El tumor sólido-pseudopapilar del páncreas (TSPP) es una neoplasia infrecuente (1-2% de los tumor... more El tumor sólido-pseudopapilar del páncreas (TSPP) es una neoplasia infrecuente (1-2% de los tumores exocrinos), que afecta predominantemente a mujeres jóvenes, con bajo potencial de malignidad (15% da metástasis). Su diagnóstico preoperatorio es difícil, principalmente debido a que a la imagenología no tiene una característica que la pueda diferenciar de otros quistes pancreáticos, por lo que generalmente es un hallazgo histopatológico. El tratamiento quirúrgico presenta buena sobrevida, incluso en presencia de metástasis. Presentamos 3 casos con cuadro clínico, imagenología, tratamiento e histopatología, con el fin de aportar más información, sobre esta infrecuente patología.

La fasciolasis es una enfermedad parasitaria producida por el tremátodo digeneo Fasciola hepática... more La fasciolasis es una enfermedad parasitaria producida por el tremátodo digeneo Fasciola hepática. Durante la fase crónica de la enfermedad los parásitos pueden entran en los conductos biliares, y ser causa de ictericia obstructiva, colangitis, pancreatitis y otras complicaciones. Se presenta el caso clínico de una paciente intervenida por sospecha de colelitiasis y colédocolitiasis, a la cual durante la exploración coledociana intraoperatoria le fueron extraídos múltiples parásitos adultos de Fasciola hepática. Este hallazgo sólo fue aclarado posteriormente en la biopsia diferida. Se realiza una revisión breve del tema, métodos de diagnóstico, tratamiento médico y alternativas quirúrgicas de tratamiento.

Comparar la sobrevida del cáncer gástrico en anillo de sello (AS) con los otros tipos de adenocar... more Comparar la sobrevida del cáncer gástrico en anillo de sello (AS) con los otros tipos de adenocarcinoma (no anillo de sello (NAS)), según el compromiso en profundidad de la pared gástrica (T) y los números de linfonodos comprometidos (N). Material y Métodos: Se realizó un estudio de cohorte retrospectivo, en el cuál se seleccionó a todos los pacientes gastrectomizados por adenocarcinoma gástrico en el Hospital Base Valdivia entre los años 1988-2005. Se compararon los pacientes cuyo informe histopatológico concluía que el cáncer gástrico era un AS (más del 50% de sus células son anillo de sello) con los que no lo eran (NAS). En el análisis estadístico se utiliza el test de χ 2 , Método de Kaplan Meier, test de Log Rank. Resultados: Total 287 casos. 60 AS y 227 NAS. No hubo diferencias estadísticamente significativas en cuanto a la profundidad tumoral entre ambos grupos. Los AS presentaron mayor compromiso linfonodal. Al comparar la sobrevida a 5 años de los AS con los NAS por estadio (TNM), no encontramos diferencias significativas. Conclusión: A la luz de este trabajo el tipo histológico anillo de sello no parece constituir un factor de mal pronóstico en el cáncer gástrico.

Cytogenetic characterization is hampered in the absence of invasion to the bone marrow, origin of cells that are usually studied in cytogenetic studies. We report a 13 years old male presenting with a mass in the right shoulder. A biopsy of the tumor disclosed a Myeloid Sarcoma. A conventional cytogenetic study of a bone marrow aspirate did not show t (8;21) translocation. A fl uorescent in situ hybridization (FISH) performed in the paraffi n embedded biopsy of the tumor, detected a chromosome 11 trisomy.

L a cistitis eosinofílica (CE) es una rara enfermedad que se caracteriza por la infiltración de t... more L a cistitis eosinofílica (CE) es una rara enfermedad que se caracteriza por la infiltración de toda la pared vesical por un proceso inflamatorio comandado por eosinófilos, que provoca engrosamiento de la mucosa, fibrosis con o sin necrosis muscular. Fue descrita por primera vez por Brown 1 y Palubinskas 2 en 1960, y desde entonces se han reportado menos de 200 casos en el mundo 3,4. La importancia de su conocimiento radica en que el cuadro clínico semeja a otras patologías de mayor frecuencia (cistitis intersticial, infecciones urinarias, neoplasias vesicales) pero su manejo es totalmente diferente. Epidemiología La incidencia de CE es extremadamente baja. Afecta a pacientes de todas las edades, pero principalmente adultos (83 %) 5. No existe diferencia entre hombres y mujeres, salvo en niños donde es más frecuente en hombres 3,4. No se han definido factores de riesgo, pero si patologías asociadas como asma bronquial, atopía, eosinofilias, parasitosis 6 , neoplasias vesicales 1,7,8 , trauma vesical 9 , enteritis eosinofílica 10 , infecciones urinarias 11. Existen reportes de CE en relación al uso de ciertos fármacos como Ciclofosfamida, Coumarina, Tranilast, Penicilina, Mitomicina C 12-16 .

