Cannabis has been recently suggested as a potential treatment for patients with sleep disorders; however, research on the relationship between cannabis and sleep is still in its infancy. Objectives: The aim of this investigation was to assess whether cannabis use was associated with improved sleep quality. Methods: Our study comprised 173 participants, 42 cannabis users and 131 non-cannabis users, who completed the Pittsburgh Sleep Quality Index (PSQI), the most common self-reported measure of sleep quality. The scale provides a global PSQI score and seven component domain scores, including subjective sleep quality, sleep latency, sleep duration, habitual sleep efficiency, sleep disturbances, use of sleep medication, and daytime functions. Results: Cannabis users self-reported statistically significantly healthier scores than non-cannabis users in the global PSQI as well as the specific domains of subjective sleep quality, sleep latency, as well as sleep disturbances. Conclusions: This preliminary evidence points to the possibility that cannabis could provide effective treatment for patients with sleep disorders. Research into the constituents of cannabis that may have a differential impact on sleep and sleep disorders is warranted.

The impact on occupational health is still unclear as it was scarcely studied. Purpose: The aim of this study was to assess the association of work performance with OSAHS and daytime sleepiness. Methods: A cross-transversal study was conducted including Tunisian active adult subjects referred for suspected sleep disordered breathing. All subjects completed the Epworth Sleepiness Scale (ESS), the Work Productivity and Activity Impairment Questionnaire (WPAI), and the Patient Health Questionnaire 9 (PHQ9). They all underwent polygraph testing level 3. Result: One hundred thirty-nine subjects completed the survey, including 107 (77%) blue-collar workers, and 32 (23%) white-collar workers. Participants were classified as following: 50 patients with apnea hypopnea index (AHI) ≥ 30 and 89 with AHI <30. Among the four outcomes of the WPAI scale, only presenteeism was associated with AHI (p=0.012). A significant association has been found between daytime sleepiness and three outcomes of WPAI. Sleepy patients with ESS ≥ 11 had lower work performance in terms of presenteeism (42.46 ± 28.40 vs. 24.71 ± 24.77 p=0.0001), absenteeism (12.26 ± 25.51 vs. 6.05 ± 18.32, p=0.101), overall work productivity loss (47.43 ± 31.65 vs. 26.58 ± 28.47, p= 0.0001), and decline in activity (46.67 ± 30.75 vs. 32.14 ± 27.02, p=0.004) in comparison with non-sleepy participants. A strong correlation of presenteeism and overall work productivity loss scores with ESS was demonstrated among white-collar workers (r=0.624, p=0.0001; r=0.602, p=0.0001 respectively). PHQ9 score was significantly higher in sleepy subjects (p=0.0001). Near miss accidents in the workplace and work related accidents were significantly more frequent in those with ESS ≥ 11 (p=0.012, p=0.026; respectively). Conclusion: The most relevant finding of this study was the involvement of daytime sleepiness in performance impairment, while the impact of AHI was less clear.

We conducted a crosssectional and exhaustive study among SMO working in the leather and footwear industry to describe the prevalence of multi-site musculoskeletal symptoms (MMS) and evaluate factors associated with their occurrence. Methods: Musculoskeletal symptoms declared by these operators were assessed through the modified Nordic questionnaire. The psychosocial work environment was assessed using the Karasek model. The variables associated with MMS were issued from binary logistic regression and decision tree using R software. Results: Of 145 operators, 65.5 % of men and 72.4 % of women had MMS. Based on binary logistic regression, a history of musculoskeletal disorders (MSDs) increased the risk of developing MMS by 8 folds. The binary decision tree identified five main nodes: history of MSDs, professional seniority, often finding the pace of work restrictive and male gender. Conclusion: Identifying homogeneous profiles of MMS's occurrence will help the implementation of an effective and targeted preventive strategy.

