It is believed that motor performance improves when individuals direct attention to movement outc... more It is believed that motor performance improves when individuals direct attention to movement outcome (external focus, EF) rather than to body movement itself (internal focus, IF). However, our previous study found that an optimal individual attentional strategy depended on motor imagery ability. We explored whether the individual motor imagery ability in stroke patients also affected the optimal attentional strategy for motor control. Individual motor imagery ability was determined as either kinesthetic- or visual-dominant by a questionnaire in 28 patients and 28 healthy-controls. Participants then performed a visuomotor task that required tracing a trajectory under three attentional conditions: no instruction (NI), attention to hand movement (IF), or attention to cursor movement (EF). Movement error in the stroke group strongly depended on individual modality dominance of motor imagery. Epilepsy surgery has become one altemative in treating medically irremediable epilepsies . This w... more Epilepsy surgery has become one altemative in treating medically irremediable epilepsies . This was achieved partially due to the development of diagnostic techniques fbr fbci . To establish some standard guide lines , we studied the c ℃ nt status of the presurgical focus diagnostic protocol in the Japanese institutes where specialized epilepsy surgeries are performed . Since 2001 till 2005, the annual number of epilepsy surgeries was counted as 489, 620, 600, 553, and 595, respectively . In 2005, three institutes recorded more than 60 surgeries each . The distribution of 丘) ci was as Fc) llows;medial temporal 46% , 1ateral temporal 11 % , and frontal 23 % . Interictal SPECT was carried out in more than 80 % of thge cases . MEG was perfbrmed in about 50% of the cases , whereas this rate was more than 80% fbr parietal and occipital fbci . Ictal SPECT was performed in about 50% of neocortical cases but in only 10% of MTLE cases , This reflects the dificuty of this method in practice . Ictal EEG was considered to be a gold standard for f()cus dlagnosis and it was p 曲 ed in more than 80% of all cases . ln contrast , intracranial EEG was used in about 40 % of MTL cases and about 80% of neocortical fbci cases . This implies that many " skip " cases were found in MTLE because 丘)cus the diagnosis is easier here than in other locations . Subdural electrodes were used in most of the neocortical fbci cases , because of the need for f6cus diagnosis and fUnctional mapping using electrical stimulation at the same time . Postsurgical outcome was good (Class I 十 II : 90%)for medial temporal f6ci cases , whereas it was less than 80% fbr neocortical foci cases . These results are considered to be comparable to those reported in the US . Several representative cases were demonstrated to delinate the methodologies used . (

Background and Purpose— Increased blood–brain barrier (BBB) permeability, brain edema, and hemorr... more Background and Purpose— Increased blood–brain barrier (BBB) permeability, brain edema, and hemorrhage are important consequences of cerebral venous sinus thrombosis (CVST). The objective of this study was to define the role of the protein C pathway in the BBB permeability and edema elicited by experimental CVST. The role of neutrophil recruitment was also evaluated. Methods— Edema, BBB permeability, leukocyte-endothelial cell adhesion (LECA) and inflammatory cytokine levels were monitored in a murine model of CVST. The role of activated protein C (APC) was assessed in wild type mice (WT) receiving APC neutralizing antibody and in endothelial protein C receptor overexpressing mice (EPCR-tg). Neutrophil involvement was evaluated using an anti-CD18 antibody (Ab) and antineutrophil serum. Results— Brain edema and increases in BBB permeability and LECA were noted 48 hours after CVST. It translates the Site Of OperatiOn intO CTfMRI COOrdinateS , and prOVideS a SUrgeOn with COrreCt 3D − OrientatiOn dUring SUrgery 、 The use of it and its analogue system enhances the ability to detect the lesion and protect important neural tissue during surgery . We have used the neuronavigator fQr 206 cases in our clinic since 1989 . Here we present several utilities which are relatively peculiar in neurosurgical services . They are utilities in surgery close to the motor cortex , in epilepsy surgery , in stereotactic helnatoma aspiration , in combination with a TCD flow meter and in an anterior apProach to cervical spondylosis . We report a case of localized convexity hypertrophic pachymeningitis HP associated with aortitis.... more We report a case of localized convexity hypertrophic pachymeningitis HP associated with aortitis. HP is considered to be one type of IgG4 related systemic disease as well as aortitis. A 38 year old man with aortitis was admitted to our department due to generalized convulsive seizures. MRI revealed a localized mass like thickening of the convexity dura mater resembling meningioma. Because MRI showed rapid growth of the mass, diagnostic removal of the mass was performed to rule out the malignant tumor. Pathological examination showed hyalinization and fibrillation with infiltration of lymphocytes and IgG4 positive plasma cells. These findings suggest that our case is an IgG4 related systemic disease. However several combinations of IgG4 related diseases have been reported, reports of HP associated with aortitis are rare. In addition, this is the first case of localized convexity HP. We propose that resection of a symptomatic localized lesion is considered to be a therapeutic alternation for HP.

