Non-pharmacological treatment is undeniably a significant part of painful shoulder syndrome treat... more Non-pharmacological treatment is undeniably a significant part of painful shoulder syndrome treatment. Medical exercises are its most important segment, according to the results of evidence-based medicine. Due to the fact that exercises are rarely used as monotherapy, we were particularly interested in the data regarding other non-pharmacological methods of treatment found in the Cochrane library and PEDro database (Physiotherapy Evidence Database). Simultaneously, we conducted a research study of painful shoulder syndrome treatment with conventional methods of physical therapy. The study included 157 patients, which allowed us to compare our preliminary results with data found in the previously mentioned databases. The majority of quality studies that can be found in the literature database involve the use of modern technology, such as extracorporeal shock wave therapy and high-intensity laser therapy, because they have proven to be very effective in the reduction of pain intensity and the increase of functional capacity of the painful shoulder. These methods of treatment have shown to be effective even in patients with calcific tendinitis, a more severe form of the disease, as well as in patients who had no calcifications. Conventional methods of treatment, often referred to as standard physical therapy (low-level laser therapy, electrotherapy, therapeutic ultrasound) have proven to be effective, but less effective compared to the aforementioned methods. The results of our research also corroborate the previously mentioned fact, thus confirming that the use of conventional methods of physical medicine (therapeutic ultrasound, diclofenac sonophoresis, interferential current therapy, low-level laser therapy, cryotherapy) has been successful in reducing pain intensity and increasing the functional capacity of the painful shoulder. Among the conventional methods of treatment, electrotherapy (interferential current therapy) turned out to be the most efficient one. Non-pharmacological treatment have to be an integral part of all painful shoulder syndrome treatment protocols, and in the future, the use of modern technology in addition to conventional methods of treatment should be encouraged.

Cell differentiation and different pathways of cell death were immunohistochemically analyzed in ... more Cell differentiation and different pathways of cell death were immunohistochemically analyzed in ovaries of six human embryos, 20 serous borderline tumors (SBT) and ovarian serous carcinomas (OSC) using markers for apoptosis (caspase-3, AIF, TUNEL) and stemness (Oct-4). In the 5-8-week ovaries, caspase-3 was absent in the ovarian surface epithelium (ose) and mildly positive in the ovarian stroma (os), AIF was expressed moderately, while Oct-4 expression gradually decreased during that period. Some ovarian cells expressed only caspase-3 or AIF together with TUNEL, while both caspase-3 and AIF were co-expressed in other ovarian cells. Mild expression of Oct-4 and caspase-3 characterized some cells of SBT, while their expression varied from mild to strong in OSC. AIF displayed mild to strong expression in ose of SBT and moderate to strong expression in OSC, while no expression of AIF was observed in os of both tumors. In the ose of both SBT and OSC, caspase-3 and AIF were co-expressed only occasionally, while AIF and Oct-4 were co-expressed strongly. Our study showed the presence of stemness cells and different pathways of cell death (caspase-3 and AIF-mediated) in the ovarian tissue during development and carcinogenesis, indicating the correlation between developmental plasticity in human embryonic ovaries and OSC.

Method: Critical collection and study of pertinent data on evolvement and present state of physical and rehabilitation medicine (PRM) education in Croatia. Results: Education in physical medicine and rehabilitation in Croatia was mainly focused on rheumatology rather than rehabilitation. In order to satisfy the new standards set for quality of rehabilitation medicine national curriculum reform was made for medical students, specialist and physiotherapists and new rehabilitation medicine training centers were established throughout the country. Conclusions: Academic setting such as PRM training center Split enables education for different health professionals at the same place and time, which provides opportunities for learning about competencies of other team members and development of future collaboration. Also, a uniform approach to education in rehabilitation medicine is provided for all health professionals. All of this sets a solid foundation for education of integrated rehabilitation team and achieving excellence in contemporary Croatian PRM. ä Implications for Rehabilitation In order to achieve high quality rehabilitation it is necessary to make education accessible to all rehabilitation team members. Implementation of rehabilitation principles in undergraduate education sets a good foundation for the development of postgraduate and specialty training in rehabilitation medicine. Academic setting such as physical and rehabilitation medicine training center Split provides a uniform approach to education in rehabilitation medicine for all health professionals.

