A case of malignant mixed mullerian tumor of the ovary in a 57 year old woman is reported along w... more A case of malignant mixed mullerian tumor of the ovary in a 57 year old woman is reported along with the results of an immunohistochemical study. The tumor, measuring 16·10·9cm, was composed predominantly of adenocarcinoma with a smaller amount of anaplastic carcinoma as an epithelial component and chondrosarcoma, liposarcoma, fibrosarcoma and rhabdomyoblasts as mesenchymal elements. Immunohistochemistry using paraffin sections demonstrated cytokeratin (CK) and epithelial membrane antigen (EMA), generally regarded as epithelial markers, not only in the epithelial component but also in chondrosarcoma cells. Vimentin and desmin, generally regarded as mesenchymal markers, were exhibited partly in carcinoma cells as well as in mesenchymal elements. Positive staining for S 100 protein was obtained not only in chondrosarcoma and liposarcoma cells, but also partly in adenocarcinoma cells. This intricate immunohistochemical picture reflected the histologic findings. It is noteworthy that both carcinoma cells and chondrosarcoma cells demonstrated simultaneous expression of CK, EMA, vimentin, desmin and S 100 protein. This somewhat unusual antigen expression by tumor cells may indicate a change in the nature of tumor cells due to microenvironmental factors. Acta Pathol Jpn 40: 845 850, 1990.

But the vascular tumors of the ovary are extremely rare. Ovarian masses are mostly discovered accidentally during surgery or imaging. These tumors may rarely be associated with systemic manifestations. Here we report a case of 17 year old young unmarried obese girl who presented with acute pain in abdomen and was treated laproscopically with conservation of ovaries. Histopathology report was suggestive of ovarian haemangioma. The diagnostic challenge and therapeutic rarity of such a tumour in a young unmarried girl has proven to be an exceptional case and an excellent investigative opportunity.

They are usually benign, but show variable grades of malignancy. They are slow growing tumors with destructive behavior and ability to metastasize and recur locally. They most commonly involve lungs, liver and musculoskeletal system and rarely involve head and neck, intestines, lymph node, pleura, peritoneum and stomach. Here, we present a case hemangioendothelioma of oral cavity and oropharynx in a 50-year-old male patient who presented with a palatal swelling.

T... more A 47-year-old man had recurrent, episodic swelling of the right upper eyelid for about 2 years. There was a progressively enlarging, palpable mass over the superotemporal region of the right orbit without tenderness, local heat, or erythema. Ophthalmic examination revealed proptosis and ptosis of the right eye without vision decrease or ocular motility disturbance. An orbital computed tomography scan showed a huge lacrimal gland causing inferior displacement of the globe with proptosis. An anterior orbitotomy was done, and the pathological analysis revealed marked lymphoplasma cell infiltration with mild to moderate fibrosis surrounding the lobules of the lacrimal glands. No lymphoepithelial islands were seen. Immunohistochemically, about 40e50% of the immunoglobulin G (IgG)-positive cells were IgG4 positive, and the ratio of serum IgG4 to total IgG was 80.9%, compatible with a diagnosis of IgG4related chronic sclerosing dacryoadenitis. IgG4-related chronic sclerosing dacryoadenitis may be a sign of Mikulicz disease, an IgG4-related systemic disease. Determination of the serum IgG4 concentration and IgG4/IgG ratio is necessary for clinical diagnosis. Long-term follow-up is also mandatory because of the possible complication of lymphoma.

Of all the gastrointestinal tumours only 1–2% account for NETs. They have an extremely low incidence of 0.17% arising in the intrahepatic bile duct epithelium. Majority of hepatic NETs occur as a result of metastases from the primary NETs. Most cases of primary hepatic NET (PHNET) present as a solid nodular mass. However, predominantly cystic PHNET is extremely rare which mimics other cystic space-occupying lesions clinically and radiologically as seen in this case.

Although cardiac lesions have been known to the existence since the Middle Ages, a search on Goog... more Although cardiac lesions have been known to the existence since the Middle Ages, a search on Google or PubMed for cardiac lesions yields very few results. There are few original articles, case series, or clinical trials available on these rare but significant lesions. Cardiac lesions are usually asymptomatic and are detected only incidentally. Symptoms, if present, are varied and depend on the location of the area involved. The overall prevalence of primary cardiac tumors is between 0.17% and 0.19%. Cardiac tumors may be primary (benign or malignant) or metastatic (malignant). Only 25% of primary cardiac tumors are malignant, whereas metastatic tumors to the heart are 30 times more common. The aim of this case series is to highlight the clinical picture, presenting signs and symptoms, and the line of diagnosis of five such primary cardiac lesions that otherwise could remain undiagnosed until a late stage.