The frequency and reporting rate concerning occupational blood exposure were investigated among f... more The frequency and reporting rate concerning occupational blood exposure were investigated among former and currently employed medical staff at a Department of Infectious Diseases (DID) having a high prevalence of HIV-positive patients. Subjects were asked to complete an anonymous questionnaire describing occupational percutaneous exposure (PCE) and mucocutaneous exposure (MCE) to blood, experienced during their employment at the DID. 135 out of 168 (80%) subjects responded. 45 subjects described 37 incidents of PCE and 15 of MCE. 44 of the exposures (85%) involved HIV-positive blood and 6 (11.5%) involved blood from a patient with hepatitis B. Annual incidence rates of PCE and MCE were: for A) interns and residents, 0.51 PCE/year and 0.17 MCE/year; B) for senior residents and senior physicians, 0.13 PCE/year and 0.21 MCE/year; C) for registered nurses, 0.11 PCE/year and 0.03 MCE/year; D) for auxiliary nurses, 0.09 PCE/year and 0.11 MCE/year. 35% of PCE and 87% of MCE were not reported to the security representative, the major reason being that the subject felt it to be unnecessary. We conclude that medical employment, especially as interns and residents at clinics for infectious diseases, carries a real and serious risk of contracting infectious diseases due to occupational exposure to blood. The importance of reporting needs to be emphasized.

Objectives: Development of a patentable method of treatment in addictology using pyracetam and nicotinic acid transcerebral electrophoresis (TCE). Methods: Valid clinical-diagnostic, laboratory, biochemical, electrophysiological, psychological (scaling, testing), statistical methods for identification of alcohol dependence complicated by amnestic disorders. Results: The method of treatment of alcohol dependence complicated by amnestic disorders (Patent 141785 UA) provides complex pharmacological and drug-free therapy. Antiamnesic drugs are administered by TCE bilaterally; pyracetam 20% solution to the left orbit through active negative electrodes, and nicotinic acid 0.1% solution to the right orbit (positive electrode in the occipital fossa), current of 2-4 mA, 20-30 minutes exposure. The procedure was performed daily with a TCE device, for a 10-day course of treatment along with psychotherapeutic potentiation. TCE provides the ionic implementation of pharmacological agents in the brain and their physiological electrical stimulation. Conclusions: In a representative clinical trial, using statistical methods and generated bank of patient-specific observations, significant potentiating effects of combined drug-free, non-invasive transcerebral electrical stimulation and electrophoretic implementation of pyracetam and nicotinic acid were demonstrated.

Prader-Willi syndrome is a congenital disease caused by a genetic alteration of chromosome 15, de... more Prader-Willi syndrome is a congenital disease caused by a genetic alteration of chromosome 15, described by doctors Prader, Labhart and Willi in 1956. It's a rare disease (prevalence 2.8/100,000) and it occurs equally in both sexes and in all races. This disorder produces changes in hypothalamic function that can lead to muscle hypotonia, short stature, a compulsion to eat, and a lack of satiety. At the psychopathological level, the clinic is very varied, being mainly important the psychomotor retardation in different degrees and behavioral problems;especially in the behavioral phenotype. Affective and psychotic symptoms are also frequent. Objectives: Psychopathology and treatment analysis through a clinical case. Methods: 40-year-old patient undergoing mental health follow-up since adolescence with a diagnosis of paranoid personality disorder. No medical history of interest. He was admitted to the hospitalization unit for serious behavioral alterations in a context of probable paranoid ideation towards the neighborhood. In the psychopathological examination, marked cognitive rigidity, high impulsivity and very low tolerance to frustration stand out, showing a hostile and defiant attitude. Poorly structured paranoid ideation. Presents obsessive-compulsive behaviors (scratching). Hyperphagia and obesity. Results: Psychometric assessment (Waiss-IV): total IQ 61 (mild mental retardation). Genetics:deletion 15 q11-q13 of chromosome 15), confirms Prader-Willi diagnosis. Remission of behavioral disorders, suspiciousness and heteroaggressive behaviors with treatment with monthly depot paliperidone (150 mg). Conclusions: Knowledge of the clinical and morphological characteristics of this syndrome would allow an early diagnosis and treat its possible complications as soon as possible. Antipsychotic treatment is effective in the management of behavioral and psychotic symptoms.
