The intrabiliary rupture of hydatid cyst of the liver is a severe and relatively frequent complic... more The intrabiliary rupture of hydatid cyst of the liver is a severe and relatively frequent complication. In this study six similar cases are presented. 4 of the cases had hydatid cyst surgery before the admission. The obstructive jaundice, cholangitis and unclosed bile fistula are the most frequent clinical findings. Ultrasound is very useful for the diagnosis. In the cases of space occupying lesions of the liver with wide choledochus (1.1 cm >) the intrabiliary rupture of hydatid cyst can be suspected by ultrasonographic examinations. Surgical therapy as the capitonnage combined with the resection of the projecting dome of the cyst and T tube application remains a method which gives good results and has low mortality and morbidity.

Between 1983 and 1986, 18 patients with colonic volvulus (94.4% sigmoid, 5.6% caecal) were seen a... more Between 1983 and 1986, 18 patients with colonic volvulus (94.4% sigmoid, 5.6% caecal) were seen at Karadeniz Medical Faculty. There were 11 male and 7 female patients, with a mean age of 68 years. In 66.6% and 22.2% of patients respectively a palliative procedure or a Hartmann operation were performed. The recurrence rate in the palliative group was 30%. The mortality rate for all patients with volvulus was 16.8%. The palliative procedures were successful in patients in good general condition and without bowel necrosis, whereas the Hartmann operation gave successful results in patients with bowel necrosis.

Thyroid-releasing hormone did not change the survival rate, serum amylase, glucose calcium, liver transaminases levels or the degree of pancreatic damage, but reduced lactate dehydrogenase. Our findings suggest that the use of thyroid-releasing hormone has no beneficial effect on the course of acute experimental pancreatitis.

The aim of this study was to investigate the influence of 178-estradiol (E 2) on acute necrotizin... more The aim of this study was to investigate the influence of 178-estradiol (E 2) on acute necrotizing pancreatitis (ANP) induced by glycodeoxycholic acid in rats. Rats were divided into six groups as sham + saline, sham + single dose E 2 (SDE z)' sham + multiple dose E, (MDE z), ANP + saline, ANP + SDE z' and ANP + MDE z. ANP in rats was induced by glycodeoxycholic acid. The extent of acinar cell injury, mortality, systemic cardiorespiratory variables, functional capillary density (FCD), renal/hepatic functions, and changes in some enzyme markers for pancreatic and lung tissue were investigated during ANP in rats. The induction of ANP resulted in a significant increase in the mortality rate, pancreatic necrosis, and serum activity of amylase, alanine aminotransferase (ALT), interleukin (IL)-6, lactate dehydrogenase (LDU) in bronchoalveolar lavage (BAL) fluid, serum concentration of urea, and tissue activity of myeloperoxidase (MPO) and malondialdehyde (MDA) in the pancreas and lung, and a significant decrease in concentrations of calcium, blood pressure, urine output, pOz' and functional capillary density (FCD). The use of E, did not alter these changes. E, demonstrated no effect on the course of ANP in rats. Therefore, it has no value in the treatment during acute pancreatitis.

The aim of this study was to evaluate the association between flow cytometric DNA ploidy and the ... more The aim of this study was to evaluate the association between flow cytometric DNA ploidy and the potential for invasiveness and lymph node metastasis in colorectal adenocarcinomas. Materials and Methods: The study included 40 colorectal carcinoma cases that were examined with flow cytometry and light microscopy. DNA ploidy in 4 different areas (deep tumor tissue, superficial tumor tissue, normal adjacent mucosa, and lymph node metastasis) was analyzed with flow cytometry and compared with invasiveness and lymph node metastasis Results: Statistically significant differences in DNA content were observed between the deep and superficial layers of tumor tissues. The rate of aneuploidy was similar in the deep region of the tumors and lymph node metastases. Conclusions: The results suggest that the level of aneuploidy observed in the deep invasive region of tumors was associated with lymph node metastases.

Catheter related sepsis rates were compared between indication for parenteral nutrition (10%) and central venous pressure measurement (18%). Intraoperative complications were not seen.

