Protein-energy malnutrition is among the comorbidities that most strongly affect the prognosis of... more Protein-energy malnutrition is among the comorbidities that most strongly affect the prognosis of patients with chronic kidney disease. Anorexia, defined as a loss of desire to eat, is one cause of such malnutrition. Tools that evaluate appetite and the correlation between appetite and nutritional parameters require further study. To evaluate the appetite status in patients from 2 hemodialysis clinics in Fortaleza, Brazil and the correlations between appetite (evaluated in the past week and in the past 4 weeks) and demographic, laboratory, and nutritional parameters. This was a cross-sectional study of patients aged ≥18 years who had undergone dialysis for >3 months. Appetite was evaluated using the first 3 questions of the Appetite and Diet Assessment Tool (ADAT) questionnaire, which evaluate the appetite status during the past week as well as 1 question from the Kidney Disease and Quality of Life™ Short Form that assesses appetite in the past 4 weeks. Diabetes mellitus (DM) is an example and patients with overlap SLE-DM can present with clinical features common to both disorders. In this review, we describe the patients with overlap SLE-DM, focussing on the clinical features common to both diseases that these patients can present, and on the challenges of managing such complications. A detailed review of the patients’ notes ( n = 485) was performed. At every outpatient appointment the patients’ urine was tested for glucose, protein and blood. Patients with persistent glycosuria were investigated with fasting blood glucose and a glucose tolerance test to help make the diagnosis of DM. Particular note was made of those patients whose symptoms could be due to SLE, DM or both. Nine patients with DM were identified. Antibodies that are directed against components of the cell nucleus almost always accompany syste... more Antibodies that are directed against components of the cell nucleus almost always accompany systemic lupus erythematosus (SLE). Formation of antibodies against double-stranded DNA is a hallmark of SLE. For many years, the development of antibodies against cell components that normally are sequestered into the nucleus remained puzzling to researchers who were working in this field of rheumatology. A decade ago, it became clear that these intracellular antigens could become exposed at the cell surface during physiologic cell death (apoptosis), and, under particular conditions, become immunogenic. The primary antigen in SLE has been believed to be naked DNA. DNA is a poor immunogen, however, and recent evidence suggests that nucleosomes, that comprise DNA and histones, constitute the primary inciting antigen in SLE. We review up-to-date data on the mechanisms by which nucleosomes and other lupus autoantigens may become immunogenic in lupus and the role of antinucleosome antibodies in the development of glomerulonephritis, a common and severe complication that occurs in approximately 60% of patients who have SLE.