Background: Primary malignant tumors of appendix are uncommon and usually found during the pathol... more Background: Primary malignant tumors of appendix are uncommon and usually found during the pathological examination of surgical pieces of appendectomies. aim: To report the clinical and pathological features of appendiceal malignant tumors. material and methods: Review of medical and pathological reports of patients subjected to an appendectomy between 1998 and 2006 in two regional hospitals. results: Fifteen appendiceal malignant tumors in 2,687 pathological studies were detected (0.55% of all studies) in one hospital. In the other 10 tumors were detected in 4,939 studies (0.2%). nine bearers of tumors were male. In each hospital 93 and 80% of patients had an acute appendicitis, respectively. The pathology report informed a neuroendocrine tumor in 87 and 70% of patients of each hospital, respectively, followed by non-Hodgkin lymphoma and adenocarcinoma. In nine patients a staging study was performed. Four patients died during follow up, one of them due to tumor disseminations. conclusions: Although appendiceal tumors appear in only 0.3% of all appendectomies, the pathological study of the excised appendix is fundamental for the diagnosis.

Gallbladder cancer (GBC) is a rare pathology in Western countries. However, it constitutes a rele... more Gallbladder cancer (GBC) is a rare pathology in Western countries. However, it constitutes a relevant health problem in Asia and Latin America, with a high mortality in middle-aged Chilean women. The limited therapeutic options for GBC require the identification of targetable proteins with prognostic value for improving clinical management support. We evaluated the expression of targetable proteins, including three epithelial tumor markers, four proteins associated with multidrug and apoptosis resistance, and eleven immunological markers in 241 primary gallbladder adenocarcinomas. We investigated correlations between tumor marker expression, the primary tumor staging, and GBC patients’ survival using automated immunohistochemistry, a semi-automatic method for image analysis, univariate and multivariate statistical analyses, and machine learning algorithms. Disruption of bicarbonate entry into airway lumen, as has been observed in cystic fibrosis, produces several defects in lung function due to thick mucus accumulation. Bicarbonate is critical for correct mucin deployment and there is increasing interest in understanding its role in airway physiology, particularly in the initiation of lung disease in children affected by cystic fibrosis, in the absence of detectable bacterial infection. The current model of anion secretion in mammalian airways consists of CFTR and TMEM16A as apical anion exit channels, with limited capacity for bicarbonate transport compared to chloride. However, both channels can couple to SLC26A4 anion exchanger to maximise bicarbonate secretion. We... more Hepatitis C Virus (HCV) is a cause of secondary chronic immune thrombocytopenic purpura (ITP). We report a 43 year old man with mild Hemophilia A, who received blood transfusions during childhood, that consulted for epistaxis and ecchymoses. The laboratory showed a platelet count of 23.000/mm³, positive HCV serology and elevated transaminases. Steroids administered in pulses followed by oral doses resulted in a partial response requiring a second pulse and association of azathioprine. A steroidal diabetes appeared. Given his refractoriness, splenectomy and liver biopsy were performed. He continued on azathioprine maintaining a platelet count near 50.000/mm³ but continued with bleeding episodes. Liver biopsy showed a chronic active hepatitis.

The identification of new tumor-associated markers with potential prognosis value is required for GBC clinical practice. Using immunohistochemistry/tumor tissue microarray, we evaluated the expression of 17 gastrointestinal tumor-associated protein markers (CK7, CK17, CK19, CK20, CKLMW, CKHMW, MUC1, MUC2, MUC5AC, MUC6, CA125, CD10, CEA, vimentin, villin, claudin-4, and CDX2) in primary gallbladder adenocarcinomas from 180 Chilean patients and analyzed potential associations with their pathological and clinical characteristics. Gallbladder cancer (GBC) is an aggressive and highly lethal disease with relatively low global in... more Gallbladder cancer (GBC) is an aggressive and highly lethal disease with relatively low global incidence, but one that constitutes a major health problem in Asian and Latin American countries, particularly in Chile. The identification of new tumor-associated markers with potential prognosis value is required for GBC clinical practice. Using immunohistochemistry/tumor tissue microarray, we evaluated the expression of 17 gastrointestinal tumor-associated protein markers (CK7, CK17, CK19, CK20, CKLMW, CKHMW, MUC1, MUC2, MUC5AC, MUC6, CA125, CD10, CEA, vimentin, villin, claudin-4, and CDX2) in primary gallbladder adenocarcinomas from 180 Chilean patients and analyzed potential associations with their pathological and clinical characteristics.
