Munchhausen's syndrome is the best-studied type of factitious disorder with predo... more Munchhausen's syndrome is the best-studied type of factitious disorder with predominantly physical signs and symptoms. However, its clinical presentation during pregnancy is rare and literature on the subject is scarce. To present a case of factitious disorder during pregnancy. Review and analysis of a clinical case. The case of a 30-year-old woman six weeks pregnant who consults on multiple occasions with emesis and abdominal pain is discussed. Various different diagnoses were considered and pathologies such as acute intermittent porphyria, narcotic bowel syndrome, abdominal epilepsy, and esophageal spasm were discarded. The patient expressed her desire to terminate the pregnancy at week 21. The pregnancy was terminated after a medical panel concluded that there were both organic and mental conditions that placed the patient's health at risk. Factitious disorders are very complex and extremely challenging to the clinician especially during the initial phases. They are seldom suspected and rarely diagnosed even in hospital settings. Therefore it is of utmost importance to educate healthcare professionals in the detection of this disorder to ensure proper management and treatment.

The Segovia-Remedios Mining District is one of the largest Colombian Gold Fields because of the a... more The Segovia-Remedios Mining District is one of the largest Colombian Gold Fields because of the amount of mineral resources produced since the last century, and its recognition for gold mining exploitation in Northeast Antioquia. The remarkable structural control of lode deposits (besides the geological complexity or the location of the area) has conducted many researchers to suggest different evolution models to achieve real fluid sources. Based on microscopic relationships, a three-stage mineral paragenesis is proposed in this work, where Ag-Au ores precipitated in phase II (Mineralizing) accompanied by a big volume of sphalerite + galena + quartz with few quantities of pyrite ± chalcopyrite ± pyrrhotite. The DMSR samples exhibit bi-phase fluid inclusions (LH2O+VH2O) which have at least three different associations according to Th (Homogenization temperature) and salinity values. The saline system is H2O+NaCl+CaCl2 likely exhibit a magmatic source.
El distrito Minero Segovia-Remedios (DMSR) es uno de los distritos auríferos más grandes de Colom... more El distrito Minero Segovia-Remedios (DMSR) es uno de los distritos auríferos más grandes de Colombia por la cantidad de recursos generados y por la trayectoria y reconocimiento que le ha dado a las explotaciones de oro en el Nordeste Antioqueño. El remarcado control estructural de las vetas del DMSR así como su localización espacial con respecto a la complejidad geológica de la zona, ha sugerido múltiples posibilidades para la procedencia de los fluidos mineralizantes. Según el presente trabajo, la paragénesis mineral global para muestras del DMSR en el área de la empresa Frontino Gold Mines (FGM), puede agruparse en tres estadíos de precipitación, donde la mena auro-argentifera estaría asociada a una Fase II denominada (Mineralizante) la cual fue acompañada de grandes volúmenes de Galena + Esfalerita + Cuarzo y en menor cantidad de Pirita ± Calcopirita ± Pirrotina. En las muestras del DMSR se observan inclusiones fluidas principalmente bifásicas acuosas (L H2O +V H2O ) las cuales poseen al menos tres asociaciones según su temperatura de homogenización y salinidad, el sistema salino dominante de las inclusiones fluidas es H 2 O+NaCl+CaCl 2 sugiriendo una fuente de derivación magmática.

INTRODUCTION: Few cases of obsessive-compulsive disorder (OCD) symptoms preceding the clinical on... more INTRODUCTION: Few cases of obsessive-compulsive disorder (OCD) symptoms preceding the clinical onset of Huntington Disease (HD) or during later stages of the disease have been reported in the literature, but the nature of this association and its neurobiological mechanisms have not been well-investigated. OBJECTIVES: To review the scientific literature regarding OCD symptoms in patients with HD and describe a case study from our clinic. METHODS: Extensive literature searches were performed to identify reports of patients with concurrent HD and OCD symptoms. RESULTS: Recent studies and the current case report suggest that OCD symptoms may predate or coincide with motor, affective or behavioral symptoms in patients with HD. The development of OCD and HD symptoms may involve structural and functional changes affecting the orbital and medial prefrontal cortex, ventromedial caudate nucleus, and pallidal sites. Few individuals have access to mental health services due in part to underdetection. As it is mor... more Few individuals have access to mental health services due in part to underdetection. As it is more likely to consult for medical conditions, primary care may be a useful gateway for early detection of mental health problems. Detection of the frequency of mental health problems in four hospital services at the Fundación Santa Fe de Bogotá: Outpatient unit, hospitalization, emergency department, and primary care through a brief detection questionnaire, the Patient Health Questionnaire (PHQ). Cross-sectional study of patients seen at the four services who answered a Demographic Data Questionnaire and the PHQ together with information gathered about current medical diagnosis, history of visits, and hospitalizations during the last year. 1094 patients seen at the four hospital services between September 2010 and May 2011 were selected at random. A mental health problem was detected in 36.7% of the total sample. Major depressive disorder (7.3%), alcohol abuse (14.4%), and any anxiety disorder (7.7%) showed the highest prevalence with the emergency department showing the highest frequency of detection. The usefulness of a brief detection questionnaire such as the PHQ in hospital settings is demonstrated and implications in the design of mental health programs in the general hospital are discussed. The need to replicate this study in other settings and to undertake further research is outlined.
