Sebuah kondisi dimana panel surya menerima iridiasi yang berbeda yang dinamakan partial shading. Partial shading menyebabkan penurunan efisiensi daya keluaran PV dan dapat menimbulkan kerusakan pada PV. Untuk mencegah kerusakan terdapat dioda bypass namun adanya dioda bypass menyebabkan kurva karakterisistik daya-tegangan memiliki puncak lebih dari satu. Untuk mendapatkan daya maksimum digunakan metode Maximum Power Point Tracking (MPPT). Penggunaan algoritma Differential Evolution (DE) ditujukan untuk menghindari terjebak pada puncak daya yang lebih rendah dan lebih optimal dalam mendapatkan puncak daya maksimum. Introduction: Endotracheal intubation is a life saving procedure, the training of which require t... more Introduction: Endotracheal intubation is a life saving procedure, the training of which require to gain several competencies simultaneously. It becomes more challenging when we have to deliver the key concepts in a limited period of time. The medium fidelity simulator is a valuable tool of training for such scenarios. For this purpose we aim to compare the efficacy of structured training in endotracheal intubation between real life simulation and the conventional teaching method. Methods: The year 4 medical students rotating in anaesthesia for a period of 6 months from Jun - Dec 2009 were randomly divided into Gp A who had conventional teaching and Gp B who were taught by four simulated sessions of endotracheal intubation. Performance of both the groups was observed by a person blinded to the study against a checklist on a 7 point rating scale in anaesthetized patient. Results: Total 57 students, 29 in Gp A and 28 in Gp B were rotated in the anaesthesia during the study period. Evaluation of the individual component tasks revealed that simulated group achieved a significant difference in the scoring for laryngoscope and intubation technique. (P = 0.026, 0.012) The comparison of overall competence again showed that the 64.3% of student in Gp B achieved an excellent score in comparison to Gp A in which only 41.4% achieved excellent. (P =0.049). Similarly the lesser number of students in Gp B (14.3%) require remediation as compared to the Gp A, in which the requirement was 40% . Objective: To determine the frequency of conduction abnormalities and inhospital mortality in pat... more Objective: To determine the frequency of conduction abnormalities and inhospital mortality in patients with acute anterior wall myocardial infarction (MI).th Methodology: This study was conducted from 1 January to 30 June 2011 in the Cardiology Department of Lady Reading Hospital, Peshawar. Patients who presented with acute anterior wall MI were included. Those patients who were having renal failure, Hypo or Hyperkalemia, history of valve replacement,coronary artery bypass graft and those who were using Beta blocker, Lanoxinand Calcium channel blocker (Verapamil, Diltiazem) were excluded. Patients were followed daily for development of AV blocks until discharged or died. Data was analyzed using statistical package for social sciences (SPSS) version 15. Results: A total of 309 patients were included, out of which 228 (73.8 %) were males and 81 (26.2 %) were females, having mean age of 58.88 years.Conduction abnormalities developed in 33(10.67%) patients during hospital stay.Among these, A-V blocks were present in 3.5% and Intraventricular blocks in7.11% patients. During hospital stay, 56 (18.1%) patients died. LBBB developedin 6 patients in which, 3(50%) patients died whereas, RBBB developed in10patients in which 6(60%) patients died, Bifascicular block developed in 6 patientsin which 3 died and complete heart block (CHB) developed in 9 patients with77.7% mortality. Conclusion: CHB in anterior wall MI is an uncommon finding in this thrombolytic era but associated with high incidence of in-hospital death even if these patients are timely thrombolysed and paced.

It is now well established that at least one-half of the patients presenting with heart failure will have a normal left ventricular ejection fraction. Here we present a summery of the basic concepts for understanding the pathophysiology of HFPEF.

In the present study, bacterial and fungal agents involved in sudden death syndrome (SDS) in chil... more In the present study, bacterial and fungal agents involved in sudden death syndrome (SDS) in chillies and their relative contribution in the development of disease complex were investigated. Out of eight microbial agents isolated from wilted chilli plants, three pathogenic species viz. Ralstonia solanacearum, Fusarium oxysporum and Rhizoctonia solani were identified and characterized depending upon their morphological, cultural and biochemical behavior. These isolates induced wilting, when inoculated on susceptible chilli cultivar California Wonder under greenhouse conditions. All the pathogens induced wilt symptoms; however, the wilting was more rapid in case of R. solanacearum followed by F. oxysporum and R. solani. Disease symptoms were more prominent when three pathogens inoculated in combinations. We present the case of an elderly woman who was referred to cardiology for investigation of possible ischaemic heart disease prior to orthopaedic surgery. The patient had developed chest pain in the setting of a septic total knee replacement associated with changes on electrocardiography. Coronary angiography revealed multiple coronary arteriovenous fistulae associated with giant coronary artery aneurysm causing steal syndrome in the setting of haemodynamic stress.

Heart failure with preserved ejection fraction (HFPEF) is common and represents a major challenge... more Heart failure with preserved ejection fraction (HFPEF) is common and represents a major challenge in cardiovascular medicine. Most of the current treatment of HFPEF is based on morbidity benefits and symptom reduction. Various pharmacological interventions available for heart failure with reduced ejection fraction have not been supported by clinical studies for HFPEF. Addressing the specific aetiology and aggressive risk factor modification remain the mainstay in the treatment of HFPEF. We present a brief overview of the currently recommended therapeutic options with available evidence.