Spindle cell carcinoma (SpCC) is an unusual variant of squamous cell carcinoma (S.C.C.) of the he... more Spindle cell carcinoma (SpCC) is an unusual variant of squamous cell carcinoma (S.C.C.) of the head and neck region. It is a rare biphasic neoplasm. SpCC is an unusual morphological variant of S.C.C. It is characterized by the presence of both carcinomatous as well as sarcomatous component and accounts for 2 to 3% of all the S.C.C.s. Seven years ago, the W.H.O. classification has placed this tumor under the malignant epithelial tumors and called it SpCC. The histogenesis of spindle cells remains controversial and is believed to be monoclonal epithelial neoplasia with a close association with squamous carcinoma cells. Since SpCC is a rare tumor, its histopathological diagnosis is often very complex. Immunohistochemistry (I.H.C.) supports the epithelial nature of this tumor. Both neoplasia components possess immunoreactivity for cytokeratin and vimentin. We are presenting a case of a middle-aged man who was diagnosed with this rare variant at an even rarer location.

It was very tough to distinguish Tuberculosis from Crohn’s Disease in this case as the sections for histology were continuously negative for Acid fast bacilli. This case demonstrates the diagnostic predicaments encountered to arrive at a diagnosis of Intestinal Tuberculosis.

Materials and Methods: A five year study was performed on salivary gland swellings in a tertiary health care center. Fine needle aspiration cytology was done and smears were stained with Leishman, Hematoxylin and eosin stains. Histopathology was done wherever possible. Cytological and histopathological correlation was done and results were statistically analyzed. Results: 100 cases were studied. Lesions were categorized into non-neoplastic and neoplastic lesions. Chronic sialadenitis was the most common non- neoplastic lesion and Pleomorphic Adenoma was the commonest benign lesion. Almost more than 257 million people live with chronic Hepatitis B infection all around the world ending up with hepatocellular carcinoma. This has become a major concern of public health nowadays. The present study was aimed to assess the prevalence of Hepatitis B infection among voluntary healthy blood donors in the industrial zone of Western Maharashtra. A total of 5000 donors fulfilled the inclusion criteria of the study and was accepted for blood donation during the study period of May 2019 to October 2020. The blood samples were tested for Hepatitis B surface Antigen as well as other Transfusion Transmitted Infections. Out of 5000 donors, the prevalence of HBsAg positive donors in our study was 0.62%. Introduction: Dural based lesions, often misinterpreted as meningiomas, may have deleterious impa... more Introduction: Dural based lesions, often misinterpreted as meningiomas, may have deleterious impact on patient care and may mislead the presurgical treatment algorithm. The study proposes the term 'Meningoids' for clubbing intracranial lesions which clinically and radiologically mimic meningiomas. The study also defines the various features of such lesions which help to raise a suspicion in surgeon's mind and also prepares him for possible intra and post-operative challenges and surprises. Materials and methods: All patients presenting with a preoperative radiological diagnosis of meningioma and a final histopathological diagnosis other than meningioma were included in the study. Patients were recognised and their relevant radiological profiles were retrospectively drawn and assessed in terms of variables based on plain and contrast enhanced CT scan and MRI findings. Data was presented in a tabulated form and variables were analysed based on features mimicking and atypical of meningioma. Results: A total of 9 patients were included in the study. A final diagnosis of tuberculoma and hemangioblastoma were made in 2 patients each. One patient each had a final diagnosis of sarcoidosis, non hodgkin's lymphoma, chondroma, ependymoma and metastasis from adenocarcinoma. On retrospectively analysing the data, the following findings were found to raise a high index of suspicion for a meningoid: cystic changes, disproportionate oedema, erosive bony changes, atypical dural tail, flow voids around the lesion and intraoperative gross features. Conclusion: Its not uncommon for neurosurgeons to encounter dural based lesions mimicking meningioma and being prepared preoperatively with 'meningoids' would be important to optimise outcomes, minimise morbidity and most importantly forcing the neurosurgeon to think critically which is eventually helpful in prognostication.

