According to several studies, women with Crohn's disease (CD) had reduced fertility, which is... more According to several studies, women with Crohn's disease (CD) had reduced fertility, which is mostly due to voluntary decisions and reduced ovarian reserve. In our study, we aimed to compare reproductive health parameters (RHP), previous pregnancy complications and outcomes, and ovarian reserve (OR) assessed by the anti-Mullerian hormone (AMH) in CD patients with healthy controls. In CD patients, we also compared OR according to disease phenotypes. Consecutive pre-menopausal women with CD from two IBD centers were included. The control group consisted of age and BMI-matched healthy controls. We used a questionnaire that included RHP, CD phenotype, and CD activity. Serum AMH was assessed by the Elecsys AMH plus essay. We enrolled 50 patients and 56 controls with a median age of 31 years. All CD patients were in clinical remission. V posledných rokoch doslo ku významnemu pokroku vo vývoji enteralnych stentov. Zlepsili sa vlastn... more V posledných rokoch doslo ku významnemu pokroku vo vývoji enteralnych stentov. Zlepsili sa vlastnosti stentov, techniky ich zavadzania a rozsiruju sa indikacie. Paliativne stentovanie inoperabilných malignych stenoz pažeraka je dnes bežnou sucasťou liecby. Okrem malignych indikacii sa rozsiruje spektrum benignych indikacii ako je liecba benignych stenoz pažeraka, fistul, anastomotických leakov perforacii alebo krvacajucich varixov. Výber optimalneho typu stentu a liecebneho protokolui je naďalej viac otazkou klinickej skusenosti, nakoľko komparativnych prospektivnych studii je malo. Novými sľubnými inovaciami v oblasti stentov su biodegradabilne steny alebo dvojvrstvove stenty. V blizkej buducnosti je možne ocakavať ďalsi intenzivny vývoj nových stentov, technik zavadzania a klinických indikacii.

Nutritional goals and adherence to ONS are difficult to achieve while studies addressing adherence are scarce. We aimed to evaluate adherence to ONS, the associated factors, and its impact on outcome among ALCD patients who are discharged from the hospital. Patients and Methods We identified consecutive hospitalized patients with ACLD from the cirrhosis registry and ONS prescription at discharge. Baseline demographics, anthropometrics, hand-grip strength (HGS), nutritional, and laboratory parameters were recorded. In recent years, the diagnostic criteria for sarcopenia and frailty are gradually merging into one, primarily for practical reasons. Both of them are equally negative predictors of disease development in hospitalized patients with cirrhosis regardless of the decision on the indication of liver transplantation, but also in patients on the waiting list, and in those after liver transplantation. Sarcopenia and frailty are strong and disease stage-independent predictors of mortality. Undiagnosed and untreated sarcopenia or frailty also increase the risk of falls and fractures, impair the natural ability to perform daily activities, and increase the degree of dependence on others. In a prospective single-center cohort study, inclusion criteria were: consecutive hospitalization for ACLD and Child-Pugh score ≥ 7 points. Baseline parameters were myostatin, C-reactive protein (CRP), hand-grip strength (HGS), mid-arm muscle circumference (MAC), transversal psoas muscle index (TPMI). Patients were followed-up for at least 400 days. We included 198 men, 157 women, and 40 controls, median myostatin levels in pg/ml were 1790.1 in women, 1959.4 in men, and 3850.0 and 2996.0 in healthy men and women. Data on disease course in different etiologies are scarce, so we aimed to compare the prevalence and risk factors of frailty and its impact on prognosis in nonalcoholic fatty liver (NAFLD) and alcoholic (ALD) cirrhosis. Patients and Methods. Cirrhosis registry RH7 operates since 2014 and includes hospitalized patients with decompensated cirrhosis, pre-LT evaluation, or curable hepatocellular carcinoma (HCC). From the RH7, we identified 280 ALD and 105 NAFLD patients with at least 6 months of follow-up. Results. Patients with NAFLD compared with ALD were older and had a higher proportion of females, higher body mass index (BMI) and mid-arm circumference (MAC), lower MELD score, CRP, and lower proportion of refractory ascites. We aimed at evaluating the prevalence and pattern of DILI. Consecutive RD patients treated with BT were followed for 6 months. ALT and ALP >the upper limit normal (ULN) and3xULN injury Grade 2. 582 liver function tests (LFTs) in 199 patients were evaluated, median age 53y, 59.3 % females, RA in 108, AS 49, and PsA 42 patients. ALT Grade 1 elevation was observed in 25.6 %, transient in 18.6 %, persisting in 7 %, Grade 2 in 1.5 %, ALP Grade 1 in 3.5 %, transient in 2 %, persisting in 1.5 %. Obesity and venous thromboembolism represent a serious medical problem. Their mutual relationship... more Obesity and venous thromboembolism represent a serious medical problem. Their mutual relationship was studied on the group of 124 patients with venous thromboembolism (the study group) and was compared to the group of 124 randomly chosen controls among hospitalized patients matched for age, sex and risk factor profile of venous thromboembolism (the control group). Recorded variables were body weight, presence of arterial hypertension, hypertriglyceridemia and type 2 diabetes or impaired glucose tolerance. We found significantly higher prevalence of obesity--40 vs 23 patients (38.5% vs 23%, p = 0.0283) and significantly higher BMI (29.254 vs 26.923, p = 0.002) in the study group. Background: The aetiology of inflammatory bowel disease (IBD) is not known but is likely to invol... more Background: The aetiology of inflammatory bowel disease (IBD) is