To evaluate the therapeutic effect of metformin on the clinical features, hormonal and metabolic ... more To evaluate the therapeutic effect of metformin on the clinical features, hormonal and metabolic profile in obese women with a hyperinsulinemic polycystic ovary syndrome (PCOS). We analyzed 25 women with PCOS, mean age 27.28+/-7.85 years, BMI 34.42+/-6.61 kg/m2, FerrimanGallwey (F/G) score 17.43+/-5.45 and duration of menstrual cycle 79.2 +/- 54.5 days. Basal hormone measurements included: FSH, LH, total testosterone, DHEAS, 17 alpha OHP and E2. All patients underwent a 75 g oral glucose tolerance test (OGTT), during which fasting and stimulated levels of glucose and insulin were measured at 0', 60' and 120'. The lipid profile was also performed. Both basal and stimulated parameters were analyzed before and after treatment with metformin (500 mg orally, three times daily for 9 months). Alström syndrome (ALMS) is an autosomal recessive disorder characterized by multiple organ involv... more Alström syndrome (ALMS) is an autosomal recessive disorder characterized by multiple organ involvement, including progressive cone-rod dystrophy, sensorineural hearing loss, childhood obesity, and type 2 diabetes mellitus. Pathogenic variants in the ALMS1 gene are the known cause for the occurrence of this devastating condition. Here we report on a 12 year old boy referred to the University Clinic with early signs of impaired hearing and vision, obesity, and scoliosis. Central vision was first affected, followed by peripheral vision. In addition, his weight began increasing after the age of two years, reaching 78 kg at a height of 157 cm (BMI 31.64). No polydactyly was present. His mental development was normal in spite of his hearing and vision impairments. There was acanthosis nigricans on the neck. ECG and the cardiac ultrasound were normal. Subjects and Methods: We conducted a prospective, cross-sectional study, which included 107 consecutive hospitalized patients with T2DM, aged 35-65 years, over the 6-month period. Patients were examined and tested for diabetic foot and classified according to the IWGDF into four groups. The presence of kidney disease was assessed based on clinical practice guideline for diabetes and chronic kidney disease and classified into grades [1-5] based on the estimated glomerular filtration rate (eGFR) calculated with Cockcroft-Gault and modification of diet in renal disease formula. One important factor in sustaining the company's existence in advancing quality and performance m... more One important factor in sustaining the company's existence in advancing quality and performance management is the mastery of capabilities possessed by human resources and the clarity of career development. A clean and healthy company is certainly due to good employee performance. Employee performance will improve if employees are given the training to improve their abilities and also a clear career development program. This study aims to determine the effect of training and career development on employee performance. The unit of analysis in this study is individual employees of academic civities. The population in this study is all employees of academic civities which amounted to 63 employees. The sampling technique in this study was carried out with saturated sampling technique. The data analysis technique used is multiple linear analysis. The results showed that training and career development both partially and simultaneously had a significant effect on employee performance. This is an open access article under the CC-BY-SA license.

CONCLUSIONS: In our experience proteinuria is a late onset adverse event during cabozantinib trea... more CONCLUSIONS: In our experience proteinuria is a late onset adverse event during cabozantinib treatment, that occurs also in patients without risk factors for renal disease. Proteinuria seems to be associated with a better response to anti-angiogenetic therapy.We are now evaluating wheather proteinuria can be related to some molecular markers that could identify patients who are candidates to develop such toxicity in order to intervene early.

Diabetes is an issue that society is struggling with for a very long time. The ease with which ECG signals can be recorded and interpreted provides an opportunity to use Deep Learning techniques to predict the estimated Sugar Levels of a patient. This research aims at describing a Deep Learning approach to provide models for different short-term heart rate variability measurements. Our approach is based on a special method to calculate Heart Rate Variability (HRV) with identification of segments, then averaging and concatenating them to exploit better feature engineering results. The short-term HRV is used for the determination of instantaneous plasma glucose levels. The Deep Learning method is based on Autokeras, the neural architectural search provided the best results for the 15-minute measurements. Our research question is to develop a solution to estimate the Instantaneous glucose value from heart rate variability with sufficient quality. The evaluated test set gave the following results: RMSE(0.368), MSE(0.193), R square(0.513), and R squared loss(0.541).

Background: The aim of the study is to describe glycemic and insulin outcomes by trimester and ma... more Background: The aim of the study is to describe glycemic and insulin outcomes by trimester and maternal and fetal outcome in patients with type 1 diabetes using an insulin pump with constant or intermittent continuous glucose monitoring (CGM). Methods: Twenty-five women with type 1 diabetes with newly diagnosed pregnancy were treated with insulin pump therapy (Medtronic 722, Medtronic Minimed, Northridge, CA) for at least 1 year. Insulin pump and CGM (Medtronic Paradigm Real-Time) were implemented at least 3 months before conception. Patients were randomized in two groups: constant CGM group, 12 patients on insulin pump with glucose sensor, 24 h/day; and intermittent CGM group, 13 patients on insulin pump with intermittent glucose sensor, 14 days/month. The following parameters were analyzed: glycosylated hemoglobin (HbA1c), mean blood glucose, insulin requirement (in IU/kg/day), weight gain, severe hypoglycemic events, diabetic ketoacidosis, macrosomia, cesarean section, and neonatal hypoglycemia. Results: Both groups achieved good glucose control during their pregnancies (P < 0.05): 6.78-1.3% and 6.92-0.9% at the beginning of the study compared with 6.14-0.9% (constant CGM group) and 6.23-0.6% (intermittent CGM group) at the end of the study (last HbA1c before delivery). There was no significant decrease of HbA1c between the two groups. The constant CGM group had a significantly lower A1c in the first trimester compared with the intermittent CGM group. Maternal and fetal outcome did not show a significant difference between the two groups. Conclusions: Insulin pump therapy together with constant or intermittent CGM can improve diabetes control and pregnancy outcome in type 1 diabetes. The quality of the glucose profile at conception was the important factor for pregnancy outcome.