Various companies are trying to enter the welfare device and system. Successful cases of the attempt are rare. There are several problems. For example, it is difficult to sell to the user because it does not known the effect. Thus, the direction of development is hard to determined. Because there is no evaluation method is the main cause of those. Evaluation of welfare system and device are limited only stability, intensity and partial operability. So, evaluation of usefulness is insufficient. Therefore, we will attempt to establish the standard to evaluate usefulness for objectively and quantitatively on the basis of including non-verbal cognition. In this paper, we measure load of sitting and standing movement to use Electoromyogram (EMG) and 3D Motion Capture and set a goal to establish objective evaluation method. We think that establishing objective evaluation method is necessity to develop useful welfare device. We examined possibility of assessing load and fatigue from measuring brain activity to use Near Infra-Red Spectroscopy (NIRS). The idea of universal design is widespread in welfare device and system. Measuring requires verification of all generations. But, we performed to measure younger subjects as a first step. We think that younger subjects were observed the significant difference, because they had enough physical function. Considering younger subjects as a benchmark is appropriate for creating evaluation method.

Therefore, we will attempt to establish the standard for evaluation about usefulness for objectively and quantitatively for including non-verbal cognition. We examine the relationship between human movements and brain activity, and consider the evaluation method of welfare devices and systems to measure the load and fatigue which were felt by human. In this paper, we measure the load for sitting and standing movement using NISR. We tried to make sure for the possibility of the quantitatively estimation for physical or psychological load or fatigue by measuring of brain activity using NIRS (Near Infra Red Spectroscopy). As results, when subjects perform the movement task, the statistical significant difference was shown in the specific part of the brain region.

Dural arteriovenous fistulas (dAVFs) of an anterior cranial fossa are rare. Because of the high r... more Dural arteriovenous fistulas (dAVFs) of an anterior cranial fossa are rare. Because of the high risk of intracranial hemorrhage and relatively easy access for direct surgery, aggressive treatment has been recommended. The natural history of anterior cranial fossa dAVFs (ACF dAVFs) is unclear in spite of many reports for the natural history of general dAVFs. To treat ACF dAVFs, direct surgery has traditionally been performed and endovascular surgery has recently been introduced. A 74year-old man was transferred with severe consciousness disturbance and presented with devastating intracerebral hemorrhage on the CT scan. Digital subtraction angiography revealed the ACF dAVFs with a large venous pouch. The patient received direct surgery, nevertheless he became vegetative state. Later on, a smaller venous pouch was recognized on the CT scan when he had suffered from the thalamic hemorrhage sixteen months before. There are twelve cases including our case which was treated for a certain period and documented in detail. Eleven of twelve cases were asymptomatic. Three of the six cases with a venous pouch had some events possibly related to the disease, though none of the six cases without a venous pouch had any events during observation. In conclusion, an ACF dAVF with a venous pouch should be treated by direct surgery or endovascular surgery even if it is incidentally found. By contrast, careful observation might be a possible therapeutic option for an ACF dAVF without a venous pouch if there is mild reflux flow.

The final our purpose in this research is to contribute to developing of assistive robot and related-apparatus. In terms of developing a new system, we thought that it is important to examine behaviors as well as spatial recognition. Therefore, experiments have been performed for an examination of human spatial perceptions, especially right and left recognition, during car driving by using NIRS. In previous research, it has been documented that there were significant differences at dorsolateral prefrontal cortex at left hemisphere during virtual driving task and actual driving. In this paper, we performed measuring the brain activity during car driving by using NIRS. And we performed statistical analysis of the brain activity. The purpose of this paper is discovering the brain region which was involved in decision making when human drive a car and considering between human movement and brain activity during car driving.

It is necessary to discuss functions of humans, spatial perception, decision-making, and determining direction, for developing assistive systems. The final goal of our research is to contribute to developing of welfare robots with functions that are responsive like human. We measured brain activities during virtual car driving using NIRS. As a result of these experiments, there were significant differences at outside frontal cortex in left brain. This go-round, we measured brain activity during actual car driving. In general roads, experiments were performed by taking f-NIRS in the car, and measuring the brain activity when car driven by subjects was went through a number of intersections and road signs. In addition, there was significant difference in common regions. We report a significant association between car driving and brain activity.
