Primary pulmonary lymphoma (PCL) is rare and represents only 0.5-1% of the malignant lung tumour.... more Primary pulmonary lymphoma (PCL) is rare and represents only 0.5-1% of the malignant lung tumour. Pulmonary MALT lymphoma is the most common histological aspect. The pathophysiological mechanism remains poorly known, unlike other locations of MALT lymphoma, no pathogen has so far been associated with the lung localization of the disease. It is characterized by slow evolution and atypical clinical presentation which delays diagnosis. The form «pneumonic» on the imaging allows to evoke the diagnosis while the confirmation is based on the immunohistochemical study which detects the presence of positive CD20 cell. Therapeutic options include surveillance of indolent types of LPP, surgery in localized tumors, chemotherapy in diffuse forms. We report two observations of diagnosed pulmonary MALT lymphoma on a scanno-guided bronchial and pulmonary biopsy.

Bronchial carcinoid tumors develop from the Kulchitsky cells of the bronchial epithelium, which a... more Bronchial carcinoid tumors develop from the Kulchitsky cells of the bronchial epithelium, which are stem cells with neuroendocrine properties. These tumors are divided into two types: typical forms and atypical forms, the latter being much rarer, more aggressive, and having a much higher probability of recurrence and distant metastasis. We report herein a rare case of an atypical lung carcinoid tumor metastatic to the liver. The patient is a 79-year-old woman who presented with purely digestive symptoms evolving for two years, with loss of appetite and deterioration of her general condition. The radiological assessment showed a pulmonary lesion with secondary hepatic and osseous nodules. A hepatic biopsy was performed and morphological and immunohistochemical results were compatible with an atypical bronchial carcinoid tumor, metastatic to the liver and bone.

The condition can also be manifested with chest pain secondary to pneumothorax. While the definitive diagnosis is based on a histological evaluation (which is not often performed), a computed tomography scan shows findings, such as apical fibrosis and pleural thickening of the apical lobes, which help assess the diagnosis. We describe a case of PPFE diagnosed radiologically in a 69-year-old man. This case highlights that PPFE is a pathology that can go unnoticed for a long time, and patients might neglect the revealing symptoms such as coughing.

It is a major cause of infertility in low-income countries, and in many cases, it is asymptomatic and typically not diagnosed until the patient seeks medical advice for infertility. The infection is usually secondary to primary pulmonary TB via hematogenous or lymphatic dissemination, but sexual transmission through genital TB of the partner is also possible. We describe a rare case of isolated ovarian TB as a fortuitous diagnosis during in vitro fertilization (IVF) workup in a 40-year-old woman with primary sterility and no specific symptoms. This case highlights the importance of screening for TB before an IVF procedure in women with infertility, especially in countries with a high prevalence of this disease.

Malignant mesothelioma originates from mesothelial surface cells covering the serous cavities, and the pleura is its most common site. Malignant pleural mesothelioma (MPM) typically presents with pleural effusion and chest wall pain with wide pleural thickening at radiological investigation. Although the histological examination along with immunohistochemistry helps yield the diagnosis, clinicians and experts face many challenges in diagnosing malignant mesothelioma not only due to the rarity of the disease but also due to the similarities that the disease share with other malignancies. Here, we report a case of a 55-year-old male patient with a history of chronic asbestos work exposure for 12 years who initially presented with unexplained pleural effusion and chest wall pain and was lost to followup but came back later with a worsening clinical state. This case is specially presented to raise awareness against cases of unexplained pleural effusion and chest pain.

Introduction : Elles constituent un chapitre important de la pathologie du médiastin en raison du... more Introduction : Elles constituent un chapitre important de la pathologie du médiastin en raison du nombre de relais lymphatiques et en raison de la fréquence et des difficultés diagnostiques qu'elles posent. Matériels et méthodes: A travers une étude rétrospective portant sur 64 cas d'adénopathies médiastinales colligées durant 2 ans. Nous avons analysé les différents moyens de diagnostic couramment pratiqués ainsi que les différentes étiologies recensées. Résultats : L'analyse des résultats des bilans a permis de poser le diagnostic étiologique dans 50 cas, soit dans 78,12 % : 32 cas de métastases d'un cancer bronchique, 7 cas de lymphomes, 6 cas de sarcoïdose, 3 cas de tuberculose ganglionnaire et 2 cas de tuberculose multifocale. A travers cette étude nous allons essayer de mettre au point sur les moyens de diagnostic positif et étiologique utilisés ainsi que les étiologies les plus fréquemment retrouvées.

