Patients and methods: A total of 21 female patients (mean age 47.14±10.39 years; range 18 to 75 years) who met 2013 American College of Rheumatology/European League Against Rheumatism criteria for SSc were evaluated for severity of organ involvement and symptoms. Turkish version of MAF, FSS, and visual analog scale of fatigue were assessed at baseline and after two to three weeks. Level of dyspnea was noted and disability, functional limitation, and quality of life were assessed by health assessment questionnaire, 6-minute walking distance, and short-form 36, respectively. Results: Ten patients had diffuse and 11 had limited SSc. MAF subscales and FSS had significant correlations with short-form 36-vitality subscale and 6-minute walking distance. Intraclass correlation coefficients for FSS and visual analog scale of fatigue were 0.824 (95% confidence interval, 0.566-0.929) and 0.932 (95% confidence interval, 0.832-0.972), respectively. The intraclass correlation coefficients for MAF subscales changed between 0.916 and 0.968, except for MAF-timing (intraclass correlation coefficient, 0.404). Conclusion: Our results revealed that FSS and MAF subscales had high reproducibility and correlated well with quality of life and disability scales which, to some extent, may suggest convergent validity of MAF subscales and FSS in SSc. The incompatible nature and four-choice answering in two items of MAF-timing may be the underlying reason for trivial relationship with other parameters. The Turkish version of MAF and FSS may be used to assess fatigue in patients with SSc.

Research paper thumbnail of Fluoroscopy-guided intra-articular steroid injection for sternoclavicular joint arthritis secondary to limited cutaneous systemic sclerosis: a case report

The Korean Journal of Pain, 2017

We report a case of fluoroscopy-guided intraarticular steroid injection for sternoclavicular join... more We report a case of fluoroscopy-guided intraarticular steroid injection for sternoclavicular joint (SCJ) arthritis caused by limited cutaneous systemic sclerosis (SSc). A 50-year-old woman diagnosed with limited cutaneous SSc presented with swelling and pain in the right SCJ. MRI revealed signs of inflammation consistent with right-sided sternoclavicular joint arthritis. After the failure of oral medications, we performed fluoroscopy-guided injection in this region. She reported complete resolution of her symptoms at 4 and 12-week follow-ups. This outcome suggests that a fluoroscopy-guided SCJ injection might be a safe and successful treatment option for sternoclavicular joint arthritis.

The World Health Organization states that dual-energy X-ray absorptiometry (DXA) is the best densitometric technique for assessing bone mineral density (BMD). DXA provides an accurate diagnosis of osteoporosis, a good estimation of fracture risk, and is a useful tool for monitoring patients undergoing treatment. Common mistakes in BMD testing can be divided into four principal categories: 1) indication errors, 2) lack of quality control and calibration, 3) analysis and interpretation errors, and 4) inappropriate acquisition techniques. The aim of this retrospective multicenter descriptive study is to identify the common errors in the application of the DXA technique in Turkey. Methods: All DXA scans performed during the observation period were included in the study if the measurements of both, the lumbar spine...

(Sakarya Med J 2019, 9(2):319-325) Materials and Methods Participants included in this study were chosen among patients who applied to Kayseri City Hospital between June 2018 and December 2018. Inclusion criteria for participants were aged between 18-65 and blood glucose levels in prediabetic range (n=96). Participants' age, gender, marital status, salary, education, profession, employment, smoking status, alcohol use, height, weight, body mass index, insulin resistance were based on self-report and our measurements. Results Half of the patients with prediabetes was in IFG (n=50) and the subgroup with fewest patients was IGT (n=14) in our study also. Combined group (IFG + IGT) was higher (n=32). Seventy-one of the patients were female (74%). Twenty-three patients smoked at anytime and mean pack-years was 27.78 (min:1, max:95, sd: 23.55), 93 patients (96.7%) never used alcohol. Fifteen patients were retired, and disablement was the cause of only one of them. The ratio of insulin-resistant patients was 50%. Marital status of patients was one single, 89 married, five widows/widower, one divorced. There was a higher rate of obesity in prediabetic patients in our study. Ratio of BMI according to cutoff points was 1%, 9%, 18%, 68.76% for ≤ 18.5 kg/m2; 18.6-24.9 kg/m2; 18 % 25.0-29.9 kg/m2; 68.76 % ≥30 kg/m2, respectively. Median and mean HAQ-DI scores were 0.125 (IQR: 0-0.625) and 0.384 (SD:0.534) respectively. 67 patients (69.9%) did not have any disability (HAQ-DI score was 0-0.375). Conclusion Prediabetes is an increasingly common disease, leading to severe morbidities. Most patients are obese and female. Most female patients of prediabetic patients were housewives. Early diagnosis and appropriate treatment of people with these characteristics, especially those with a high risk of prediabetes, can prevent the development of diabetes. Keywords disability; prediabetes; sociodemographic Öz Amaç Bu çalışmada 3. basamak iç hastalıkları polikliniğine başvuran prediyabetik hastalarda sosyodemografik ve klinik verileri tanımlamayı ve engellilik ile ilişkisini araştırmayı amaçladık.

Paradoxical psoriasis or psoriasiform lesion is an adverse effect, represented by the occurrence ... more Paradoxical psoriasis or psoriasiform lesion is an adverse effect, represented by the occurrence of a psoriasiform lesion or exacerbation of psoriasis caused by the drugs normally used for the management of psoriasis. In this article, we present the first case of a 45-year-old male patient with rheumatoid arthritis who developed psoriasiform lesions following treatment with tofacitinib, and highlight the possible pathogenetic mechanisms involved in such an occurrence.