not known but is likely to involve a combination of genetic predisposition and environmental risk factors. Smoking has been associated consistently with a higher risk of Crohn's disease (CD), while appendectomy and smoking appear to diminish the risk of ulcerative colitis (UC). The roles of other environmental factors are unclear. The aim of the present study was to evaluate the association of CD and UC with several environmental risk factors. Methods: This case-control study included 338 patients (190 CD, 148 UC) and 355 controls. All subjects completed a detailed questionnaire regarding breastfeeding duration, history of helminthic infections, allergic diseases, appendectomy, household size, housing type, contact with specific domestic animals, physical activity, and smoking. Associations between risk factors and CD and UC were investigated by univariate and multivariate analysis. Results: On multivariate analysis, CD associated with smoking at diagnosis (odds ratio, OR, 3.7, 95% CI 2.2-6.2; p < 0.001), being breastfed for <6 months (OR 2.7, 95% CI 1.7-4.4; p < 0.001), and less than two childhood sporting activities weekly (OR 2.7, 95% CI 1.5-5.0; p < 0.001) and inversely associated with frequent contact with cats in childhood (OR 0.6, 95% CI 0.4-0.9; p < 0.03). UC associated with less than two sporting weekly activities in childhood (OR 2.0, 95% CI 1.1-3.5, p ¼ 0.02), fewer household members in childhood (OR 0.8, 95% CI 0.7-0.98, p ¼ 0.03), and being breastfed for <6 months (OR 1.7, 95% CI 1.02-2.8, p ¼ 0.04). A composite environmental risk index for CD revealed that 47 and 14% of the controls and patients with CD had no risk factors, respectively, and that 14 and 38% of the controls and patients with CD had at least two risk factors, respectively. Conclusion: CD and UC associated with infrequent childhood sports activities and short breastfeeding. Furthermore, CD associated with smoking and infrequent contact with animals in childhood. UC associated with a smaller family size in childhood.

The goal of our prospective study was to evaluate the rate of residual pulmonary emboli in repeated CT pulmoangiography 13-17 weeks after setting the diagnosis of PE and to determine the relationship between primary risk factors for PE, the results of the primary entry tests and the incidence of residual pulmonary embolism. Fifty-three patients aged 59.8 (54.9-67.7) years with confirmed first episode of PE were enrolled into the study, 37 patients after appropriate anticoagulation underwent the repeated CT pulmoangiography. Obesity and venous thromboembolism represent a serious medical problem. Their mutual relationship... more Obesity and venous thromboembolism represent a serious medical problem. Their mutual relationship was studied on the group of 124 patients with venous thromboembolism (the study group) and was compared to the group of 124 randomly chosen controls among hospitalized patients matched for age, sex and risk factor profile of venous thromboembolism (the control group). Recorded variables were body weight, presence of arterial hypertension, hypertriglyceridemia and type 2 diabetes or impaired glucose tolerance. We found significantly higher prevalence of obesity--40 vs 23 patients (38.5% vs 23%, p = 0.0283) and significantly higher BMI (29.254 vs 26.923, p = 0.002) in the study group. Increased body weight was found to be the only significant risk factor for venous thromboembolism (odds ratio = 2.507, p = 0.0026) and normal body weight was the only protective factor (odds ratio = 0.3989, p = 0.0026). Background: The aetiology of inflammatory bowel disease (IBD) is not known but is likely to invol... more Background: The aetiology of inflammatory bowel disease (IBD) is not known but is likely to involve a combination of genetic predisposition and environmental risk factors. Smoking has been associated consistently with a higher risk of Crohn's disease (CD), while appendectomy and smoking appear to diminish the risk of ulcerative colitis (UC). The roles of other environmental factors are unclear. The aim of the present study was to evaluate the association of CD and UC with several environmental risk factors. Methods: This case-control study included 338 patients (190 CD, 148 UC) and 355 controls. All subjects completed a detailed questionnaire regarding breastfeeding duration, history of helminthic infections, allergic diseases, appendectomy, household size, housing type, contact with specific domestic animals, physical activity, and smoking. Associations between risk factors and CD and UC were investigated by univariate and multivariate analysis. Results: On multivariate analysis, CD associated with smoking at diagnosis (odds ratio, OR, 3.7, 95% CI 2.2-6.2; p < 0.001), being breastfed for <6 months (OR 2.7, 95% CI 1.7-4.4; p < 0.001), and less than two childhood sporting activities weekly (OR 2.7, 95% CI 1.5-5.0; p < 0.001) and inversely associated with frequent contact with cats in childhood (OR 0.6, 95% CI 0.4-0.9; p < 0.03). UC associated with less than two sporting weekly activities in childhood (OR 2.0, 95% CI 1.1-3.5, p ¼ 0.02), fewer household members in childhood (OR 0.8, 95% CI 0.7-0.98, p ¼ 0.03), and being breastfed for <6 months (OR 1.7, 95% CI 1.02-2.8, p ¼ 0.04). A composite environmental risk index for CD revealed that 47 and 14% of the controls and patients with CD had no risk factors, respectively, and that 14 and 38% of the controls and patients with CD had at least two risk factors, respectively. Conclusion: CD and UC associated with infrequent childhood sports activities and short breastfeeding. Furthermore, CD associated with smoking and infrequent contact with animals in childhood. UC associated with a smaller family size in childhood.
