Coronavirus COVID-19 appeared in China, in Wuhan in December 2019, which has spread at a lightning speed across the planet earth, becoming a pandemic. Senegal, as well as countries around the world, has also been affected by this pandemic, which has had a strong impact on all its sectors of activity, particularly the health system. The objective of this study was to identify and evaluate the factors that increase the time required to treat myocardial infarctions, received late at H?pital Aristide le Dantec and to assess the impact of the Covid-19 pandemic on these delays. Results: A total of 25 patients were included: 17 patients in pre-pandemic period and 8 patients in pandemic period. The predominance was male with a sex ratio of 2.5. The mean age was 59.20 years. Introduction: The objectives of this study were to assess the late or early infectious complicati... more Introduction: The objectives of this study were to assess the late or early infectious complications associated with the implantation of pacemakers in the cardiology department of the Grand Yoff General Hospital. Methodology: This was a retrospective study concerning the infectious complications of conventional cardiac stimulation in patients who received a pacemaker between January 1, 2006 and December 31, 2014 in the cardiology department of Grand Yoff General Hospital. The different parameters studied included sociodemographic, clinical, paraclinical data, indications and data on definitive stimulation. Infectious complication, type, treatment and course had also been studied. Early complications are those observed in the first six weeks and late ones beyond six weeks after implantation. Introduction Brugada syndrome is due to calcium channels dysfunction that can be responsible for ... more Introduction Brugada syndrome is due to calcium channels dysfunction that can be responsible for serious ventricular disorders and conduction abnormalities. Observation It was a 37-year-old man, who had been seen for bradycardia. The interrogation found a notion of undocumented loss of consciousness at the age of 17 consecutive to an effort. Blood pressure was at 150/70 mmHg and the heart rate was 42 bpm. The physical examination was featureless apart from bradycardia. The electrocardiogram recorded a straight branch block aspect, an ST segment elevation in straight precordial leads (V1 to V3), and sinus pauses. The Doppler echocardiography was normal. The Holter monitor showed sinus pauses, the longest one lasting 6095 ms. No rhythm disorder was noted during the 24 hours. Electrophysiological exploration showed an extension of the AH intervals at 140 ms and HV at 70 ms. The corrected sinus recovery time was calculated at 3772 ms. Luciani Wenckebach's time was measured at 480 ms and the point 2/1 at 450 ms. The programmed ventricular pacing had not triggered ventricular arrhythmia. The patient benefited an implantable cardioverter defibrillator (ICD). The evolution was good at 3 months with control of the ICD parameters. Genetical analysis was not done due to lack of financial ressources. A clinical family survey is in progress. Conclusion The association Brugada syndrome and sinus node dysfunction is not uncommon. Only the implantation of an ICD is an approved therapy.

Introduction: Sudden cardiac death (SCD) remains a major public health issue. In Senegal, there i... more Introduction: Sudden cardiac death (SCD) remains a major public health issue. In Senegal, there is a lack of data on this phenomenon; this underlies the relevance of this study whose objective is to study the epidemiological profile of victims experiencing unrecovered sudden cardiac arrest at Aristide LE DANTEC Hospital. Patients and Methods: This is a transverse study with retrospective data collection from January 1, 2016 to January 31, 2017. Included, in the study were all victims of sudden cardiac death in Dakar who had a necropsy in the morgues of Aristide Le Dantec Hospital. Results: We recorded 69 cases of sudden death of cardiovascular origin from 169 forensic autopsy reports, or 40.8% of autopsies. The average age was 44 years with a male predominance in 76.81%. The large majority of death occurred at home (26%), without witnesses (48%) and at rest (36%). Two cases of sudden death, or 3%, occurred at exertion. Abstract Rupture of an ancurysm of a sinus of Valsalva into a right heart chamber is classically ... more Abstract Rupture of an ancurysm of a sinus of Valsalva into a right heart chamber is classically associated with a continuous murmur. This was heard in the present case with rupture into the right atrium. Hemodynamic data suggest that shunt flow was largely diastolic, systolic shunt flow being impaired by a Venturi effect in the aortic root and by a mechanical occlusive or kinking effect of ventricular systole upon the fistula tract. Right atrial pressure was normal despite the large shunt; this may be related to increased right atrial compliance permitting this chamber to dilate and still maintain normal pressures. The localized continuons murmur heard externally was probably a direct transmission of the continuous murmur in the mid and distal portions of the aortico-right atrial communication. However, accentuation of certain phases of this murmur and the systolic and diastolic murmurs heard at other precordial locations are probably related to other factors, including phasic changes in shunt flow and increased blood flow across the pulmonic and aortic valves.

Nous rapportons le cas d’une patiente, âgée de 26 ans, atteinte d’une cirrhose auto-immune. Elle présentait une douleur thoracique aiguë, une dyspnée d’effort et une toux. Le diagnostic fut posé par l’échocardiographie qui montrait une dilatation des cavités cardiaques droites et une pression artérielle pulmonaire systolique à 95 mmHg. Les inhibiteurs de la phosphodiestérase-5 tels que le sildénafil représentent une option thérapeutique de l’hypertension portopulmonaire. Elle constitue une complication rare mais grave de la cirrhose évoluant vers l’insuffisance cardiaque droite et le décès.AbstractPortopulmonary hypertension is the combination of pulmonary arterial hypertension and portal hypertension. We report the case of a 26-year-old woman suffering from autoimmune cirrhosis. She presented with acute chest pain, exertional dyspnea, and cough. The diagnosis was made by echocardiography which showed a dilation of right heart chambers and a pulmonary arterial systolic pressure up to 95 mmHg. Phosphodiesterase-5 inhibitors such as sildenafil represent a therapeutic option for portopulmonary hypertension. It is a rare but serious complication of cirrhosis progressing to right heart failure and death.
