Purpose We present a rare case of a Sjogren’s syndrome presented as bilateral multifocal cystic m... more Purpose We present a rare case of a Sjogren’s syndrome presented as bilateral multifocal cystic major salivary gland disease. Material and methods A 38 year-old woman presented to the Oral & Maxillofacial dpt for examination of a progressing enlargement of her right preauricular area during the last six mounts period. A non painful, smooth discrete mass was obvious and palpated superficially of the right parotid. Regarding her medical history she reported recurrent, moderate episodes of dry keratoconjuctivitis and hypothyroidism due to Hasimmoto disease, well controlled by her endocrinologist. An USS examination of major salivary glands followed by an MRI scan were ordered. An FNA biopsy of the right parotid mass was suggested, followed by a sublabial glad biopsy. Results Both USS and MRI scan revealed multiple cystic lesions different in size within the major salivary glands, especially in parotids and diffuse enlargement of the parotid glands. Reports of FNA biopsy of the right parotid mass plus sublabial glad biopsy described diffuse lymphocyte infiltration of the glands with benign lymphoepithilial lesions. Conclusions Based mostly on the clinical and histological findings the diagnosis of Sjogren’s syndrome was carried out. Regarding the radiological findings we have to conclude that beyond the expected findings of dilated ducts, punctuate or globular collections and cavitary lesions, which characterize the disease, an appearance of multifocal cystic lesions bilaterally in major salivary gland, even rarely should be expected. Such findings should be under consideration for the differential diagnosis of diseases with benign lymphoepithilial lesions.

Purpose Sarcoma of the jaws is a rare, aggressive malignancy with a 5-year survival rate ranging ... more Purpose Sarcoma of the jaws is a rare, aggressive malignancy with a 5-year survival rate ranging between 27% and 84%. The purpose of the present study is to present our experience with a cohort of 11 patients treated in two departments of Oral and maxillofacial Surgery. Materials and Methods We present the experience of two departments of Oral and Maxillofacial Surgery with patients suffering from sarcomas of the jaws. The demographic,clinical and histopathologic records of 11 patients were obtained for retrospective review. Results In a retrospective chart review 10 cases of sarcomas of the jaws were identified. The age of the patients ranged from 20 to 69 years (mean age 34 years).The gender distribution consisted of six females and five males. The most frequent site distribution was the mandible which was affected in seven patients. The histologic subtypes included five cases of chondroblastic osteosarcomas, two cases of chondrosarcomas, two cases of osteoblastic osteosarcoma, one case of metastatic osteosarcoma and one case of malignant histiocytoma-like sarcoma. The primary treatment modality was surgery followed by radiotherapy in two patients, chemotherapy in one patient and combined therapy in five patients. To date five patients died from disease and six patients are disease-free. Follow-up for the patients who are still alive ranges from 12 months to 16 years. Conclusions Sarcoma of the jaws is a rare entity occurring mainly in young patients. Chemotherapy has not proven as effective as in sarcomas of the long bones and, in light of the rarity of the disease, it is difficult to establish a standard protocol. Early diagnosis and radical surgery is currently the treatment of choice in order to achieve higher survival rates.

Introduction Central mucoepidermoid carcinoma is a rare lesion, whose exact pathogenesis is still... more Introduction Central mucoepidermoid carcinoma is a rare lesion, whose exact pathogenesis is still not fully understood. Generally, it is more frequently encountered in the mandible than in the maxilla and in the posterior than in the anterior aspect of the jaws. Cases of central mucoepidermoid carcinomas are rarely mentioned in the literature, and therefore, additional information concerning this type of lesion is needed. Case report In the present paper, we report an extremely unusual case of a central mucoepidermoid carcinoma in the anterior region of the mandible of a 37-year-old female patient. The diagnosis and the surgical management of the tumor along with the reconstructive techniques utilized to restore the mandible and a 6-year follow-up are discussed. Discussion Central mucoepidermoid carcinoma is an unusual tumor of the jaws, especially in the anterior region. It is only locally aggressive and has most frequently a good prognosis if treated with wide surgical excision.