Histopathological evaluation of GI lesions is the gold standard for providing essential diagnostic and prognostic information to clinicians for the best and timely management of each patient. Aims and Objectives: To study the pattern of gastrointestinal non-neoplastic and neoplastic lesions and the correlation between age, sex and site distribution of various lesions. Material and Methods: This is a unicentre retrospective and prospective study of 2941 patients with GI lesions over a period of four years from January 2015 to December 2018. Biopsies and resected specimens of gastrointestinal tract were fixed in 10% Formalin. These specimens were then; processed in automatic tissue processor. Routine Hematoxylin and Eosin stain and special stains, including immunohistochemical stains were done; wherever indicated. The slides were examined and the data generated from observations was used for statistical analysis. Results: Out of a total 21831 histopathology specimens received in the four years, over 17 % specimens were GIT lesions. Of the 3738 GI specimens, appendix was the most common specimen received (26.9%) followed by gastric specimens (23.2%) and small intestine specimens (18.9%). Non-neoplastic GI lesions (94.5%) were most common followed by neoplastic lesions (5.5%). Males outnumbered female patients having M: F ratio of 1.2:1. The peak incidence for non-neoplastic lesions was found in the age group of 21-40 years and that for neoplastic lesions found in the age group of 50-70 years. Appendicitis was the most common GI pathology (26.8%) followed by gastritis (21.1%). Adenocarcinoma was the most common histological malignancy (65.4%) followed by Squamous cell carcinoma (26.7%). Most common lesion of the esophagus was squamous cell carcinoma whereas adenocarcinoma was common in the colo-rectum. Conclusion: Apart from an overview of the spectrum of histopathological lesions encountered in the surgical pathology department, this study reiterates that histopathological evaluation is a valuable diagnostic tool for definitive, early diagnosis of GI lesions, which has impact on management of the neoplasia.

Methods: Cytological smears of 58 cases of breast carcinoma diagnosed on fine needle aspiration cytology (FNAC) were graded according to the Robinson grading system. The corresponding histological sections were classified as per the classification proposed by World Health Organization (WHO) in 2003 and histological grading was done by Elston and Ellis’s modification of BloomRichardson method. Results: Cytological grading showed an absolute correlation with the histological grade in 82.76% cases. In 2010 WHO classification of neuroendocrine tumors consists of an adenocarcinoma component and a neuroendocrine carcinoma component in which each of the components accounts for 30% of the tumor. Such tumors are defined as mixed adenoneuroendocrine carcinomas. Occurrence of exocrine and endocrine tumors of the pancreas is extremely rare. The aim of our study was to describe a case in a 60 years old male who was diagnosed with this rare tumor. Gross, microscopic features and immunohistochemistry were used to diagnose this rare condition. Immunohistochemistry markers such as synaptophysin, chromogranin, EMA and Pan CK were used to come to a definitive diagnosis. Meningiomas are tumours that originate from the arachnoid cells (meningocytes) and the majority a... more Meningiomas are tumours that originate from the arachnoid cells (meningocytes) and the majority are benign grade I tumours according to World Health Organisation (WHO). Chordoid meningioma is an uncommon variant of meningioma and corresponds to grade II tumour in WHO classification of tumours of the Nervous System 2016, because of its more aggressive behaviour and increased likelihood of recurrence. These meningiomas may appear extracranially (i.e. head and neck region, sinonasal tract, ear, temporal bone, scalp, etc) in only 2% cases. The histopathological and immunohistochemical evaluation is usually diagnostic. Here, the authors present a case of Chordoid meningioma in a 32-year-old patient who presented with complaints of nasal obstruction clinically suspected to be due to nasal polyp.

Of all the lesions of the stomach, inflammatory conditions and neoplastic lesions are most common. Primary malignant lymphoma of the stomach accounts for only a small percentage (about 10%) of all malignant tumors of the stomach, and majority of them arise from B-cells. Most common among these are indolent extranodal marginal zone lymphomas, and in the stomach, they are called MALTomas. Occasionally, gastric lymphomas of one type or another are seen adjacent to adenocarcinoma. Sometimes, lymphoma precedes carcinoma by several years. The aim was to study the microscopic patterns of gastric biopsies and mastectomy specimens received for the case in our department. Multiple gastric biopsy specimens as well as gastrectomy specimens were received in our department. Multiple sections were taken and stained with hematoxylin and eosin. Immunohistochemical (IHC) stains were used as applicable. Studies posed a diagnostic difficulty by showing picture of gastritis from biopsy of the cardia; signet ring cell adenocarcinoma was seen in small gastrointestinal biopsies from lesser curvature and gastric lymphoma with gross picture of linitis plastica on gastrectomy specimen. For confirmation and to know the type of gastric lymphoma, multiple B- and T-cell IHC markers were used. CD20, CD10, BCL2, and c-Myc were positive in tumor cells, CD3 positivity was seen in reactive T-cells, and MIB1 positivity was seen in 70%–75% of tumor cells. Hence, it was confirmed to be high-grade large cell lymphoma (double-expresser type). Hence, it can be concluded that IHC markers are necessary to solve the diagnostic difficulties.

