However, there is a few research conducted on this problem in Turkey. The purpose of this case report is to compose a scientific approach in the view of literature. We report an alcohol related sexual assault case for presenting adequate approach to these cases according to current guidelines. The victim was 17 years old female adolescent diagnosed and treated in the emergency service. The patient was hospitalized in the Child and Adolescent Psychiatry Service after emergency treatment in the Pediatric Emergency Medicine and Intensive Care Unit. A review and discussion of alcohol related sexual assault case is presented, and also multidisciplinary diagnosis and treatment approach is offered for this entity. The present study aims to investigate the role of Interleukin-6 (IL-6), total antioxidant activity (TAA), total oxidant activity (TOA) and tumor necrosis factor alpha (TNFα) levels on the development of the mitral valve chordae tendinea rupture. Methods: Our study consisted of 30 patients with mitral chordae rupture, 30 patients with severe rheumatic mitral regurgitation, and 20 healthy participants who were admitted to our polyclinic unit. The participants do not have a previously known comorbid disease. After transthoracic echocardiography, their diagnoses were confirmed by the transesophageal echocardiography. Plasma IL-6, TAA-TOA and TNF-α level of all patients were measured and their oxidative stress index (OSI) was calculated.

Despite being a widespread medical problem, very little is known about the etiology. Determining which agent initiates or worsens symptoms is important to the therapeutic approach. Organic causes that may lead to abdominal pain must first be excluded in these patients, after which symptoms should be assessed in the light of psychological and social conditions. An accompanying picture of psychiatric impairment has been reported in approximately 30% of patients. Studies have reported that psychosocial interventions are of considerable benefit in treating RAP. OBJECTIVES Percutaneous closure of secundum atrial septal defects (ASD) has become an important a... more OBJECTIVES Percutaneous closure of secundum atrial septal defects (ASD) has become an important alternative treatment to surgery. We evaluated our clinical experience with, and short-term results of transcatheter closure of ASDs with the Amplatzer septal occluder in adult patients. STUDY DESIGN The study included 52 patients (36 women, 16 men; mean age 33±14 years; range 14 to 69 years) who underwent transcatheter ASD closure with the Amplatzer occluder device. The mean ASD diameter measured by transesophageal echocardiography was 19.5±5.7 mm and the mean device diameter was 24.5±5.7 mm. All the patients were assessed clinically and echocardiographically one month after the procedure. RESULTS Transcatheter ASD closure was successfully performed in 48 patients (92.3%) and failed in four patients (7.7%). The worldwide prevalence of ADHD in children has been reported at 4-7%. Numerous populationand clinical-based studies have reported that more than half of cases of ADHD have at least one psychiatric comor‐ bidity. The presence of psychiatric comorbidities complicates the diagnosis, treat‐ ment, and prognosis of ADHD; thus, diagnosis of comorbidities is of great importance. Possible comorbidities should therefore be investigated in cases diagnosed with ADHD before treatment planning.
This study investigated the characteristics of abuse suffered by children, the dimensions of the ... more This study investigated the characteristics of abuse suffered by children, the dimensions of the psychiatric effects associated with abuse, and the factors affecting these. One thousand two cases aged under 18, exposed to sexual abuse, and referred over a 7-year period were assessed. Girls represented 80.8% of cases, and the numbers rose with age. The aggressors were all male, and 88.2% were known to their victim. Approximately half the children were exposed to sexual abuse involving penetration. Psychological pathology was identified in 62.1%. Female gender, the presence of penetration, physical violence, and incest significantly increased the development of psychological pathology. Levels of awareness in people close to and trusted by the child must be raised to minimize the adverse effects of trauma in the long term, pre- ventive measures must be taken, and medical and social support units from which victims can receive assistance need to be established.
