Date: 14th December 1889 Colliery: Hetton Company: Hetton Coal Co., Ltd. Cause: (See description below) Lives Lost: 1 Category: No. 77 on the list. — This man was killed by being run over by a locomotive whilst engaged uncoupling a waggon. He was not using, as he should have done, a coupling pole. This is one of the class of accidents it was sought to minimise when I, by your directions, addressed the Circular, B. 2852/27, a copy of which was given in my report for the year 1888, to every colliery in this district. The coroner did not inform me when the inquest in this case was to be held, and consequently it was not attended by myself or either of my assistants.
