Background To achieve the goal of United Nations of elimination of new HIV infections, a program ... more Background To achieve the goal of United Nations of elimination of new HIV infections, a program of prevention of mother-to-child transmission (PMTCT) was launched in Guangdong province. The objective of this study is to evaluate the effectiveness of the PMTCT program. Methods The retrospective cross-section analysis was conducted using the data of case reported cards of HIV positive mothers and their infants from 2007 to 2010 in Guangdong province, and 108 pairs of eligible subjects were obtained. We described the data and compared the rates of MTCT by various PMTCT interventions respectively. Results The overall rate of HIV MTCT was 13.89% (15) among 108 pairs of HIV positive mothers and their infants; 60.19% (65) of the mothers ever received ARVs, 80.56% (87) of infants born to HIV positive mothers ever received ARVs, but 16.67% (18) of the mothers and infants neither received ARVs. The ideal treatment is still controversial, ranging from radical surgeries to the application of chemotherapy (QT) with intracystic interferon-alpha (IFN-a). OBJECTIVE: To describe a series of cases of pediatric patients diagnosed with adamantinomatous craniopharyngioma treated using three therapeutic possibilities: total resection, partial resection followed by radiotherapy (RT) or QT with IFN-a. METHODS: Description of a series of cases, between 2000 and 2017, with 17 children diagnosed with craniopharyngioma submitted to current treatments. RESULTS: Of the 17 patients, there were 52.9% girls, aged 9 months to 14 years. There were 76.5% of patients diagnosed with hydrocephalus and the main symptoms were: headache (64.7%), anopsias (41.2%), amaurosis (41.2%) and hormonal changes (94.1%). The expression of EBV gene products was studied by in situ hybridization for EBER. EBV positive tumor cells were found in 87% (47/54) pediatric BL. An unexpected expression of latent membrane protein (LMP)-1 of EBV was observed in the neighborhood of Schistosoma mansoni granulomas and scars. The frequency of EBV infection was higher in pediatric (87%) than in adulthood HL (55%) in the same area (p <0,005). The EBV-infection was related more frequently to LNH occurring in AIDS patients than in immunocompetent patients (p=0.002). The overall EBV infection rate in Bahian and German tonsils was similar (50 per cent of Bahian and 45 per cent of German tonsils). However, a significantly higher number of EBERpositive lymphoid cells were found in the germinal centers (GCs) of 8/20 EBV-positive tonsils from Brazil), while only 3/18 tonsils from Germany displayed a few EBER positive cells (1-9 cells/GC; mean: 0.5 cell/GC per case) in this compartment (p < 0.007). In addition, in two cases the EBV-infected GC cells in Bahian samples expressed an oncogenic protein, the EBV-encoded latent membrane protein (LMP)-1, findings not present in German samples. In conclusion, we shown a high frequency of EBV-associated lymphoma in pediatric and AIDS patients and therefore EBV-infection may play a major role in the lymphomagenesis in these groups. Since BL and HL are derived from GC cells, the similar rate of EBV-infection in pediatric BL and HL corroborate the hypothesis that the pattern of EBV-infection in GC may be related to the development of these lymphomas in developing areas. O objetivo desta pesquisa foi investigar as características sócio-demográficas e clínico-patológi... more O objetivo desta pesquisa foi investigar as características sócio-demográficas e clínico-patológicas dos linfomas não-Hodgkin (LNH) em crianças e adolescentes no estado da Bahia. Do ponto de vista da metodologia, trata-se de estudo descritivo com dados secundários de 111 pacientes com LNH, procedentes de dois centros de referência. Critérios de inclusão: idade <20 anos, residência na Bahia e admissão no período de janeiro de2000 a dezembro de 2005, com diagnóstico anátomo-patológico de LNH. Obtiveram-se os seguintes resultados: dos 111 pacientes, 76 (68,5%) apresentaram idade entre cinco e 14anos, com uma média de 8,6±4,3 anos. Linfomas de Burkitt (LB) predominaram (56,8%), seguidos pelos linfomas linfoblásticos e linfomas de grandes células, cada um com 23 casos (20,7%). Laboratory tests obtained after fasting revealed elevated serum gastrin, supporting the suspected diagnosis of gastrinoma. In the endoscopy an elevated lesion with central depression was observed. The immunohistochemical examination revealed the benign nature of the tumor and the hyperplasia of argentaffin cells. Gastrinoma is a rare disease that predominantly affects young adults, but it must be considered in the pediatric group when clinical and laboratorial features of this disease are observed.

