Novel coronavirus disease-2019 (COVID-19) is a new respiratory disease that has spread widely thr... more Novel coronavirus disease-2019 (COVID-19) is a new respiratory disease that has spread widely throughout the world. On February 20, 2020, the first Italian case of COVID-19 was reported. The infection rapidly spread across the country, and by August 11, 2020, a total of 250 566 official cases with 32 205 deaths (12.8%) were reported in Italy, counting a total of 96 884 positive cases and 16 833 deaths (17.3%) in the Lombardy region only. A huge demand to handle the COVID-19 outbreak challenged both the health care providers and the ordinary work in the hospital. From the beginning of the crisis, San Raffaele Scientific Institute, a 1318-bed tertiary care university hospital located in Lombardy, Northern Italy, has played a major role in supporting the national health care system for the treatment of COVID-19 cases, and a significant reorganization of the hospital was immediately required. The reorganization was carried out both structurally and with regard to the distribution of medical and nonmedical staff. This article aims to highlight the management strategies for the health care staff subsequent to the pandemic intense workload in San Raffaele Scientific Institute.

Ethical challenges are traditionally described in a negative light, even though moral conflict ca... more Ethical challenges are traditionally described in a negative light, even though moral conflict can express the individual ability to perceive when something is not working and promote change. The true question, therefore, is not to how to silence moral conflict but how to educate it. Although the need for ethical support of health-and social-care professionals in elderly care is clearly perceived, there is no universal method for providing effective interventions. The authors hypothesize that adequate training sessions can help care professionals enhance this skill, once clear goals and specific educational techniques are set. This research tests the hypothesis on twenty care professionals working in acute and residential care settings for the elderly, building on the ethics round method. Mixed methods drawn from literature and the experience of educators were adapted to meet different educational goals. Moral issues can hardly be removed from a context characterized by increasing demand and decreasing resources, but they can be recognized and addressed with common efforts, a critical attitude, and a growth mindset. Enhancing these skills in qualified workers can help them accept the reality of work, release pressure, and identify common team goals. Introducing these skills before graduation can help future workers avoid unreal expectations and reduce frustration and early job quit rates.

Methods: This study was conducted in the interpretative paradigm using a hermeneutic phenomenological approach and data were analyzed following the principles of the Interpretative Phenomenological Analysis. Some evidence highlighted significant sex-related differences in self-care behaviors in this population. However, the literature on this topic has not been synthesized before. This study aimed to summarize, critically review, and interpret evidence regarding sex-related differences in adults' self-care behaviors with type 1 diabetes mellitus (T1DM). METHODS A systematic search of articles in the following databases was performed: PubMed, Scopus, Embase, and CINAHL. Articles published from 2009 to 2019 were reviewed. The Preferred Reporting Items for Systematic Reviews and Meta-Analyses statement and flowchart were used to support the systematic review and study reporting. Two reviewers independently screened and extracted data from the included articles. RESULTS From the 596 identified records, only 15 articles were included because pertinent to this review. Both females and males with T1DM must improve their ability to respond to signs and symptoms when they occur. Females are more likely to perform adequate blood glucose monitoring and adequate diet than males. In contrast, males are more likely to show more adequate levels of physical activity. CONCLUSIONS Clinicians should pay greater attention to supporting females with T1DM to enhance their behaviors for maintaining an adequate health status and glycemic control. Likely, greater attention is required to sustain males to monitor changes in their signs and symptoms. Self-care management should be improved in both sexes.

Background. Chronic renal failure is an epidemic in elderly patients. Older population have an in... more Background. Chronic renal failure is an epidemic in elderly patients. Older population have an increased prevalence of frailty and sarcopenia, associated with a wide range of adverse health outcomes such as falls, hospitalization, disability. Aim. Describe the sociodemographic and clinical variables of an elderly Lombard population and identify predictors of renal insufficiency. Materials and methods. Cross-sectional observational study conducted in hospitals, in recreational centers for the elderly, in the Universities of the Third Age of the provinces of Milan and Monza-Brianza conducted through a convenience sampling of 1250 subjects over the age of 65. Results. The study identified living alone, annual individual income < € 10,000, polypharmacy, sarcopenia and frailty as predictors of chronic kidney failure. The sample has a mean eGFR of 71.74 mL/min/1.73m2 (SD ± 16.56). Older people living alone are more likely to develop CRI (P = 0.031, confidence interval, CI [1.031-1.905]) as well as having an income < € 10,000 (P = 0.002, CI [0.392-0.923]). Taking more than 11 drugs a day increases the probability of having chronic renal failure by 16 times (P = 0.012, CI [1.155-3.16]). Sarcopenia and frailty increase the likelihood of having chronic renal failure (CRI) (P = 0.001, CI [1.198-2.095]). Conclusions. Identifying predictors of chronic kidney failure is a key step in introducing preventive measures and providing better care to the elderly population.