ABSTRACT Objective: First-line treatment for Obsessive-Compulsive Disorder (OCD) includes Selecti... more ABSTRACT Objective: First-line treatment for Obsessive-Compulsive Disorder (OCD) includes Selective Serotonin Reuptake Inhibitors (SSRI) and Cognitive-Behavioral therapy (Storch & Merlo 2006). However, an important proportion of the childhood OCD cases do not respond to there treatments. Augmentation prescriptions are recommended especially for the children with OCD who do not respond to two different SSRI regimens. In OCD augmentation therapy, mostly the atypical antipsychotics, e.g. risperidone, olanzapine, and quetiapine are used (Grados & Riddle 2001). In these presentation, we aimed to present three pediatric OCD cases who were resistant to two different SSRI treatments and prescribed aripiprazole for augmentation therapy. Case 1: 17 year-old male patient had contamination obsessions and cleaning and counting compulsions for the recent one year period. The OCD symptoms did not respond to the fluoxetine 20-60 mg/day and afterwards sertraline 50-200 mg/day therapy. While he was on sertraline 200 mg/day, risperidone were added and tapered to 4 mg/day. He had partial remission. However, because of the extrapyramidal symptoms and lack of optimal efficacy, risperidone were switched to aripiprazole and tapered to 20 mg/day. In the progress, the patient had almost whole recovery for OCD symptoms in the fourth month of this regimen. Side effects of aripiprazole included “headache, fatigue, and insomnia” and disappeared in the fourth week of treatment. Case 2: 9 year-old female patient had contamination obsessions and cleaning and checking compulsions for the recent one year period. The OCD symptoms did not respond to the fluoxetine 20-60 mg/day and afterwards sertraline 50-150 mg/day therapy. While he was on sertraline 150 mg/day, aripiprazole were added and tapered to 15 mg/day. In the progress, the patient had almost whole recovery for OCD symptoms in the sixth month of this regimen. Side effects of aripiprazole included “dizziness and insomnia” and disappeared in the fourth week of treatment. Case 3: 16 year-old female patient had contamination obsessions and cleaning and checking compulsions. The OCD symptoms did not respond to the sertraline 50-200 mg/day and afterwards paroxetine 20-40 mg/day therapy. While he was on paroxetine 40 mg/day, olanzapine 5 mg/day were added. The patient did not want to use olanzapine because of sedation. Olanzapine were switched to aripiprazole and tapered to 15 mg/day. In the progress, the patient had almost whole recovery for OCD symptoms in the seventh month of this regimen. Discussion: Aripiprazole is a partial dopamine agonist and its prescription for childhood psychiatric disorders, e.g. bipolar disorder, autism, and schizophreina is increasing. Scarce number of case reports are presented on aripiprazole augmentation for child and adolescent OCD (Storch et al. 2008; Ulay 2010, Ozturk 2009, Akay 2011). The results of the previous case reports and these three cases suggest that aripiprazole is an effective and safe alternative as augmentation for childhood OCD treatment. Objective: The aim of this study is to present the outcome of the Communication with Hard of Hear... more Objective: The aim of this study is to present the outcome of the Communication with Hard of Hearing People and Turkish Sign Language elective program which is implemented for medical students. Materials and Methods : Feedback from the students was collected using a questionnaire of 21 statements under 10 headings. Student success was evaluated by theoretical and skill examination results and program success was evaluated by portfolio essays. In the data analysis, descriptive qualitative and quantitative analysis was used. Results : The mean overall score given to the program by the students was 8.53±0.88 out of 10. The students expressed the contribution levels of the block to the competence areas of professionalism as 8.45±1.64, to medical knowledge and understanding as 7.72±1.66, to clinical skills as 7.92±1.99 and to high-level skills as 8.09±1.52. Abstract Aim: The aim of this study was to determine the prevalence of childhood psychopathologie... more Abstract Aim: The aim of this study was to determine the prevalence of childhood psychopathologies in Turkey. Method: A nation-wide, randomly selected, representative population of 5830 children (6–13 years-old) enrolled as a 2nd,3rd or 4th grade student in 30 cities were evaluated for presence of a psychiatric or mental disorder by a Sociodemographic Form, Kiddie Schedule for Affective Disorders and Schizophrenia for School Age Children-Present and Lifetime Version (K-SADS-PL), and DSM-IV-Based Screening Scale for Disruptive Behavior Disorders in Children and Adolescents scales. Impairment criterion was assessed via a 3 point-Likert scale by the parent and the teacher independently. Results: Overall prevalence of any psychopathology was 37.6% without impairment criterion, and 17.1% with impairment criterion. Attention-deficit hyperactivity disorder was the most frequent diagnosis, followed by anxiety (19.5% and 16.7% without impairment, 12.4% and 5.3% with impairment, respectively). Lower education level and presence of a physical or psychiatric problem of the parents were independent predictors of any psychopathology of the offspring. Conclusion: This is the largest and most comprehensive epidemiological study to determine the prevalence of psychopathologies in children and adolescents in Turkey. Our results partly higher than, and partly comparable to previous national and international studies. It also contributes to the literature by determining the independent predictors of psychopathologies in this age group.