this study aimed to determine the prevalence and characteristics of gastrointestinal manifestatio... more this study aimed to determine the prevalence and characteristics of gastrointestinal manifestations on initial clinical presentation of acute leukemias (AL) in childhood. this is a retrospective and descriptive study that assessed medical records of 354 patients with AL from January 1995 to December 2004. acute lymphoid leukemia (ALL) was diagnosed in 273 (77.1%) patients and acute non-lymphocytic leukemia (AML) in 81 (22.9%). There were 210 males (59.4%) and 144 females (40.6%). The most common presenting features were: abdominal pain (19.5% in ALL and 11.8% in AML), nausea and vomiting (14.9 in ALL and 14% in AML), abdominal distention (18.5 in ALL and 8.6% in AML; p 0.024), constipation (5% in ALL and 6.5% in AML), diarrhea (3.6% in ALL and 11.8% in AML; p 0.03%), and gastrointestinal bleeding (7.9% in ALL and 9.7% in AML). Objective: This study was to determine the prevalence and characteristics of the osteoarticular m... more Objective: This study was to determine the prevalence and characteristics of the osteoarticular manifestations on initial clinical presentation of acute leukemias (ALs) on childhood in the state of Bahia, Brazil. Materials and Methods: This retrospective study assessed the medical records of 406 patients with AL from January 1995 to December 2004. Results: Acute lymphocytic leukemia (ALL) was diagnosed in 313 (77.1%) patients and acute myeloid leukemia (AML), in 93 (22.9%) patients, including 241 males (59.4%) and 165 females (40.6%). Age ranged from 9 months to 15 years (average: 6.18 y). The most common presenting features were fever (18.5%), musculoskeletal diffuse tenderness (15.0%), pallor (11.4%), and leg tenderness (5.7%). Prior referral to our center, the most frequent initial diagnosis was anemia (15.8%), leukemia (15.0%), amygdalitis (3.7%), and rheumatic fever (2.7%). Osteoarticular manifestations were found on 54.7% of the patients with AL, with a higher frequency among patients between 1 and 9 years of age (58.7%, P = 0.0007). The presence of joint tenderness (16.2% in ALL Â 5.4% in AML), arthritis (26.6% in ALL Â 9.7 in AML), bone tenderness (26.1% in ALL Â 16.1% in AML), limb tenderness (49.5% in ALL Â 25.8% in AML), and antalgic gait (32.8% in ALL Â 9.7% in AML) had higher prevalence on ALL. The large joints, chiefly the knees (10.6%), ankles (9.4%), elbows (4.4%), and shoulders (3.6%) were more often affected. Conclusions: AL should be considered on the differential diagnosis of osteoarticular symptoms of unknown etiology in children.

Despite the use of race in clinical and epidemiologic research, its characterization remains unclear, specially for those multiracial communities. Both racial mix and discordance about the concept of race make it difficult to classify and analyse emerging diseases and their pattern since biological concept of pure race is not well established. This brings to our mind some questions about social issues, such as education, economics, culture, politics, religion, ethnic, racism, and others. The literature about race and ethnic related to health was reviewed and the importance of this theme was commented. The influence of race in the risk of become ill and in the pattern of illness were reported. Cancer is reported apart. Special comments are done about childhood cancer. For all disease, available data show that race does not matter biologically, but the literature on patterns of care also shows that, in some cases, race does matter.