Cilj: Utvrditi vrijednost vježbi u vodi kod osteoartritisa koljena i kuka u odnosu na vježbe na s... more Cilj: Utvrditi vrijednost vježbi u vodi kod osteoartritisa koljena i kuka u odnosu na vježbe na suhom kroz narativni pregled znanstvene literature dostupan u PubMed tražilici. Materijali i metode: Odabrana su tri reprezentativna istraživanja koja su zadovoljavala ulazne kriterije: osteoartritis koljena, osteoartritis kuka, vježbe u vodi, postojanje kontrolne skupine, broj ispitanika više od 30 po skupini, jasni prediktori ishoda: kondicija, opseg pokreta zgloba i/ili funkcionalni testovi. Kriterij isključivanja bila je postoperativna rehabilitacija. Rezultati: Sva istraživanja opisala su vježbe u vodi kao neštetne, uz statistički značajne pozitivne učinke na mjere ishoda (opseg pokreta, snaga, funkcionalni testovi), dok ni u jednog pacijenta nisu pridonijele pogoršanju stanja. Depression and anxiety are common complaints in patients with multiple sclerosis (MS). The study ... more Depression and anxiety are common complaints in patients with multiple sclerosis (MS). The study objective was to investigate the factor structure, internal consistency, and correlates of the Croatian version of the Hospital Anxiety and Depression Scale (HADS) in patients with MS. A total of 179 patients with MS and 999 controls were included in the online survey. All subjects completed the HADS and self-administered questionnaires capturing information of demographic, education level, disease-related variables, and the Multiple Sclerosis Impact Scale-29 (MSIS-29). Psychometric properties were examined by estimating the validity, reliability, and factor structure of the HADS in patients with MS. The two HADS subscales (anxiety and depression) had excellent internal consistencies (Cronbach’s α value 0.82–0.83), and factor analysis confirmed a two-factor structure. Prikazan je način operativnog zbrinjavanja i tijek rehabilitacije, 32-godišnjeg muškarca tijekom ... more Prikazan je način operativnog zbrinjavanja i tijek rehabilitacije, 32-godišnjeg muškarca tijekom 32 mjeseca, koji je u prometnoj nesreći zadobio prijelom nadlaktične kosti uz potpuni prekid radijalnog živca lijeve ruke kompliciranog dislokacijom metakarpalnih kostiju i trapezoidne kosti karpusa lijeve šake. Prijelom humerusa zbrinut je anterolateralnom osteosintezom, učinjena je neurorafija radijalisa te repozicija metakarpalnih kostiju. Rehabilitacija je započeta mjesec dana nakon prometne nesreće. Kako bi obnovili obrazac normalnog hvata šake pet mjeseci od ozljede primijenjena je dinamička ortoza za šaku. Oporavak živca praćen je EMG-om, no ni nakon 11 mjeseci nije utvrđen oporavka radijalisa što se klinički prezentiralo kao viseća šaka. U dogovoru s kirurgom pristupi se odstranjenu osteosintetskog materijala koji je zbog svog smještaja u području samog prolaska radijalnog živca mogao biti jedan od uzroka zbog kojeg nije došlo do oporavka živca. We report the first finding of bone metaplasia of left ventricular aneurysm following myocardial ... more We report the first finding of bone metaplasia of left ventricular aneurysm following myocardial infarction in young rat. Areas of cartilagineous and osseous metaplasia were found in the papillary muscle, chordae tendineae and subendocardial tissue seven weeks after experimental ligation of the left coronary artery. In this experiment, a breed of normotensive, two-month-old rats was used, meaning that all potential genetic and age-related factors for the development of metaplasia can be ruled out. Trichrome Mallory and hematoxylin-eosin staining showed an aneurysmatically changed heart wall with central parts of several trabeculae occupied by cartilaginous tissue, that in which some areas displayed signs of endochondral ossification: characteristic chondrocyte hypertrophy with small areas of calcified matrix and osteoid bone formation. The metaplastic areas expressed strong immunoreactivity with bone morphogenetic proteins 4 and 7. Cartilage tissue formation in the rat's heart was the result of oxygen diffusion from the oxygen-enriched blood inside the left ventricle and the local slowing of blood current inside the aneurysm, because the coronary artery supply in that area was cut off from ligation. Cartilage is known to be the only tissue that can survive in ischemic environments because it can obtain oxygen by diffusion from nearby blood. The presence of bone morphogenetic protein in the ischemic area directs myofibroblasts in the scar area to differentiate into chondrocytes and osteocytes, presenting as ossifying cartilage in the papillary muscle and subendocardial zone.

We report the first finding of bone metaplasia of left ventricular aneurysm following myocardial ... more We report the first finding of bone metaplasia of left ventricular aneurysm following myocardial infarction in young rat. Areas of cartilagineous and osseous metaplasia were found in the papillary muscle, chordae tendineae and subendocardial tissue seven weeks after experimental ligation of the left coronary artery. In this experiment, a breed of normotensive, two-month-old rats was used, meaning that all potential genetic and age-related factors for the development of metaplasia can be ruled out. Trichrome Mallory and hematoxylin-eosin staining showed an aneurysmatically changed heart wall with central parts of several trabeculae occupied by cartilaginous tissue, that in which some areas displayed signs of endochondral ossification: characteristic chondrocyte hypertrophy with small areas of calcified matrix and osteoid bone formation. The metaplastic areas expressed strong immunoreactivity with bone morphogenetic proteins 4 and 7. Cartilage tissue formation in the rat's heart was the result of oxygen diffusion from the oxygen-enriched blood inside the left ventricle and the local slowing of blood current inside the aneurysm, because the coronary artery supply in that area was cut off from ligation. Cartilage is known to be the only tissue that can survive in ischemic environments because it can obtain oxygen by diffusion from nearby blood. The presence of bone morphogenetic protein in the ischemic area directs myofibroblasts in the scar area to differentiate into chondrocytes and osteocytes, presenting as ossifying cartilage in the papillary muscle and subendocardial zone.