Pulmonary sarcomatoïd carcinomas are a heterogeneous group of poorly differentiated non-small cel... more Pulmonary sarcomatoïd carcinomas are a heterogeneous group of poorly differentiated non-small cell tumors with a sarcomatous component. On imaging, they appear as peripheral or central masses, sometimes excavated. We report two cases of pulmonary sarcomatoïd lung carcinoma. The first case involves a 73-yearold active smoker who presented with dyspnea. A computed tomography (CT) scan showed a large locally advanced left lower lobar tumor process. A CT-guided biopsy was performed and the histopathological examination concluded a pulmonary sarcomatoïd carcinoma. The second case involves a 52-year-old chronic smoker who presented with hemoptysis. CT pulmonary angiography showed an excavated right upper lobar tumor. Histologic work-up of the right upper lobectomy piece objectified a pulmonary sarcomatoïd carcinoma. Pulmonary sarcomatoïd carcinoma has a nonspecific appearance on imaging and should be a part of imaging differential diagnoses in front of a large, lobulated, highly invasive lung tumor with or without excavation.

Objectifs We aimed to compare the levels of D-dimer in diabetic and non-diabetic patients with COVID 19. A link between D-dimer and mortality has also been established. Materials A retrospective study was carried out at the University Hospital Center of Oujda (Morocco) from November 01st to December 01st, 2020. Our study population was divided into two groups: a diabetic group and a second group without diabetes to compare clinical and biological characteristics between the two groups. In addition, the receiver operator characteristic curve was used to assess the optimal D-dimer cut-off point for predicting mortality in diabetics. Introduction: le cancer du poumon est le cancer le plus fréquent chez l'homme au Maroc Oriental. ... more Introduction: le cancer du poumon est le cancer le plus fréquent chez l'homme au Maroc Oriental. Nous présentons ici le premier rapport sur les caractéristiques cliniques, pathologiques et thérapeutiques du cancer du poumon au Maroc Oriental. Méthodes: etude rétrospective sur 738 patients diagnostiqués avec un cancer du poumon au niveau du Centre d'Oncologie Hassan II entre octobre 2005 et décembre 2014. Résultats: parmi les cas étudiés, 671 étaient des hommes et 67 des femmes. Les fumeurs représentaient 95,01% des cas chez l'homme et 1,54% chez la femme. L'âge moyen au diagnostic était de 59,1 ± 11,9 ans. La majorité des patients (97%) étaient diagnostiqués à des stades avancés de la maladie. Parmi les 227 patients atteints d'adénocarcinome en stade avancé, le test moléculaire a été réalisé chez seulement 4 patients. En plus, aucun patient de notre série n'a bénéficié d'un traitement par la thérapie ciblée. Dans cette série, 20,46% des patients avaient moins de 50 ans. Comparé aux patients âgés de 50 ans et plus, les patients âgés moins de 50 ans présentaient une consommation de cannabis plus élevé (p<0,001) et un pourcentage d'adénocarcinome plus important (p<0,01), par contre, ils avaient une consommation tabagique moins importante (p<0,001), et un pourcentage plus faible de carcinome epidermoïde (p<0,01) et de CPC (p<0,05). Conclusion: contrairement aux pays occidentaux, le cancer du poumon est diagnostiqué tardivement, touche une population plus jeune au Maroc Oriental et l'accès aux tests moléculaires est très faible. Ces résultats justifient la nécessité d'établir un programme efficace de lutte contre le cancer du poumon au Maroc Oriental et faciliter l'accès aux tests moléculaires et aux nouveaux outils thérapeutiques.

It is not iatrogenic, due to a trauma or underlying lung disease. It is a rare condition affecting young adults and adolescents. The main risk of diagnostic error is due to the fact that features can be confused with those of aerodigestive fistula, sometimes leading to investigations and unnecessary and sometimes deleterious therapeutic measures. We here report a case of spontaneous, vomiting-induced pneumomediastinum in a young woman.