BACKGROUND: Metronomic chemotherapy (MC) consists of the administration of a low dose of chemothe... more BACKGROUND: Metronomic chemotherapy (MC) consists of the administration of a low dose of chemotherapy on a daily or weekly basis without a long break to achieve an antitumoral effect through an antiangiogenic effect or stimulation of the immune system. The potential effect of MC with continuous oral cyclophosphamide and methotrexate in patients with high-grade operable osteosarcomas (OSTs) of the extremities was investigated. METHODS: Patients with high-grade OSTs who were 30 years old or younger were eligible for registration at diagnosis. Eligibility for randomization included 1) nonmetastatic disease and 2) complete resection of the primary tumor. The study design included a backbone of 10 weeks of preoperative therapy with methotrexate, adriamycin, and platinum (MAP). After surgery, patients were randomized between 2 arms to complete 31 weeks of MAP or receive 73 weeks of MC after MAP. The primary endpoint was event-free survival (EFS) from randomization. RESULTS: There were 422 nonmetastatic patients registered (May 2006 to July 2013) from 27 sites in 3 countries (Brazil, Argentina and Uruguay), and 296 were randomized to MAP plus MC (n 5 139) or MAP alone (n 5 157). At 5 years, the EFS cumulative proportions surviving in the MAP-MC group and the MAP-alone group were 61% (standard error [SE], 0.5%) and 64% (SE, 0.5%), respectively, and they were not statistically different (Wilcoxon [Gehan] statistic 5 0.724; P 5.395). The multivariate analysis showed that necrosis grades 1 and 2, tumor size, and amputation were associated with shorter EFS.

From May 1999 to October 2009, 579 participants were enrolled in the Brazilian GCT-99 study. Treatment, defined as specific chemotherapy regimen and number of cycles, was allocated by means of risk-group assignment at diagnosis with consideration for stage and primary tumor site. Patients at low risk received no chemotherapy. Patients at intermediate risk (IR) with a good response (GR) received four cycles of platinum and etoposide (PE), for total doses of platinum 420 mg/m(2) and etoposide 2,040 mg/m(2). Methylenetetrahydrofolate reductase (MTHFR: EC 1.5.1.20) polymorphisms are associated to acute ly... more Methylenetetrahydrofolate reductase (MTHFR: EC 1.5.1.20) polymorphisms are associated to acute lymphoid leukemia in different populations. We used the polymerase chain reaction and the restriction fragment length polymorphism method (PCR-RFLP) to investigate MTHFRC677T and A1298C polymorphism frequencies in 67 patients with chronic myeloid leukemia (CML), 27 with acute myeloid leukemia FAB subtype M3 (AML-M3) and 100 apparently healthy controls. The MTHFR mutant allele frequencies were as follows: CML = 17.2% for C677T, 21.6% for A1298C; AML-M3 = 22.2% for C677T, 24.1% for A1298C; and controls = 20.5% for C677T, 21% for A1298C. Taken together, our results provide evidence that MTHFR polymorphisms have no influence on the development of CML or AML-M3.

The aim of this study is to investigate the clinical, hematological, and immunophenotypic charact... more The aim of this study is to investigate the clinical, hematological, and immunophenotypic characteristics of Brazilian children with B-cell acute lymphoblastic leukemia (B-ALL) to identify prognostic biomarkers of the disease. Thirty-three children newly diagnosed with B-ALL were followed between March 2004 and December 2009. Information about the demographic profile, diagnosis, immunophenotype, clinical manifestations, and disease outcome were gathered from the patients&amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;#39; medical records. Of the 33 patients with B-ALL, 18 were male and 15 female. Eighteen patients were classified as high risk; 13 as low risk, and 2 as true low risk. The frequencies of cluster of differentiation (CD)10, CD19, and CD20 antigens were 69.7%, 81.8%, and 18.2%, respectively. Six patients (18.2%) had aberrant expression of myeloid antigens. At diagnosis, patients immunopositive for CD20 had elevated white blood cell counts (P=0.018) and lower platelet counts (P=0.017). The 6-year overall survival was 67.5%±3.47%. Our results demonstrate the distinct immunophenotypic and prognostic characteristics of patients with B-ALL, which can be related to the Brazilian racial admixture. Consequently, these results will most likely aid in the selection of additional prognostic markers and their use in monitoring the clinical manifestations and treatment response among B-ALL patients.