The purpose of this paper is to conceptualize the brand of tourism resources and examine the effe... more The purpose of this paper is to conceptualize the brand of tourism resources and examine the effects of brand equity on brand loyalty focusing on the brand of tourism resources in cities and counties level. To this end, we reviewed the literatures and analysed 410 surveys from the 8 provinces. The results show that brand awareness and brand image of tourism resources have significant effects on brand preference, and brand awareness, image and preference have effects on brand loyalty measured by revisit and recommendation intention. However the one of most important results is the brand equity(awareness, image, preference) has partially significant effects on brand loyalty under the effects of visitor's satisfaction. Additionally, the positive effect of brand awareness on brand image is examined. More specific results and implications are provided.

Of all the gastrointestinal tumours only 1–2% account for NETs. They have an extremely low incidence of 0.17% arising in the intrahepatic bile duct epithelium. Majority of hepatic NETs occur as a result of metastases from the primary NETs. Most cases of primary hepatic NET (PHNET) present as a solid nodular mass. However, predominantly cystic PHNET is extremely rare which mimics other cystic space-occupying lesions clinically and radiologically as seen in this case.

There are few original articles, case series, or clinical trials available on these rare but significant lesions. Cardiac lesions are usually asymptomatic and are detected only incidentally. Symptoms, if present, are varied and depend on the location of the area involved. The overall prevalence of primary cardiac tumors is between 0.17% and 0.19%. Cardiac tumors may be primary (benign or malignant) or metastatic (malignant). Only 25% of primary cardiac tumors are malignant, whereas metastatic tumors to the heart are 30 times more common. The aim of this case series is to highlight the clinical picture, presenting signs and symptoms, and the line of diagnosis of five such primary cardiac lesions that otherwise could remain undiagnosed until a late stage.

It is a rare biphasic neoplasm. SpCC is an unusual morphological variant of S.C.C. It is characterized by the presence of both carcinomatous as well as sarcomatous component and accounts for 2 to 3% of all the S.C.C.s. Seven years ago, the W.H.O. classification has placed this tumor under the malignant epithelial tumors and called it SpCC. The histogenesis of spindle cells remains controversial and is believed to be monoclonal epithelial neoplasia with a close association with squamous carcinoma cells. Since SpCC is a rare tumor, its histopathological diagnosis is often very complex. Immunohistochemistry (I.H.C.) supports the epithelial nature of this tumor. Both neoplasia components possess immunoreactivity for cytokeratin and vimentin. We are presenting a case of a middle-aged man who was diagnosed with this rare variant at an even rarer location.

A total of 5000 donors fulfilled the inclusion criteria of the study and was accepted for blood donation during the study period of May 2019 to October 2020. Introduction: Dural based lesions, often misinterpreted as meningiomas, may have deleterious impa... more Introduction: Dural based lesions, often misinterpreted as meningiomas, may have deleterious impact on patient care and may mislead the presurgical treatment algorithm. The study proposes the term 'Meningoids' for clubbing intracranial lesions which clinically and radiologically mimic meningiomas. The study also defines the various features of such lesions which help to raise a suspicion in surgeon's mind and also prepares him for possible intra and post-operative challenges and surprises. Materials and methods: All patients presenting with a preoperative radiological diagnosis of meningioma and a final histopathological diagnosis other than meningioma were included in the study. Patients were recognised and their relevant radiological profiles were retrospectively drawn and assessed in terms of variables based on plain and contrast enhanced CT scan and MRI findings. Data was presented in a tabulated form and variables were analysed based on features mimicking and atypical of meningioma. Results: A total of 9 patients were included in the study. A final diagnosis of tuberculoma and hemangioblastoma were made in 2 patients each. One patient each had a final diagnosis of sarcoidosis, non hodgkin's lymphoma, chondroma, ependymoma and metastasis from adenocarcinoma. On retrospectively analysing the data, the following findings were found to raise a high index of suspicion for a meningoid: cystic changes, disproportionate oedema, erosive bony changes, atypical dural tail, flow voids around the lesion and intraoperative gross features. Conclusion: Its not uncommon for neurosurgeons to encounter dural based lesions mimicking meningioma and being prepared preoperatively with 'meningoids' would be important to optimise outcomes, minimise morbidity and most importantly forcing the neurosurgeon to think critically which is eventually helpful in prognostication.