The rarest anatomical location where intraductal papillomas occur is the major salivary glands. In the present study, we report an intraductal papilloma of the left sublingual gland with the relevant clinical, imaging, and histopathological findings and postoperative follow-up. To our knowledge, this is the fourth case of an intraductal papilloma of a sublingual gland to be reported in the international literature. We present this rare case because of its clinical and radiological imitation of a ranula.

Based on its histological features, ossifying fibroma is divided into: a) juvenile trabecular OS and b) juvenile psammomatoid OS which affects mainly the paranasal sinuses of children and teenagers aging from 5 to 15 years. A rare case of juvenile psammomatoid ossifying fibroma in a 30 year old male patient located in the left mandibular ramus is presented. Treatment plan included radical surgical excision of the lesion and restoration with autologous osteochondral graft from the 6th rib of the ipsilateral side.

Background: Post-operative complications of various degrees of severity are commonly observed in ... more Background: Post-operative complications of various degrees of severity are commonly observed in third molar impaction surgery. For this reason, a surgical procedure that decreases the trauma of bone and soft tissues should be a priority for surgeons. In the present study, we compare the efficacy and the post-operative complications of patients to whom two different surgical techniques were applied for impacted lower third molar extraction. Material and Methods: Patients of the first group underwent the classical bur technique, while patients of the second group underwent another technique, in which an elevator was placed on the buccal surface of the impacted molar in order to luxate the alveolar socket more easily. Results: Comparing the two techniques, we observed a statistically significant decrease in the duration of the procedure and in the need for tooth sectioning when applying the second surgical technique, while the post-operative complications were similar in the two groups. We also found a statistically significant lower incidence of lingual nerve lesions and only a slightly higher frequency of sharp mandibular bone irregularities in the second group, which however was not statistically significant. Conclusions: The results of our study indicate that the surgical technique using an elevator on the buccal surface of the tooth seems to be a reliable method to extract impacted third molars safely, easily, quickly and with the minimum trauma to the surrounding tissues.

Material and methods Thirty cases of Mucoepidermoid Carcinomas and thirty cases of Adenoid Cystic Carcinomas of salivary glands treated at the Oral & Maxillofacial Surgery University Clinic of Papanikolaou Hospital of Thessaloniki and histologically examined at the Histology dpt of Papanikolaou Hospital of Thessaloniki are being studied retrospectively. The surgical specimens have already been stained by the four different antibodies and now their expressions are being immunohistochemically examined and clarified. Results Some results regarding the expressions of the molecules above and their combinations are expected to be ready for scientific announcement by the time of the 4th World Congress of IAOO. Conclusions We expect to utilize our results in conclusions regarding the association of the expression of the molecules with parameters such as tumor grading and furthermore the possibility of using the expression as prognostic or predictive factors.

Background Squamous cell carcinoma (SCC) of the tongue is one of the most common cancers in the o... more Background Squamous cell carcinoma (SCC) of the tongue is one of the most common cancers in the oral and maxillofacial region. The incidence of SCC of the tongue has increased during the recent decades in the developed world. The incidence of occult cervical lymph node metastasis is high in SCC of the tongue and the role of neck dissection in the management and its relevance to locoregional control is well known. Patients Complete clinical, histopathological and follow up data of 88 patients with pathologically confirmed SCC of the tongue were retrospectively analysed. All patients were treated at our Oral and Maxillofacial Surgery Department between 2000 and 2010 and all of them underwent a resection of the primary tumor and a neck dissection. Methods Overall and disease-free survival in relation to demographic characteristics of the patients and pathological features of the specimens is being investigated. Survival analysis is performed using the Kaplan–Meier analysis, comparisons among groups are analysed using a log-rank test and multivariate analysis is conducted using the cox proportional hazard model. Results They will be presented during the meeting.