Objective To describe predictors of adjustment to living with an ostomy among Italian adults with... more Objective To describe predictors of adjustment to living with an ostomy among Italian adults with an enterostomy or a colostomy. Methods A multicenter, cross-sectional design was performed, sampling 403 patients with an ostomy in three different outpatient clinics of northern Italy between April 2018 and December 2020. Data were collected by stoma therapists in ambulatory settings using the Italian version of the Ostomy Adjustment Inventory-23 and patient medical records. Results Acceptance was lower among women, patients who underwent emergency surgery, those with a urostomy, and those with a body mass index of less than or equal to 25 kg/m2. Negative feelings were associated with higher body mass index, colostomies, shorter length of time of living with an ostomy, and emergency ostomy creation. What are the dietary, fluid intake and lifestyle indications for the disease management and recur... more What are the dietary, fluid intake and lifestyle indications for the disease management and recurrences prevention in patients with urolithiasis? Despite the progress in urolithiasis diagnosis and treatment, its incidence has increased in the last decades and a high rate of recurrence is still a real problem. Dietary and lifestyle modifications represent the most effective preventive measures. Recommendations and evidence available in literature are not always accurate and consistent, negatively affecting patients' education. A systematic review was conducted starting from a research in electronic databases; the study selection process was based on PRISMA indications. Methodological quality and risk of bias were assessed. Data were analysed in three phases: (1) critical review of articles; (2) data extraction; (3) evidence synthesis. Thirty-six studies were included: researchers used prevalently non-probabilistic sampling methods and data from several studies derived from small samples, mainly from Western countries. Despite these limitations, the below listed themes for urolithiasis prevention arose: (1) the importance of a prevalently vegetarian regime with a reduced intake of meats and fats, decreasing the risk of stones formation from the accumulation of uric acid; (2) water intake greater than two litres per day is proven to prevent recurrences and drinking beverages, such as decaffeinated coffee, tea, beer and wine, also seems to be associated with a decrease in risk; (3) being physically active and having a healthy lifestyle can help to prevent recurrences. Various dietary and lifestyle modifications have a potential to prevent urolithiasis and recurrences. Clinicians should advise patients to reduce animal proteins and fats, increase fruits and vegetables, and increase water intake, as well as having a healthy lifestyle. Further rigorous methodological studies are needed.

Testicular cancer incidence rates are increasing among young men, who typically demonstrate poor awareness and a lack of knowledge about this disease. Although testicular self‐examination might be an effective method for early detection, few people know about this process or regularly perform it. This lack of examination could delay the diagnosis, which would affect treatment outcomes and mortality rates. A literature review was conducted starting from a bibliographic research in the major databases, and 450 documents were identified. The final analysis included 19 studies and found that men feel anxious about testicular cancer, which leads to a delay in their diagnosis and treatment, especially in developing countries. The purpose of this article is to identify strategies that improve environmental sustainability, improve prevention of COVID-19 infection in dialysis centers, and improve the environmental impact of hemodialysis centers.

What are the dietary, fluid intake and lifestyle indications for the disease management and recur... more What are the dietary, fluid intake and lifestyle indications for the disease management and recurrences prevention in patients with urolithiasis? Despite the progress in urolithiasis diagnosis and treatment, its incidence has increased in the last decades and a high rate of recurrence is still a real problem. Dietary and lifestyle modifications represent the most effective preventive measures. Recommendations and evidence available in literature are not always accurate and consistent, negatively affecting patients' education. A systematic review was conducted starting from a research in electronic databases; the study selection process was based on PRISMA indications. Methodological quality and risk of bias were assessed. Data were analysed in three phases: (1) critical review of articles; (2) data extraction; (3) evidence synthesis. Thirty-six studies were included: researchers used prevalently non-probabilistic sampling methods and data from several studies derived from small samples, mainly from Western countries. Despite these limitations, the below listed themes for urolithiasis prevention arose: (1) the importance of a prevalently vegetarian regime with a reduced intake of meats and fats, decreasing the risk of stones formation from the accumulation of uric acid; (2) water intake greater than two litres per day is proven to prevent recurrences and drinking beverages, such as decaffeinated coffee, tea, beer and wine, also seems to be associated with a decrease in risk; (3) being physically active and having a healthy lifestyle can help to prevent recurrences. Various dietary and lifestyle modifications have a potential to prevent urolithiasis and recurrences. Clinicians should advise patients to reduce animal proteins and fats, increase fruits and vegetables, and increase water intake, as well as having a healthy lifestyle. Further rigorous methodological studies are needed.

Testicular cancer incidence rates are increasing among young men, who typically demonstrate poor awareness and a lack of knowledge about this disease. Although testicular self‐examination might be an effective method for early detection, few people know about this process or regularly perform it. This lack of examination could delay the diagnosis, which would affect treatment outcomes and mortality rates. A literature review was conducted starting from a bibliographic research in the major databases, and 450 documents were identified. The final analysis included 19 studies and found that men feel anxious about testicular cancer, which leads to a delay in their diagnosis and treatment, especially in developing countries.