Research paper thumbnail of Tofacitinib suppresses disease activity and febrile attacks in a patient with coexisting rheumatoid arthritis and familial Mediterranean fever

Acta reumatologica portuguesa

Familial Mediterranean fever (FMF) is the most common hereditary auto-inflammatory (periodic feve... more Familial Mediterranean fever (FMF) is the most common hereditary auto-inflammatory (periodic fever) syndrome, and usually successfully treated with colchicine. However, nearly 5-10% of FMF cases are resistant or intolerant to colchicine and treatment options are highly restricted in these cases. Biologics including anakinra, canakinumab, rilonacept, etanercept, infliximab, interferon-alpha, and tocilizumab are shown to have efficacy to control FMF attacks. Tofacitinib, a Janus kinase (JAK) inhibitor, is an orally administered non-biologic disease modifying anti-rheumatic drug for the treatment of rheumatoid arthritis (RA). Herein we report a female patient with coexisting RA and colchicine resistant FMF whose FMF attacks and disease activity were completely controlled after treatment with tofacitinib, a small-molecule JAK3 inhibitor.

Medicina Clínica, 2022

INTRODUCTION Familial Mediterranean fever (FMF) is an autoinflammatory disease characterized by p... more INTRODUCTION Familial Mediterranean fever (FMF) is an autoinflammatory disease characterized by polyserositis and arthritis attacks, which are mediated by increased plasma levels of cytokines. Our hypothesis was that the increase in specific cytokines can also lead to portal hypertension, even in the absence of overt hepatic steatosis. METHODS We designed a comparative cross-sectional study with 41 patients and 30 healthy individuals to show if there is a relationship between portal hypertension and FMF. B mode ultrasound and Doppler ultrasound were utilized to evaluate liver echogenicity, portal vein diameter, peak portal blood flow velocity, and portal vein flow direction, which are important diagnostic criteria for portal hypertension. RESULTS Spleen and liver sizes and portal vein diameters of the FMF patients and the healthy controls were not significantly different. Imaging of 4 patients revealed periportal cuffing and one patient with periportal cuffing also had a fine granular appearance of the liver. The peak portal blood flow velocity of the FMF patients was lower than that of the control group (p<0.007). CONCLUSION The FMF group had significantly lower peak portal blood flow velocity than the control group, indicating the existence of portal hypertension. However, the differences between the other findings that correlate with portal hypertension were not significant.

This study aimed to determine the prevalence of neuropathic pain (NeP) and fibromyalgia syndrome ... more This study aimed to determine the prevalence of neuropathic pain (NeP) and fibromyalgia syndrome (FMS) in prediabetics and to compare them with normoglycemic controls, to disclose the impact of NeP and FMS in disease burden. People, 18–65 years old, who were admitted to a tertiary hospital’s internal medicine outpatient clinic for routine health check-ups and then those who were newly diagnosed with prediabetes were recruited as a prediabetic group and those who were normoglycemic were recruited as a control group. Participants’ demographics and clinical data were recorded. The ShortForm-36 and Hospital Anxiety-Depression Scale were answered by the participants. The 2016 ACR Fibromyalgia Diagnostic Criteria and the painDETECT questionnaire were used for evaluation of FMS and NeP, respectively. One hundred nine prediabetics and 53 controls were enrolled. Eighty-four (77.1%) of 109 prediabetics and 37 of 53 controls (69.8%) were female. The mean age of the prediabetics was 48.85 (SD = 9.8) and the controls was 47.37 (SD = 11.11). Age, gender, BMI, smoking status, occupational, marital, and educational status were similar between the groups. FMS was more common in prediabetics 32 (29.6%) of 109 than in normoglycemics 7 (13.2 %) of 53 (p = 0.022). Eight of the prediabetics (7.4%) and 2 of the normoglycemics (3.8%) had possible or likely NeP (p = 0.273). The frequency of possible or likely NeP was higher in prediabetics with FMS than without FMS (p = 0.001). The prevalence of FMS was higher in prediabetics with NeP (87.5%) than without NeP (25.5%) (p = 0.001). Prediabetics with FMS or NeP had lower QoL than without FMS or NeP (p < 0.001, p = 0.014, respectively). While evaluating prediabetics, it is important to assess both FMS and NeP.

Purpose To evaluate the effect of long-term effects of hydroxychloroquine (HCQ) use on the cornea... more Purpose To evaluate the effect of long-term effects of hydroxychloroquine (HCQ) use on the corneal endothelium in patients with systemic lupus erythematosus (SLE). Methods The study included 30 eyes of 15 patients who used HCQ for at least five years and were followed up in the rheumatology clinic for SLE and 29 eyes of 15 patients newly diagnosed with SLE who had not taken any medication. All patients underwent complete ophthalmologic examinations, central corneal thickness measurements, endothelial cell counts and endothelial morphological examinations by specular microscopy. Results The mean age was 41.25 ± 6.89 years in group 1 and 41.81 ± 5.78 years in group 2 (p = 0.737). The mean corneal endothelial cell density was 2662 ± 184 cells/mm2 in group 1 and 2649 ± 315 cells/mm2 in group 2 (p = 0.843). The percentage of cell hexagonality was 46.6 ± 11.8% in group 1 and 48.7 ± 13.8% in group 2 (p = 0.625). A difference was noted in endothelial cell variation in both groups, at 36.7 ± 6.9% in group 1 and 39.9 ± 6.2% in group 2 (p = 0.057) but not statistically significant. No difference was observed between the two groups in terms of central corneal thickness. The duration of HCQ use was negatively correlated with hexagonality (r =-0.564, p = 0.001) Conclusion Long-term use of HCQ in SLE patients had no negative effect on endothelial cell density and cell variation, but a negative correlation existed between the duration of HCQ usage and hexagonality.

International Journal of Diabetes in Developing Countries, 2021