To investigate alternative possibilities for the intraoperative evaluation of surgical margins af... more To investigate alternative possibilities for the intraoperative evaluation of surgical margins after bone resection utilizing more conventional hospital infrastructure technologies. MATERIALS AND METHODS: A small pilot study was performed using digital mammograph imaging intraoperatively on 16 surgical specimens of bone tumours or malignancies with bone infiltration of the head and neck area, with the aim of evaluating the resection margins. RESULTS: In thirteen cases the intraoperative specimen images indicated clinically complete excision. In two cases incomplete resection or close proximity of margins was detected, which required additional resection. CONCLUSIONS: The results indicated that intraoperative specimen radiography can prove useful in evaluating completeness of excision. The significance of intraoperative assessment of surgical margin is of paramount importance when immediate reconstruction is performed. This proposed method is cheap, easy to perform and fast. Its costebenefit ratio is superior than that of any other available technique. Intraoperative analysis of specimens with digital mammography imaging can potentially become a useful tool for immediate evaluation of osseous margins after resection.

CASE PRESENTATION: The case presented here illustrates the hypothesis that its specific pathogenesis is unknown. CONCLUSIONS: Mucoepidermoid carcinoma of the jaw is a rare tumour of unknown aetiology. Although about a 100 cases has been reported in literature, the speculation on its aetiopathogenesis has mainly centred on the pluripotential capabilities of the epithelial lining of odontogenic cysts.

Conclusions Sarcoma of the jaws is a rare entity occurring mainly in young patients. Chemotherapy has not proven as effective as in sarcomas of the long bones and, in light of the rarity of the disease, it is difficult to establish a standard protocol. Early diagnosis and radical surgery is currently the treatment of choice in order to achieve higher survival rates.

Generally, it is more frequently encountered in the mandible than in the maxilla and in the posterior than in the anterior aspect of the jaws. Cases of central mucoepidermoid carcinomas are rarely mentioned in the literature, and therefore, additional information concerning this type of lesion is needed. Case report In the present paper, we report an extremely unusual case of a central mucoepidermoid carcinoma in the anterior region of the mandible of a 37-year-old female patient. The diagnosis and the surgical management of the tumor along with the reconstructive techniques utilized to restore the mandible and a 6-year follow-up are discussed. Discussion Central mucoepidermoid carcinoma is an unusual tumor of the jaws, especially in the anterior region. It is only locally aggressive and has most frequently a good prognosis if treated with wide surgical excision.

The rarest anatomical location where intraductal papillomas occur is the major salivary glands. In the present study, we report an intraductal papilloma of the left sublingual gland with the relevant clinical, imaging, and histopathological findings and postoperative follow-up. To our knowledge, this is the fourth case of an intraductal papilloma of a sublingual gland to be reported in the international literature. We present this rare case because of its clinical and radiological imitation of a ranula.

Based on its histological features, ossifying fibroma is divided into: a) juvenile trabecular OS and b) juvenile psammomatoid OS which affects mainly the paranasal sinuses of children and teenagers aging from 5 to 15 years. A rare case of juvenile psammomatoid ossifying fibroma in a 30 year old male patient located in the left mandibular ramus is presented. Treatment plan included radical surgical excision of the lesion and restoration with autologous osteochondral graft from the 6th rib of the ipsilateral side.

For this reason, a surgical procedure that decreases the trauma of bone and soft tissues should be a priority for surgeons. In the present study, we compare the efficacy and the post-operative complications of patients to whom two different surgical techniques were applied for impacted lower third molar extraction. Material and Methods: Patients of the first group underwent the classical bur technique, while patients of the second group underwent another technique, in which an elevator was placed on the buccal surface of the impacted molar in order to luxate the alveolar socket more easily. Results: Comparing the two techniques, we observed a statistically significant decrease in the duration of the procedure and in the need for tooth sectioning when applying the second surgical technique, while the post-operative complications were similar in the two groups. We also found a statistically significant lower incidence of lingual nerve lesions and only a slightly higher frequency of sharp mandibular bone irregularities in the second group, which however was not statistically significant. Conclusions: The results of our study indicate that the surgical technique using an elevator on the buccal surface of the tooth seems to be a reliable method to extract impacted third molars safely, easily, quickly and with the minimum trauma to the surrounding tissues.

