The article presents a review of latest researches related to various aspects of primary glaucoma... more The article presents a review of latest researches related to various aspects of primary glaucoma and optic neuropathy etiology. The effect of somatic factors on glaucoma progression is described. Arguments in favor of the interrelation of glaucoma and neurodegenerative processes are presented. The genetic basis for the development of glaucoma and a variety of its conjoined syndromes is considered. Immunological mechanisms that initiate the programmed cell death are analyzed. The processes that influence the increase of trabecular meshwork retention are also described as well as its role in the glaucoma pathogenesis.

В части, посвященной рефракционной хирургии детально рассматривается операция фемтоLASIK, её преимущества перед классической модификацией, специфические осложнения, характерные для проведения операции с применением фемтосекундных технологий, а также кераторефракционные вмешательства SMILE, ReLex и возможности коррекции пресбиопии с помощью имплантации инлаев. Также в статье рассмотрены особенности проведения различных видов кератопластики с участием фемтолазерных технологий: преимущества, недостатки и послеоперационный прогноз.

The article presents a literature review of the latest research in the field of ocular biomechani... more The article presents a literature review of the latest research in the field of ocular biomechanics with an accent on the role of biomechanical properties of anatomical structures in the development of ocular pathologies. Close attention is paid to biomechanical properties of the iris, the study of its structure and functioning in representatives of different races, as well as its role in anterior eye chamber angle closure and the pathogenesis of angle-closure glaucoma. Experimental and clinical researches of trabecular meshwork biomechanics and modern outlooks on its structure characteristics are described. The review provides information on theoretical developments and practical implications of the development of a new class of local hypotensive drugs that influence the trabecular meshwork tonus (Rho-kinase inhibitors).

Nowadays, a wide choice of local IOP-lowering medications allows ensuring a successful intraocula... more Nowadays, a wide choice of local IOP-lowering medications allows ensuring a successful intraocular pressure compensation and primary open angle glaucoma stabilization. However, taking into consideration a long term action of drugs on the ocular surface, even without clinical manifestations there is a direct inflammatory cells’ activation with mixed toxic and allergic changes, which is confirmed by histology, immune-histology, and impression cytology methods. Such chronic sub-clinical inflammation represent a potential risk of excessive fibroblast proliferation with subsequent rapid postoperative scarring of new outflow pathways and atypical filtration bleb formation. It is shown that there is a clear relationship between number of glaucoma drugs used, treatment duration, intensity of conjunctival infiltration with inflammatory cells and fibroblasts, and risk of episcleral fibrosis and subconjunctival scarring during post-op period. Vascular endothelial growth factor (VEGF) is a signaling protein, that controls vasculogenesis, a... more Vascular endothelial growth factor (VEGF) is a signaling protein, that controls vasculogenesis, angiogenesis, vascular support and stimulates permeability of small blood vessels. The following isoforms are presently known: VEGF-A, VEGF-B, VEGF-C, VEGF-В and PGF. VEGF-A, that regulates neoangiogenesis and fibroblast formation, is thought to play the most important role in human organism. Increased expression of VEGF may lead to development and aggravation of pathological conditions including oncology. The article presents a review of preclinical and clinical studies of the main VEGF-inhibitors - bevacizumab and ranibizumab, as well as a brief account on other existing medications of this group. It describes ophthalmological indications for the use of antiangiogenetic agents, as well as the ways of their possible off-label use.
