Diagnosis and measuring the level of increase in intracranial pressure (ICP) is critical, especia... more Diagnosis and measuring the level of increase in intracranial pressure (ICP) is critical, especially for the management of trauma patients in the emergency department and intensive care unit. However, measurements are operator-dependent as in all of the sonographic diagnoses. The aim of this study is to assess the operator variations in the measurement of optic nerve sheath diameter (ONSD). There were four emergency medicine specialists involved in the study. Each had at least 1 year of experience of ultrasound scans and performed at least 25 prior ocular scans examining the ONSD. Two measurements were made 1 week apart from both axial and longitudinal planes. Sixty healthy adults were involved in the study and every investigator obtained four measurements from each. Intra-interobserver reliabilities were tested. The investigators performed 60 ocular ultrasounds on individual healthy adults and obtained two measurements in axial and longitudinal planes 1 week apart. Therefore, 960 measurements were analyzed. The levels of compatibilities for most of the measurements were found at acceptable levels statistically. However, it is not possible to say that there was a perfect compatibility among the sonographers according to the previously conducted reliability studies of ultrasound measurements. According to our results, it is hard to say that sonographic measurement of the ONSD is a highly reliable method both in longitudinal and transverse planes.

The pressure of adsorbent bed is preferred to be constant during the isobaric adsorption and desorption processes to have constant evaporation and condensation temperatures. In the present study, the effects of desorption heating rate and insufficient condenser capacity on the variation of desorption pressure are investigated. The experiments were performed on an intermittent adsorption heat pump in laboratory environment. The results of the two cycles, one is the adsorption heat pump with a condenser having 0.038 m 2 heat transfer area and another is the same adsorption heat pump but with the condenser of 0.226 m 2 heat transfer area, are presented. Bu çalışmanın amacı 14-17 yaş grubu ergenlerin cinsel yolla bulaşan hastalıklar hakkındaki (CYBH)... more Bu çalışmanın amacı 14-17 yaş grubu ergenlerin cinsel yolla bulaşan hastalıklar hakkındaki (CYBH) bilgi düzeylerini ve bunu etkileyen faktörleri araştırmaktır. Gereç ve Yöntemler: Çocuk Polikliniğine başvuran 14-17 yaş arası, 527 ergene sosyodemografi k özellikleri ve CYBH hakkındaki bilgilerini sorgulayan anket verildi. CYBH Bilgi Testinde "doğru" cevaplara "1", "yanlış" ve "bilmiyorum" cevaplarına "0" puan verildi. Bulgular: Ergenlerin cinsel konuları en sık olarak (%34.0) sağlık görevlileri ile konuşmayı tercih ettikleri saptandı. "Son bir yıl içinde cinsel ilişkide bulundunuz mu?" sorusuna ergenlerin %9.3'ü "evet" yanıtı verdi. CYBH Bilgi Testi puan ortancası 5 olarak hesaplandı; 36 ergen "0" puan aldı. Yaş ile CYBH hakkındaki bilgi düzeylerinde istatistiksel anlamlı ilişki saptandı (p=0,001; r=0,232). Anne ve baba eğitim düzeyi, gelir düzeyi arttıkça bilgi düzeyinin arttığı saptandı (p<0,05). Cinsel ilişkide bulunduğunu belirten ergenlerde bilgi puanı diğer gruplara göre daha yüksek bulundu (p<0,05). Sonuç: Çalışmamızda, ülkemizde cinsel deneyim yaşının düşmekte olduğu, CYBH hakkında bilgi düzeylerinin düşük olduğu ve cinsel konular hakkında eğitimi sağlık çalışanlarından, okul ve görsel basın aracılığıyla almak istedikleri saptandı. Bu nedenle ergenlerin CYBH'dan korunmak ve sağlıklı cinsel davranış geliştirebilmeleri amacı ile üreme sağlığı-cinsel sağlık konularına örgün eğitim kapsamında daha fazla yer verilmelidir. Ayrıca ülkemizde sağlık birimlerinin içinde "ergen polikliniklerinin" kurulması önemli olabileceğini düşünmekteyiz.

We aimed to evaluate the diagnostic and preoperative localization capacity of 99 m Tc methoxyisob... more We aimed to evaluate the diagnostic and preoperative localization capacity of 99 m Tc methoxyisobutylnitrile (MIBI) parathyroid scintigraphy and ultrasonography (USG) in enlarged parathyroid glands in the primary hyperparathyroidism (pHPT), as well as the relationship between the success rate of these techniques and biochemical values. We retrospectively evaluated 39 patients with clinical and biological evidence of pHPT who referred to the university hospital for MIBI parathyroid scintigraphy. Patients were examined with USG and double-phase MIBI parathyroid scintigraphy for the detection of enlarged parathyroid glands. Preoperative serum intact parathyroid hormone (iPTH) levels, calcium (Ca), phosphate and alkaline phosphatase measurements were obtained. A total of 45 parathyroid lesions in 39 patients were reviewed. Thirty-four patients had a single adenoma and five patients with multi-gland disease had 11 abnormal parathyroid glands including three adenomas, whereas the remaining 8 glands showed hyperplasia. The overall sensitivities of MIBI parathyroid scintigraphy, USG, and combined techniques were 85.3%, 72.5% and 90.4%, respectively; the positive predictive values (PPV) were 89.7%, 85.2%, and 92.6%, respectively. The most successful approach for detection of enlarged parathyroid glands in hyperparathyroidism is the concurrent application of USG and MIBI parathyroid scintigraphy modalities. The concomitance of thyroid diseases decreases the sensitivity of both MIBI parathyroid scintigraphy and USG in enlarged parathyroid glands.