Usually on a plot of immunosuppression especially HIV infection, but it can also occur in immunocompetent children. This is a retrospective and prospective study over a period of 11 years between January 2004 and July 2015. During that period, 1 122 children are hospitalized Pneumo-pediatric department at the Moulay Youssef hospital in Rabat. Amongthem, 35 patients (3.11%) are achieved multifocal tuberculosis. We show through this work the frequency of multifocal tuberculosis in immunocompetent children and the different reported damages while noting the difficulty of diagnosis in front of clinical polymorphism

Pulmonary sarcomatoïd carcinomas are a heterogeneous group of poorly differentiated non-small cel... more Pulmonary sarcomatoïd carcinomas are a heterogeneous group of poorly differentiated non-small cell tumors with a sarcomatous component. On imaging, they appear as peripheral or central masses, sometimes excavated. We report two cases of pulmonary sarcomatoïd lung carcinoma. The first case involves a 73-yearold active smoker who presented with dyspnea. A computed tomography (CT) scan showed a large locally advanced left lower lobar tumor process. A CT-guided biopsy was performed and the histopathological examination concluded a pulmonary sarcomatoïd carcinoma. The second case involves a 52-year-old chronic smoker who presented with hemoptysis. CT pulmonary angiography showed an excavated right upper lobar tumor. Histologic work-up of the right upper lobectomy piece objectified a pulmonary sarcomatoïd carcinoma. Pulmonary sarcomatoïd carcinoma has a nonspecific appearance on imaging and should be a part of imaging differential diagnoses in front of a large, lobulated, highly invasive lung tumor with or without excavation.

Objectifs We aimed to compare the levels of D-dimer in diabetic and non-diabetic patients with COVID 19. A link between D-dimer and mortality has also been established. Materials A retrospective study was carried out at the University Hospital Center of Oujda (Morocco) from November 01st to December 01st, 2020. Our study population was divided into two groups: a diabetic group and a second group without diabetes to compare clinical and biological characteristics between the two groups. In addition, the receiver operator characteristic curve was used to assess the optimal D-dimer cut-off point for predicting mortality in diabetics. Introduction: le cancer du poumon est le cancer le plus fréquent chez l'homme au Maroc Oriental. ... more Introduction: le cancer du poumon est le cancer le plus fréquent chez l'homme au Maroc Oriental. Nous présentons ici le premier rapport sur les caractéristiques cliniques, pathologiques et thérapeutiques du cancer du poumon au Maroc Oriental. Méthodes: etude rétrospective sur 738 patients diagnostiqués avec un cancer du poumon au niveau du Centre d'Oncologie Hassan II entre octobre 2005 et décembre 2014. Résultats: parmi les cas étudiés, 671 étaient des hommes et 67 des femmes. Les fumeurs représentaient 95,01% des cas chez l'homme et 1,54% chez la femme. L'âge moyen au diagnostic était de 59,1 ± 11,9 ans. La majorité des patients (97%) étaient diagnostiqués à des stades avancés de la maladie. Parmi les 227 patients atteints d'adénocarcinome en stade avancé, le test moléculaire a été réalisé chez seulement 4 patients. En plus, aucun patient de notre série n'a bénéficié d'un traitement par la thérapie ciblée. Dans cette série, 20,46% des patients avaient moins de 50 ans. Comparé aux patients âgés de 50 ans et plus, les patients âgés moins de 50 ans présentaient une consommation de cannabis plus élevé (p<0,001) et un pourcentage d'adénocarcinome plus important (p<0,01), par contre, ils avaient une consommation tabagique moins importante (p<0,001), et un pourcentage plus faible de carcinome epidermoïde (p<0,01) et de CPC (p<0,05). Conclusion: contrairement aux pays occidentaux, le cancer du poumon est diagnostiqué tardivement, touche une population plus jeune au Maroc Oriental et l'accès aux tests moléculaires est très faible. Ces résultats justifient la nécessité d'établir un programme efficace de lutte contre le cancer du poumon au Maroc Oriental et faciliter l'accès aux tests moléculaires et aux nouveaux outils thérapeutiques.