Histopathological evaluation of GI lesions is the gold standard for providing essential diagnostic and prognostic information to clinicians for the best and timely management of each patient. Aims and Objectives: To study the pattern of gastrointestinal non-neoplastic and neoplastic lesions and the correlation between age, sex and site distribution of various lesions. Material and Methods: This is a unicentre retrospective and prospective study of 2941 patients with GI lesions over a period of four years from January 2015 to December 2018. Biopsies and resected specimens of gastrointestinal tract were fixed in 10% Formalin. These specimens were then; processed in automatic tissue processor. Routine Hematoxylin and Eosin stain and special stains, including immunohistochemical stains were done; wherever indicated. The slides were examined and the data generated from observations was used for statistical analysis. Results: Out of a total 21831 histopathology specimens received in the four years, over 17 % specimens were GIT lesions. Of the 3738 GI specimens, appendix was the most common specimen received (26.9%) followed by gastric specimens (23.2%) and small intestine specimens (18.9%). Non-neoplastic GI lesions (94.5%) were most common followed by neoplastic lesions (5.5%). Males outnumbered female patients having M: F ratio of 1.2:1. The peak incidence for non-neoplastic lesions was found in the age group of 21-40 years and that for neoplastic lesions found in the age group of 50-70 years. Appendicitis was the most common GI pathology (26.8%) followed by gastritis (21.1%). Adenocarcinoma was the most common histological malignancy (65.4%) followed by Squamous cell carcinoma (26.7%). Most common lesion of the esophagus was squamous cell carcinoma whereas adenocarcinoma was common in the colo-rectum. Conclusion: Apart from an overview of the spectrum of histopathological lesions encountered in the surgical pathology department, this study reiterates that histopathological evaluation is a valuable diagnostic tool for definitive, early diagnosis of GI lesions, which has impact on management of the neoplasia.

Methods: Cytological smears of 58 cases of breast carcinoma diagnosed on fine needle aspiration cytology (FNAC) were graded according to the Robinson grading system. The corresponding histological sections were classified as per the classification proposed by World Health Organization (WHO) in 2003 and histological grading was done by Elston and Ellis’s modification of BloomRichardson method. Results: Cytological grading showed an absolute correlation with the histological grade in 82.76% cases. The aim of our study was to describe a case in a 60 years old male who was diagnosed with this rare tumor. Gross, microscopic features and immunohistochemistry were used to diagnose this rare condition. Chordoid meningioma is an uncommon variant of meningioma and corresponds to grade II tumour in WHO classification of tumours of the Nervous System 2016, because of its more aggressive behaviour and increased likelihood of recurrence. These meningiomas may appear extracranially (i.e. head and neck region, sinonasal tract, ear, temporal bone, scalp, etc) in only 2% cases. Of all the lesions of the stomach, inflammatory conditions and neoplastic lesions are most common. Primary malignant lymphoma of the stomach accounts for only a small percentage (about 10%) of all malignant tumors of the stomach, and majority of them arise from B-cells. Most common among these are indolent extranodal marginal zone lymphomas, and in the stomach, they are called MALTomas. Occasionally, gastric lymphomas of one type or another are seen adjacent to adenocarcinoma. Sometimes, lymphoma precedes carcinoma by several years. The aim was to study the microscopic patterns of gastric biopsies and mastectomy specimens received for the case in our department. Multiple gastric biopsy specimens as well as gastrectomy specimens were received in our department. Multiple sections were taken and stained with hematoxylin and eosin. Immunohistochemical (IHC) stains were used as applicable. Studies posed a diagnostic difficulty by showing picture of gastritis from biopsy of the cardia; signet ring cell adenocarcinoma was seen in small gastrointestinal biopsies from lesser curvature and gastric lymphoma with gross picture of linitis plastica on gastrectomy specimen. For confirmation and to know the type of gastric lymphoma, multiple B- and T-cell IHC markers were used. CD20, CD10, BCL2, and c-Myc were positive in tumor cells, CD3 positivity was seen in reactive T-cells, and MIB1 positivity was seen in 70%–75% of tumor cells. Hence, it was confirmed to be high-grade large cell lymphoma (double-expresser type). Hence, it can be concluded that IHC markers are necessary to solve the diagnostic difficulties.

To this end, we reviewed the literatures and analysed 410 surveys from the 8 provinces. The results show that brand awareness and brand image of tourism resources have significant effects on brand preference, and brand awareness, image and preference have effects on brand loyalty measured by revisit and recommendation intention. However the one of most important results is the brand equity(awareness, image, preference) has partially significant effects on brand loyalty under the effects of visitor's satisfaction. Additionally, the positive effect of brand awareness on brand image is examined. More specific results and implications are provided.
