Assim que a cabeça passa pelo canal de parto, geralmente o obstetra passa um dedo à volta do pescoço para verificar se o cordão se encontra enrolado.
