ABSTRACT Aims: To present a clinical report emphasizing the therapeutic approach done by a dentist with the purpose of improving the quality of life of patients with Sjögren's Syndrome. Case description: Female patient with Sjögren's Syndrome referred for prosthetic rehabilitation, presented with drying of the face, eyes, lips and of all the buccal mucosa. She complained of difficulty of speaking, chewing and swallowing. The buccal adequacy was carried through and related instructions to the alimentary diet were given. For rehabilitation of the type Class III of Kennedy edentulous space, a conventional partial removable prosthesis with extra coronary retainers was placed. Fluorine in form of daily mouth washing and a moisturizing oral gel two times a day were prescribed. A program of follow-up was delineated in order that the patient could successfully keep buccal health.

Research paper thumbnail of Alveolar bone loss and aging : A model for the study in mice

Journal of Periodontology, Nov 1, 2005

Several animal studies have shown a positive correlation between aging and alveolar bone loss (AB... more Several animal studies have shown a positive correlation between aging and alveolar bone loss (ABL). The purpose of this study was to develop a model for the study of ABL in mice and aging. Mucoperiosteal flap surgery (MFS) was performed on the buccal aspect of the left side of the mandible (BL) in 72 CF(1) Mus domesticus mice and divided into three groups as follows: males, breeding (non-virgin) females, and virgin females. The MFS was performed in 3-, 6-, 9-, and 12-month-old animals under anesthesia. The buccal aspects of right hemimandibles were used as controls (BR). Animals were sacrificed under anesthesia 21 days after surgery. Mandibles were removed, defleshed, stained with toluidine blue, and photographed in a microscope. The photographs were digitized, and ABL was measured as the exposed root surface area (mm(2)). Blinded measurements were performed using a computer-assisted image analysis system. In terms of alveolar bone loss, the BL (operated) area showed a significant difference (paired Student t test; P <0.001) when compared to the BR area in all three groups. Sex and breeding differences were not observed in this experiment. ABL in the left hemimandibles was significantly larger in 3- (mean: 0.70; 95% confidence interval [CI]: 0.59 to 0.80) and 12-month-old animals (mean: 0.58; 95% CI: 0.46 to 0.71) than in 6- (mean: 0.39; 95% CI: 0.33 to 0.46) and 9-month-old animals (mean: 0.42; 95% CI: 0.35 to 0.48); P <0.001. The results suggest that 3-month-old CF(1) mice, in a sex and breeding independent process, could be a useful model for provoked alveolar bone loss studies in aging.

However, an alternative protocol employing three implants and immediate loading has been developed. The objective of the present study was to assess the rehabilitation of edentulous patients treated with a complete fixed mandibular prosthesis with immediate loading and supported by three implants.MATERIALS AND METHODS: In this observational study, a total of 99 implants placed in 33 patients was evaluated after 18 months in use. Panoramic radiographs were digitized to measure bone loss at the mesial and distal surfaces of each implant.RESULTS: Mean peri-implant bone loss was 0.66 ± 0.51 mm for the left implant, 0.92 ± 0.61 mm for the central implant, and 0.82 ± 0.53 mm for the right implant. The bone loss observed around the distal implants was similar to that seen around the central implant, and there were no statistically significant differences in peri-implant bone loss between the three implant locations. There was no significant correlation between implant length and bone loss. The results were compatible with the peri-implant bone loss that has been described for prostheses of the same type supported by larger numbers of implants.CONCLUSIONS: The use of a complete fixed mandibular prosthesis with immediate loading supported by three implants proved to be an adequate option for the rehabilitation of edentulous patients. The protocol allowed simplification of treatment while maintaining similar bone loss to that reported for the same type of treatment supported by a larger number of implants.

As the population ages, the demand rises for health professionals that know the particularities of aging and of the care of aged people. It is not different for the dentists or dentistry itself. There are many physiological changes in mouth and maxillofacial tissues that are related to the aging.

Four samples of each system evaluated (Ball-Attachment [metal]; O-Ring, and ERA [plastic]) were prepared and underwent fatigue and retention force in a universal testing machine. Maximum retention force (N) was measured at 6 time points (from 0 to 36 months-0.5 mm/min). Two-way analysis of variance and the Tukey test were used (α = 0.05). The Ball-Attachment group showed the highest retention values, statistically greater than the other systems, without differences during the trial (42.75-44.75). The ERA group had intermediate values and a statistically significant decrease during the trial (20.75-14). The lowest retention forces were found for the O-Ring group (13.75-11.75), with no statistical differences during the trial. Retention forces are significantly affected by abutment type because the metal retainer was more retentive than plastics, possibly providing better fixation of the prosthesis in position, and more difficult to remove. After 6 months of simulation, retentiveness of both plastic components was similar. There was no fracture of any component.

Research paper thumbnail of Implant-supported palatal lift prosthesis in a patient with velopharyngeal incompetence: a case report

Gerodontology, 2012

Background: Velopharyngeal incompetence is characterized by the inability to contract the tissues of the soft palate. The most common causes are neuromuscular problems resulting from concussion and degenerative conditions of the central nervous system. Materials and methods: The treatment using palatal lift prosthesis is well established in the literature; however, reports on the use of osseointegrated implants are scarce. Rehabilitation using only this type of fibromucosal support is difficult in edentulous patients, since the prosthesis has a palatal lift extension. Results: The implants provided retention and stability to the prosthesis, improving swallowing and speech and contributing to improve the patient's quality of life. Conclusion: This article reports the rehabilitation of a patient with a totally edentulous maxilla with velopharyngeal incompetence using an implant-retained palatal lift prosthesis.