O objetivo desta pesquisa foi investigar as características sócio-demográficas e clínico-patológi... more O objetivo desta pesquisa foi investigar as características sócio-demográficas e clínico-patológicas dos linfomas não-Hodgkin (LNH) em crianças e adolescentes no estado da Bahia. Do ponto de vista da metodologia, trata-se de estudo descritivo com dados secundários de 111 pacientes com LNH, procedentes de dois centros de referência. Critérios de inclusão: idade <20 anos, residência na Bahia e admissão no período de janeiro de2000 a dezembro de 2005, com diagnóstico anátomo-patológico de LNH. Obtiveram-se os seguintes resultados: dos 111 pacientes, 76 (68,5%) apresentaram idade entre cinco e 14anos, com uma média de 8,6±4,3 anos. Linfomas de Burkitt (LB) predominaram (56,8%), seguidos pelos linfomas linfoblásticos e linfomas de grandes células, cada um com 23 casos (20,7%). Laboratory tests obtained after fasting revealed elevated serum gastrin, supporting the suspected diagnosis of gastrinoma. In the endoscopy an elevated lesion with central depression was observed. The immunohistochemical examination revealed the benign nature of the tumor and the hyperplasia of argentaffin cells. Gastrinoma is a rare disease that predominantly affects young adults, but it must be considered in the pediatric group when clinical and laboratorial features of this disease are observed.

There were 210 males (59.4%) and 144 females (40.6%). Objective: This study was to determine the prevalence and characteristics of the osteoarticular m... more Objective: This study was to determine the prevalence and characteristics of the osteoarticular manifestations on initial clinical presentation of acute leukemias (ALs) on childhood in the state of Bahia, Brazil. Materials and Methods: This retrospective study assessed the medical records of 406 patients with AL from January 1995 to December 2004. Results: Acute lymphocytic leukemia (ALL) was diagnosed in 313 (77.1%) patients and acute myeloid leukemia (AML), in 93 (22.9%) patients, including 241 males (59.4%) and 165 females (40.6%). Age ranged from 9 months to 15 years (average: 6.18 y). The most common presenting features were fever (18.5%), musculoskeletal diffuse tenderness (15.0%), pallor (11.4%), and leg tenderness (5.7%). Prior referral to our center, the most frequent initial diagnosis was anemia (15.8%), leukemia (15.0%), amygdalitis (3.7%), and rheumatic fever (2.7%). Osteoarticular manifestations were found on 54.7% of the patients with AL, with a higher frequency among patients between 1 and 9 years of age (58.7%, P = 0.0007). The presence of joint tenderness (16.2% in ALL Â 5.4% in AML), arthritis (26.6% in ALL Â 9.7 in AML), bone tenderness (26.1% in ALL Â 16.1% in AML), limb tenderness (49.5% in ALL Â 25.8% in AML), and antalgic gait (32.8% in ALL Â 9.7% in AML) had higher prevalence on ALL. The large joints, chiefly the knees (10.6%), ankles (9.4%), elbows (4.4%), and shoulders (3.6%) were more often affected. Conclusions: AL should be considered on the differential diagnosis of osteoarticular symptoms of unknown etiology in children.

Despite the use of race in clinical and epidemiologic research, its characterization remains unclear, specially for those multiracial communities. Both racial mix and discordance about the concept of race make it difficult to classify and analyse emerging diseases and their pattern since biological concept of pure race is not well established. This brings to our mind some questions about social issues, such as education, economics, culture, politics, religion, ethnic, racism, and others. The literature about race and ethnic related to health was reviewed and the importance of this theme was commented. The influence of race in the risk of become ill and in the pattern of illness were reported. Cancer is reported apart. Special comments are done about childhood cancer. For all disease, available data show that race does not matter biologically, but the literature on patterns of care also shows that, in some cases, race does matter.