AIM: To estimate suspect nodule for benign or malignant characteristics, and to verify cytologica... more AIM: To estimate suspect nodule for benign or malignant characteristics, and to verify cytological features of the node with the fine needle aspiration (FNA) under ultrasound. DESIGN: A total of 106 patients were analyzed. FNA biopsy was performed at outpatient clinic via ultrasound. Inform consent was signed for each patient. Preparation of procedure with local anaesthesia was made by assistant nurse. PROCEDURE: Parallel approach of ultrasound guided fine needle aspiration (USGFNA) was used for each patient. This approach allows the operator to observe needle penetration, location and pathway of the entire needle within the neck, thyroid and nodule, which remain visible on the monitor. Introduction Le pied diabetique est une lesion dans le pied ou membres inferieurs, qui est causee... more Introduction Le pied diabetique est une lesion dans le pied ou membres inferieurs, qui est causee par une angiopathie diabetique et de l’infection neurologique. C’est une des complications les plus graves du diabete. Patients et methodes Le but d’etude se d’evaluer la correlation entre les scores de risque d’ulceration du pied chez les malades atteints de diabete de type 2 et le degre de maladie renale chronique (MRC). De comparer ces risques et leur impact sur les facteurs metaboliques ainsi que les facteurs qui influencent le temps d’identification de risque d’ulceration du pied et le degre de MRC. Resultats Il s’agit d’une etude transversale, dans lequel, pendant la visite 1 (V1), le nombre total de patients atteints de DM2 et MRC ont ete examines ainsi que 6 mois plus tard durant la visite 3 (V3). Les malades ont ete examines et testes pour le pied diabetique et classifie selon le groupe de travail international (IWGDF) dans trois groups : risque moyenne, haut risque et tres haut risque. De plus les malades ont ete examines et classifies pour le degre de MRC selon la formule de Cockroft–Gault et MDRD. Le nombre total des malades qui ont entre dans l’etude est 107, et parmi 50,5 % etaient des hommes et 49,5 % etaient des femmes. L’âge moyen des malades etait 59,12 ans, et moyen niveau d’HbA1c etait 9,5 % et six mois plus tard le niveau d’HbA1c etait 8,8 % (p Discussion Chez les malades d’âge moyen atteints de DM2 et MRC, la duree de DM2, HbA1c et la pression arterielle elevee ont un role important dans l’evaluation du risque d’ulceration du pied et la progression de la MRC. Conclusion Les patients presentant un risque eleve pour les scores d’ulceration, se rapportent a un degre de nephropathies superieur.

Introduction Le pied diabetique est une lesion dans le pied ou membres inferieurs, qui est causee... more Introduction Le pied diabetique est une lesion dans le pied ou membres inferieurs, qui est causee par une angiopathie diabetique et de l’infection neurologique. C’est une des complications les plus graves du diabete. Patients et methodes Le but d’etude se d’evaluer la correlation entre les scores de risque d’ulceration du pied chez les malades atteints de diabete de type 2 et le degre de maladie renale chronique (MRC). De comparer ces risques et leur impact sur les facteurs metaboliques ainsi que les facteurs qui influencent le temps d’identification de risque d’ulceration du pied et le degre de MRC. Resultats Il s’agit d’une etude transversale, dans lequel, pendant la visite 1 (V1), le nombre total de patients atteints de DM2 et MRC ont ete examines ainsi que 6 mois plus tard durant la visite 3 (V3). Les malades ont ete examines et testes pour le pied diabetique et classifie selon le groupe de travail international (IWGDF) dans trois groups : risque moyenne, haut risque et tres haut risque. De plus les malades ont ete examines et classifies pour le degre de MRC selon la formule de Cockroft–Gault et MDRD. Le nombre total des malades qui ont entre dans l’etude est 107, et parmi 50,5 % etaient des hommes et 49,5 % etaient des femmes. L’âge moyen des malades etait 59,12 ans, et moyen niveau d’HbA1c etait 9,5 % et six mois plus tard le niveau d’HbA1c etait 8,8 % (p Discussion Chez les malades d’âge moyen atteints de DM2 et MRC, la duree de DM2, HbA1c et la pression arterielle elevee ont un role important dans l’evaluation du risque d’ulceration du pied et la progression de la MRC. Conclusion Les patients presentant un risque eleve pour les scores d’ulceration, se rapportent a un degre de nephropathies superieur.

Contrarily, there is a proven benefit of the rapid decrease of triglycerides. Although the exact mechanism is unknown, a lot of studies showed that lipoprotein lipase stimulated via insulin reduces triglyceride values. Aim: Our aim of the study is to prove the treatment benefits with fluids and intravenous rapid insulin in patients with type 2 diabetes and their role of decreasing the risk of acute pancreatitis due to high TG (above 5,6 mmol/L/500 mg/dL) and very high TG (above 11.3 mmol/L/1000 mg/dL), as a result of lipoprotein lipase activity.
