A case of a woman who is among the longest surviving people with Down Syndrome was described. The... more A case of a woman who is among the longest surviving people with Down Syndrome was described. The life expectancy of persons with Down syndrome has increased more than six-fold to 56 years since the turn of the century. The literature regarding life expectancy for persons with Down syndrome was reviewed, and the implications regarding Down syndrome and Alzheimer's disease were discussed. Down syndrome is the most frequent genetic or chromosomal cause of mental retardation, occurring in approximately 1 per 800 to 1 per 1,000 live births in developed countries (Baird & Sadovnick, 1989). The syndrome is characterized by trisomy 21, particular morphologic features, mental retardation, and associated health problems. Despite the unique clinical comorbidities among adults with Down syndrome, there are no clinical guidelines for the care of these patients. OBJECTIVE To develop an evidence-based clinical practice guideline for adults with Down syndrome. EVIDENCE REVIEW The Global Down Syndrome Foundation Medical Care Guidelines for Adults with Down Syndrome Workgroup (n = 13) developed 10 Population/Intervention/ Comparison/Outcome (PICO) questions for adults with Down syndrome addressing multiple clinical areas including mental health (2 questions), dementia, screening or treatment of diabetes, cardiovascular disease, obesity, osteoporosis, atlantoaxial instability, thyroid disease, and celiac disease. In an effort to better support the adults with Down syndrome that it serves, parents from the Nat... more In an effort to better support the adults with Down syndrome that it serves, parents from the National Association for Down Syndrome, a parent organization, worked with staff members from The Family Study and Service Program of the University Affiliated Program for Developmental Disabilities of the University of Illinois and the Department of Family Practice of Lutheran General Hospital to develop a clinic for adults with Down syndrome. The clinic was developed to provide comprehensive health care with an emphasis on preventative services. The multidisciplinary approach of the clinic has proved to be effective in the management of a variety of patient care needs. The most common health problems found were discussed.

The Center was established to provide comprehensive health care using a community-oriented primary care model. Twenty years later, the multidisciplinary approach of the Center has continued to be effective in the management of a variety of patient care needs. Quality of care indicators and patient satisfaction data are provided to assess the progress of the Center.

Resumen Entender lo que es normal o tipico para las personas con sindrome de Down ayuda a definir... more Resumen Entender lo que es normal o tipico para las personas con sindrome de Down ayuda a definir el continuum que va desde el comportamiento normal hasta el anormal. Este entendimiento proporciona un punto de referencia para la comprension del comportamiento de los adolescentes y de los adultos con sindrome de Down. Evaluar la conducta con la debida comprension del continuum presenta una serie de ventajas. Proporciona un marco para apreciar y reconocer las cualidades unicas y las caracteristicas conductuales comunes de las personas con sindrome de Down. Enfatiza la importancia del papel que desempena el entorno para apoyar a la persona con sindrome de Down en el desarrollo de su salud mental. Y tambien proporciona una estructura que ayuda a determinar cuando la conducta es anormal, y en consecuencia requiera una mayor intervencion y la aplicacion de un tratamiento.

In response to a request by the local parent group, a clinic was developed to provide for the med... more In response to a request by the local parent group, a clinic was developed to provide for the medical and psychosocial needs of adults with Down syndrome. One of the concerns of the parents was that their sons and daughters were not receiving an adequate evaluation when they had a decline in function and were being given a diagnosis of Atlzeheimer's disease. The thorough evaluation that each patient receives at the clinic is described. The majority of the adults seen showed no decline in function. Of those that showed a decline in function, a small percentage were diagnosed with Alzheimer's disease, but most of those with a decline had a potentially reversible problem and, with treatment, most returned to their premorbid level of function.

Prevalence of overweight among the general population has been reported to be 33% for males and 3... more Prevalence of overweight among the general population has been reported to be 33% for males and 36% for females. We undertook this study to establish overweight prevalence data in a cohort with Down syndrome and to stratify the incidence of overweight by living arrangement. We measured Body Mass Index (BMI) in 283 persons with Down syndrome and found a higher prevalence of overweight in this group compared to the general population. Individuals with Down syndrome living in a family setting had a higher incidence of overweight than did those living in a group home setting. Overweight prevalence among persons with Down syndrome should be considered a major public health concern that warrants further attention from researchers, practitioners, family members, and individuals with Down syndrome.

This paper presents a transcontextual model for intervening with families of underachieving schoo... more This paper presents a transcontextual model for intervening with families of underachieving school children. Following a brief review of the literature, the model is presented with specific guidelines for implementation. Finally, the results of a pilot study, using this model, indicate an improvement of general school functioning and of grade point averages in 81.3% of those surveyed in the study.