BACKGROUND: Metronomic chemotherapy (MC) consists of the administration of a low dose of chemothe... more BACKGROUND: Metronomic chemotherapy (MC) consists of the administration of a low dose of chemotherapy on a daily or weekly basis without a long break to achieve an antitumoral effect through an antiangiogenic effect or stimulation of the immune system. The potential effect of MC with continuous oral cyclophosphamide and methotrexate in patients with high-grade operable osteosarcomas (OSTs) of the extremities was investigated. METHODS: Patients with high-grade OSTs who were 30 years old or younger were eligible for registration at diagnosis. Eligibility for randomization included 1) nonmetastatic disease and 2) complete resection of the primary tumor. The study design included a backbone of 10 weeks of preoperative therapy with methotrexate, adriamycin, and platinum (MAP). After surgery, patients were randomized between 2 arms to complete 31 weeks of MAP or receive 73 weeks of MC after MAP. The primary endpoint was event-free survival (EFS) from randomization. RESULTS: There were 422 nonmetastatic patients registered (May 2006 to July 2013) from 27 sites in 3 countries (Brazil, Argentina and Uruguay), and 296 were randomized to MAP plus MC (n 5 139) or MAP alone (n 5 157). At 5 years, the EFS cumulative proportions surviving in the MAP-MC group and the MAP-alone group were 61% (standard error [SE], 0.5%) and 64% (SE, 0.5%), respectively, and they were not statistically different (Wilcoxon [Gehan] statistic 5 0.724; P 5.395). The multivariate analysis showed that necrosis grades 1 and 2, tumor size, and amputation were associated with shorter EFS.

From May 1999 to October 2009, 579 participants were enrolled in the Brazilian GCT-99 study. Treatment, defined as specific chemotherapy regimen and number of cycles, was allocated by means of risk-group assignment at diagnosis with consideration for stage and primary tumor site. Patients at low risk received no chemotherapy. Patients at intermediate risk (IR) with a good response (GR) received four cycles of platinum and etoposide (PE), for total doses of platinum 420 mg/m(2) and etoposide 2,040 mg/m(2). We used the polymerase chain reaction and the restriction fragment length polymorphism method (PCR-RFLP) to investigate MTHFRC677T and A1298C polymorphism frequencies in 67 patients with chronic myeloid leukemia (CML), 27 with acute myeloid leukemia FAB subtype M3 (AML-M3) and 100 apparently healthy controls. The MTHFR mutant allele frequencies were as follows: CML = 17.2% for C677T, 21.6% for A1298C; AML-M3 = 22.2% for C677T, 24.1% for A1298C; and controls = 20.5% for C677T, 21% for A1298C. Taken together, our results provide evidence that MTHFR polymorphisms have no influence on the development of CML or AML-M3.

Thirty-three children newly diagnosed with B-ALL were followed between March 2004 and December 2009. Information about the demographic profile, diagnosis, immunophenotype, clinical manifestations, and disease outcome were gathered from the patients&amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;#39; medical records. Of the 33 patients with B-ALL, 18 were male and 15 female. Eighteen patients were classified as high risk; 13 as low risk, and 2 as true low risk. The frequencies of cluster of differentiation (CD)10, CD19, and CD20 antigens were 69.7%, 81.8%, and 18.2%, respectively. Six patients (18.2%) had aberrant expression of myeloid antigens. At diagnosis, patients immunopositive for CD20 had elevated white blood cell counts (P=0.018) and lower platelet counts (P=0.017). The 6-year overall survival was 67.5%±3.47%. Our results demonstrate the distinct immunophenotypic and prognostic characteristics of patients with B-ALL, which can be related to the Brazilian racial admixture. Consequently, these results will most likely aid in the selection of additional prognostic markers and their use in monitoring the clinical manifestations and treatment response among B-ALL patients.