The adsorbent-adsorbate pair features for in order to be employed in the adsorption heat pumps are described. The adsorption heat pumps are compared with the vapor compression and absorption heat pumps. The problems and troubles of adsorption heat pumps are classified and researches to overcome the difficulties are discussed.

The influence of operation time of microwave oven on performance of the adsorption heat pump was studied. The performance criteria: coefficient of performance, specific cooling power and volumetric cooing power, were calculated for the designed and tested adsorption heat pump system. The regeneration of adsorbent bed was achieved very rapidly (35 min) by using microwave heating system. The poor thermal conductivity of adsorbent did not affect the periods of isosteric heating and isobaric desorption processes.

The heat and mass transfer equations for the adsorbent bed and the mass balance equation for the adsorbent granules are numerically solved to obtain the distributions of temperature, pressure, adsorptive density and adsorbate concentration in the adsorbent bed. The study is performed for the silica gel-water pair and for three different values of porosity as 0.1, 0.2 and 0.3. The distributions of temperature and adsorbate concentration are considerably influenced from the bed porosity. The adsorption period increases with the increase of the porosity value. The porosity affects the pressure and adsorptive density distributions at the beginning of the process and after a relatively short time, the averages of these dependent variables approach to the final equilibrium state.

The effects of coupling agents on the mechanical, morphological, and water sorption properties of... more The effects of coupling agents on the mechanical, morphological, and water sorption properties of luffa fiber (LF)/polypropylene(PP) composites were studied. In order to enhance the interfacial interactions between the PP matrix and the luffa fiber, three different types of coupling agents, (3-aminopropyl)-triethoxysilane (AS), 3-(trimethoxysilyl)-1-propanethiol (MS), and maleic anhydride grafted polypropylene (MAPP) were used. The PP composites containing 2-15 wt% of LF were prepared in a torque rheometer. The tensile properties of the untreated and treated composites were determined as a function of filler loading. Tensile strength and Young's modulus increased with employment of the coupling agents accompanied by a decrease in water absorption with treatment due to the better adhesion between the fiber and the matrix. The maximum improvement in the mechanical properties was obtained for the MS treated LF composites. The interfacial interactions improved the filler compatibility, mechanical properties, and water resistance of composites. The improvement in the interfacial interaction was also confirmed by the Pukanszky model. Good agreement was obtained between experimental data and the model prediction. Morphological studies demonstrated that better adhesion between the fiber and the matrix was achieved especially for the MS and AS treated LF composites. Atomic force microscope (AFM) studies also showed that the surface roughness of LFs decreased with the employment of silane-coupling agents.

Different alternative approaches to thoracotomy have been developed because of the considerable morbidity associated with the standard posterolateral incision. Methods. We studied a prospective, randomized, blinded study of 60 consecutive patients to compare surgical approcah time, postoperative pain (quantitated by narcotic requirements and the visual analogue scale), pulmonary function, shoulder strength, and range of motion between standard posterolateral (group I) and muscle-sparing (group II) thoracotomy techniques. Results. There were no differences in postoperative surgical time, pulmonary function, shoulder range of motion, mortality, or hospitalization time. There was significantly less postoperative pain in group II. In this group, narcotic requirement was less in the first 24 hours, and visual analogue scale scores were significantly lower (p < 0.05) throughout the first postoperative week. Muscle strength had returned to preoperative levels by 1 month in both groups. Morbidity was identical in the two groups with the exception of postoperative seromas. The prevalence of seroma was 16.6% in the musclesparing group. Conclusions. We conclude that the muscle-sparing incision may be a sensible alternative to a standard posterolateral thoracotomy.

Traumatic brain injury (TBI) is a common cause of death and disability throughout the world. A mu... more Traumatic brain injury (TBI) is a common cause of death and disability throughout the world. A multifunctional peptide adrenomedullin (AM) has protective effects in the central nervous system. We evaluated AM in an animal model as a therapeutic agent that reduces brain damage after traumatic brain injury. A total of 36 rats was divided into 3 groups as sham, head trauma plus intraperitoneal (ip) saline, and head trauma plus adrenomedullin ip. The diffuse brain injury model of Marmarou et al. was used. Blood samples were taken from all groups at the 1st, 6th and 24th hours for analysis of TNF-␣ (tumor necrosis factor-␣), IL-1␤ (interleukin-1␤) and IL-6 (interleukin-6) levels. At the end of the study (at the 24th hour) a neurological examination was performed and half of the rats were decapitated to obtain blood and tissue samples, the other half were perfused transcardiacally for studying the histopathology of the brain tissue. There were no statistically significant changes in plasma levels of IL-1␤, IL-6 and TNF-␣ relative to the sham group. Also, changes in tissue levels of malonedialdehyde, myeloperoxidase and glutathione were not statistically significant. However, neurological scores and histopathological examinations revealed healing. AM individually exerts neuroprotective effects in animal models of acute brain injury. But the mechanisms of action remain to be assessed.