ABSTRACT To investigate the effects of the constitutive nitric oxide (NO) synthase inhibitor NG-n... more ABSTRACT To investigate the effects of the constitutive nitric oxide (NO) synthase inhibitor NG-nitro-L-arginine methyl ester (L-Name) and the inducible NO synthase inhibitor amino ethyl-isothiourea (AE-ITU) on acute necrotising pancreatitis (ANP) in rats. Laboratory study. Medical school, Turkey. Morbidity, mortality, effects on activities of various enzymes, and histological picture. NO inhibitors increased the mortality (from 8/15, 53%, for ANP plus saline, to 12/15, 80%, for ANP plus L-Name, and 13/15, 87%, for ANP plus AE-ITU and serum amylase activity, but had no effects on serum calcium concentrations, volume of ascites, or degree of pancreatic damage. L-Name caused hypoglycemia, and AE-ITU reduced activities of lactate dehydrogenase and liver transaminases, and concentrations of urea and creatinine. Constitutive NO synthase inhibition worsens the course of ANP, and inducible NO inhibition has beneficial effects on various systems.

Materials and Methods: We retrospectively analyzed 80 consecutive elderly patients over 65 years of age with significant comorbid conditions who underwent open cholecytectomy under epidural anesthesia for the surgical treatment of gallstone disease between January 1, 2009 and December 31, 2019, all performed by one surgeon. Results: Mean age was found 77 ± 16 years. Fifty of the patients (62.5%) were females. Forty-seven patients (58.75%) showed an American Society of Anesthesiologist Physical Status (ASA, PS) of 3. The most frequently associated comorbidity involved the cardiovascular system (46 patients, 57.5%). Surgical procedures may be risky due to potential comorbidities in advanced age. Our study was intended to evaluate the reliability of total thyroidectomy due to benign thyroid diseases in patients aged 50 and over. Material and method: Following approval from the Karadeniz Technical University Faculty of Medicine Ethical Committee, 150 patients aged between 20 and 80 on whom total thyroidectomy was performed due to benign thyroid diseases between August 2008 and August 2009 were prospectively enrolled. Patients were divided in two groups; age under 50 (Group 1) and age 50 and above (Group 2). The reliability of total thyroidectomy surgery in patients aged over 50 was investigated by analyzing the difference between the two groups in terms of complications arising. Results: Seventy-six of the patients included in the study were aged under 50 (Group 1), and 74 were aged over 50 (Group 2). Patients' demographic characteristics are given in Table 1. In terms of complications, wound site infection was determined in 1 of the 76 patients in Group 1, permanent hypocalcemia in 3, unilateral permanent RLN injury in Group 1. Wound site infection was determined in 1 of the 74 patients in Group 2, permanent hypocalcemia in 5 and unilateral permanent RLN injury in 1. V.A.S. (Visual Analog Scale) scoring was made in evaluation of the operation scar of the patients and the mean V.A.S. score was 10.11. In Group 1 the mean value was 9,36 and in Group 2 the score was 10.88. Conclusion: Thyroid cancer being endemic in the Eastern Black Sea region of Turkey and hyperthyroid-associated clinical findings being manageable has led to surgeons performing total thyroidectomy more frequently. Our study revealed that with the exception of wound healing, total thyroidectomy was similar in young and older patients in terms of morbidity and length of hospitalization. We consider that total thyroidectomy can also be performed safely in the elderly when indicated.

Left paraduodenal hernia (LPH) is a congenital condition which develops as a result of abnormal r... more Left paraduodenal hernia (LPH) is a congenital condition which develops as a result of abnormal rotation of the midgut. Generally, small intestines are wedged into the hernial sac leading to symptoms of ileus. In neglected cases, necrosis may be present and these cases may progress to fatal outcomes. In this article, we present a case of LPH which was detected on computed tomography in a patient admitted to the emergency department at our hospital with the complaints of abdominal pain who had no history of previous surgery. In laparotomy, jejunal and ileal segments which were inside the hernial sac were removed back to the abdominal cavity. The defect in the patient without any sign of necrosis was closed primarily.