Material and methods Thirty cases of Mucoepidermoid Carcinomas and thirty cases of Adenoid Cystic Carcinomas of salivary glands treated at the Oral & Maxillofacial Surgery University Clinic of Papanikolaou Hospital of Thessaloniki and histologically examined at the Histology dpt of Papanikolaou Hospital of Thessaloniki are being studied retrospectively. The surgical specimens have already been stained by the four different antibodies and now their expressions are being immunohistochemically examined and clarified. Results Some results regarding the expressions of the molecules above and their combinations are expected to be ready for scientific announcement by the time of the 4th World Congress of IAOO. Conclusions We expect to utilize our results in conclusions regarding the association of the expression of the molecules with parameters such as tumor grading and furthermore the possibility of using the expression as prognostic or predictive factors.

The incidence of SCC of the tongue has increased during the recent decades in the developed world. The incidence of occult cervical lymph node metastasis is high in SCC of the tongue and the role of neck dissection in the management and its relevance to locoregional control is well known. Patients Complete clinical, histopathological and follow up data of 88 patients with pathologically confirmed SCC of the tongue were retrospectively analysed. All patients were treated at our Oral and Maxillofacial Surgery Department between 2000 and 2010 and all of them underwent a resection of the primary tumor and a neck dissection. Methods Overall and disease-free survival in relation to demographic characteristics of the patients and pathological features of the specimens is being investigated. Survival analysis is performed using the Kaplan–Meier analysis, comparisons among groups are analysed using a log-rank test and multivariate analysis is conducted using the cox proportional hazard model. Results They will be presented during the meeting.

The purpose of the present study is to present our experience with a cohort of 11 patients treated in two departments of Oral and maxillofacial Surgery. Materials and Methods We present the experience of two departments of Oral and Maxillofacial Surgery with patients suffering from sarcomas of the jaws. The demographic,clinical and histopathologic records of 11 patients were obtained for retrospective review. Results In a retrospective chart review 10 cases of sarcomas of the jaws were identified. The age of the patients ranged from 20 to 69 years (mean age 34 years).The gender distribution consisted of six females and five males. The most frequent site distribution was the mandible which was affected in seven patients. The histologic subtypes included five cases of chondroblastic osteosarcomas, two cases of chondrosarcomas, two cases of osteoblastic osteosarcoma, one case of metastatic osteosarcoma and one case of malignant histiocytoma-like sarcoma. The primary treatment modality was surgery followed by radiotherapy in two patients, chemotherapy in one patient and combined therapy in five patients. To date five patients died from disease and six patients are disease-free. Follow-up for the patients who are still alive ranges from 12 months to 16 years. Conclusions Sarcoma of the jaws is a rare entity occurring mainly in young patients. To investigate alternative possibilities for the intraoperative evaluation of surgical margins af... more To investigate alternative possibilities for the intraoperative evaluation of surgical margins after bone resection utilizing more conventional hospital infrastructure technologies. MATERIALS AND METHODS: A small pilot study was performed using digital mammograph imaging intraoperatively on 16 surgical specimens of bone tumours or malignancies with bone infiltration of the head and neck area, with the aim of evaluating the resection margins. RESULTS: In thirteen cases the intraoperative specimen images indicated clinically complete excision. In two cases incomplete resection or close proximity of margins was detected, which required additional resection. CONCLUSIONS: The results indicated that intraoperative specimen radiography can prove useful in evaluating completeness of excision. The significance of intraoperative assessment of surgical margin is of paramount importance when immediate reconstruction is performed. This proposed method is cheap, easy to perform and fast. Its costebenefit ratio is superior than that of any other available technique. Intraoperative analysis of specimens with digital mammography imaging can potentially become a useful tool for immediate evaluation of osseous margins after resection.

CASE PRESENTATION: The case presented here illustrates the hypothesis that its specific pathogenesis is unknown. CONCLUSIONS: Mucoepidermoid carcinoma of the jaw is a rare tumour of unknown aetiology. Although about a 100 cases has been reported in literature, the speculation on its aetiopathogenesis has mainly centred on the pluripotential capabilities of the epithelial lining of odontogenic cysts.