The adsorption heat pumps are compared with the vapor compression and absorption heat pumps. The influence of operation time of microwave oven on performance of the adsorption heat pump was studied. The performance criteria: coefficient of performance, specific cooling power and volumetric cooing power, were calculated for the designed and tested adsorption heat pump system. The regeneration of adsorbent bed was achieved very rapidly (35 min) by using microwave heating system. The poor thermal conductivity of adsorbent did not affect the periods of isosteric heating and isobaric desorption processes.

The heat and mass transfer equations for the adsorbent bed and the mass balance equation for the adsorbent granules are numerically solved to obtain the distributions of temperature, pressure, adsorptive density and adsorbate concentration in the adsorbent bed. The study is performed for the silica gel-water pair and for three different values of porosity as 0.1, 0.2 and 0.3. The distributions of temperature and adsorbate concentration are considerably influenced from the bed porosity. The adsorption period increases with the increase of the porosity value. The porosity affects the pressure and adsorptive density distributions at the beginning of the process and after a relatively short time, the averages of these dependent variables approach to the final equilibrium state.

The effects of coupling agents on the mechanical, morphological, and water sorption properties of... more The effects of coupling agents on the mechanical, morphological, and water sorption properties of luffa fiber (LF)/polypropylene(PP) composites were studied. In order to enhance the interfacial interactions between the PP matrix and the luffa fiber, three different types of coupling agents, (3-aminopropyl)-triethoxysilane (AS), 3-(trimethoxysilyl)-1-propanethiol (MS), and maleic anhydride grafted polypropylene (MAPP) were used. The PP composites containing 2-15 wt% of LF were prepared in a torque rheometer. The tensile properties of the untreated and treated composites were determined as a function of filler loading. Tensile strength and Young's modulus increased with employment of the coupling agents accompanied by a decrease in water absorption with treatment due to the better adhesion between the fiber and the matrix. The maximum improvement in the mechanical properties was obtained for the MS treated LF composites. The interfacial interactions improved the filler compatibility, mechanical properties, and water resistance of composites. The improvement in the interfacial interaction was also confirmed by the Pukanszky model. Good agreement was obtained between experimental data and the model prediction. Morphological studies demonstrated that better adhesion between the fiber and the matrix was achieved especially for the MS and AS treated LF composites. Atomic force microscope (AFM) studies also showed that the surface roughness of LFs decreased with the employment of silane-coupling agents.

Different alternative approaches to thoracotomy have been developed because of the considerable morbidity associated with the standard posterolateral incision. Methods. We studied a prospective, randomized, blinded study of 60 consecutive patients to compare surgical approcah time, postoperative pain (quantitated by narcotic requirements and the visual analogue scale), pulmonary function, shoulder strength, and range of motion between standard posterolateral (group I) and muscle-sparing (group II) thoracotomy techniques. Results. There were no differences in postoperative surgical time, pulmonary function, shoulder range of motion, mortality, or hospitalization time. There was significantly less postoperative pain in group II. In this group, narcotic requirement was less in the first 24 hours, and visual analogue scale scores were significantly lower (p < 0.05) throughout the first postoperative week. Muscle strength had returned to preoperative levels by 1 month in both groups. Morbidity was identical in the two groups with the exception of postoperative seromas. The prevalence of seroma was 16.6% in the musclesparing group. Conclusions. We conclude that the muscle-sparing incision may be a sensible alternative to a standard posterolateral thoracotomy.

Traumatic brain injury (TBI) is a common cause of death and disability throughout the world. A mu... more Traumatic brain injury (TBI) is a common cause of death and disability throughout the world. A multifunctional peptide adrenomedullin (AM) has protective effects in the central nervous system. We evaluated AM in an animal model as a therapeutic agent that reduces brain damage after traumatic brain injury. A total of 36 rats was divided into 3 groups as sham, head trauma plus intraperitoneal (ip) saline, and head trauma plus adrenomedullin ip. The diffuse brain injury model of Marmarou et al. was used. Blood samples were taken from all groups at the 1st, 6th and 24th hours for analysis of TNF-␣ (tumor necrosis factor-␣), IL-1␤ (interleukin-1␤) and IL-6 (interleukin-6) levels. At the end of the study (at the 24th hour) a neurological examination was performed and half of the rats were decapitated to obtain blood and tissue samples, the other half were perfused transcardiacally for studying the histopathology of the brain tissue. There were no statistically significant changes in plasma levels of IL-1␤, IL-6 and TNF-␣ relative to the sham group. Also, changes in tissue levels of malonedialdehyde, myeloperoxidase and glutathione were not statistically significant. However, neurological scores and histopathological examinations revealed healing. AM individually exerts neuroprotective effects in animal models of acute brain injury. But the mechanisms of action remain to be assessed.