The present study was designed to evaluate the characteristics of pulmonary hypertension (PH) and... more The present study was designed to evaluate the characteristics of pulmonary hypertension (PH) and adult cardiology practice patterns for PH in our country. We evaluated preliminary survey data of 1501 patients with PH (females, 69%; age, 44.8±5.45) from 20 adult cardiology centers (AdCCs). The average experience of AdCCs in diagnosing and treating patients with PH was 8.5±3.7 years. Pulmonary arterial hypertension (PAH) was the most frequent group (69%) followed by group 4 PH (19%), group 3 PH (8%), and combined pre- and post-capillary PH (4%). PAH associated with congenital heart disease (APAH-CHD) was the most frequent subgroup (47%) of PAH. Most of the patients' functional class (FC) at the time of diagnosis was III. It is impossible to avoid contact with aluminum because of its existence in food to medications. Therefore, removal of aluminum from the blood or wastewater is urgently important. The cost-effective and easy-to-prepare adsorbents are needed to get efficient aluminum removal. For that purpose, the poly(2-hydroxyethylmethacrylate-co-acrylic acid), poly(HEMA-co-AA), microparticles was synthesized to remove aluminum in a very short interaction time. The achievement of the desired polymeric structure was confirmed via Fourier transform infrared spectroscopy (FTIR), scanning electron microscope (SEM) and transmission electron microscope (TEM), etc. Additionally, particle features such as swelling ratio, size, and surface area were determined. The microparticles synthesized in this study have been determined with very good adsorption capacity even in small aluminum concentrations.

We aimed to explore the shared and non-shared perinatal risk factors for autism spectrum disorder... more We aimed to explore the shared and non-shared perinatal risk factors for autism spectrum disorders (ASD) and attention deficit/hyperactivity disorder (ADHD) in a clinical sample. Additionally, we aimed to compare these groups regarding pre/postpartum maternal stress and the duration of breastfeeding. Children aged 3-18 years old with ASD (n = 100) were compared with age and gender matched children with ADHD (n = 100) and age and gender matched healthy controls (n = 80). This study revealed prematurity of the neonate and maternal stress/depressive mood in pregnancy as common risk factors shared by ASD and ADHD. The results suggested that postpartum maternal depressive mood may be more specific to ASD while shorter duration of breastfeeding may be related to ADHD. The results of this study revealed that ASD and ADHD may share some perinatal features in common. Identification of perinatal factors for ASD and ADHD carries clinical implications in terms of primary prevention.

Two hundred forty five expecting mothers and 150 expecting fathers were first interviewed between the 22-34 weeks of pregnancy. Sociodemografic data form, Adult Attachment Style Scale (AASS), State Anxiety Inventory (SAI), Edinburgh Postnatal Depression Scale (EPDS), Multidimensional Scale of Perceived Social Support (MSPSS) and Brief Symptom Inventory (BSI) were filled out by each expecting parent. The current study examined the rate and psychiatric correlates of sexual abuse involving the use ... more The current study examined the rate and psychiatric correlates of sexual abuse involving the use of digital technologies by the offender in a wide sample of juvenile victims. Sociodemographic, abuse, and psychiatric characteristics of 662 sexually abused children and adolescents were evaluated. Of these, 93 reported that digital devices were used by the offender in several ways to facilitate the sexual abuse. The offender-victim relationship was initiated through the Internet in 39 victims. Involvement of digital technologies in sexual abuse was significantly associated with penetrative and recurrent form of sexual abuse commited by multiple offenders with coexisting violence. Additionally, victims of sexual abuse with a digital component were 4.21 times more likely to develop any psychopathology, 3.77 times more likely to have depression, and 2.14 times more likely to have post-traumatic stress disorder as a result of sexual abuse. These results indicated that the offender's use of digital technology may aid the initiation and facilitation of the sexual abuse of youths and may relate to more severe outcomes. This study revealed the importance of raising the awareness of professionals and the community about the potential risks associated with digital technologies and sexual abuse. Mental health professionals should consider this additional form of victimization, especially when dealing with sexual abuse victims.

One thousand two cases aged under 18, exposed to sexual abuse, and referred over a 7-year period were assessed. Girls represented 80.8% of cases, and the numbers rose with age. The aggressors were all male, and 88.2% were known to their victim. Approximately half the children were exposed to sexual abuse involving penetration. Psychological pathology was identified in 62.1%. Female gender, the presence of penetration, physical violence, and incest significantly increased the development of psychological pathology. Levels of awareness in people close to and trusted by the child must be raised to minimize the adverse effects of trauma in the long term, pre- ventive measures must be taken, and medical and social support units from which victims can receive assistance need to be established.

However, an important proportion of the childhood OCD cases do not respond to there treatments. Augmentation prescriptions are recommended especially for the children with OCD who do not respond to two different SSRI regimens. In OCD augmentation therapy, mostly the atypical antipsychotics, e.g. risperidone, olanzapine, and quetiapine are used (Grados & Riddle 2001). In these presentation, we aimed to present three pediatric OCD cases who were resistant to two different SSRI treatments and prescribed aripiprazole for augmentation therapy. Case 1: 17 year-old male patient had contamination obsessions and cleaning and counting compulsions for the recent one year period. The OCD symptoms did not respond to the fluoxetine 20-60 mg/day and afterwards sertraline 50-200 mg/day therapy. While he was on sertraline 200 mg/day, risperidone were added and tapered to 4 mg/day. He had partial remission. However, because of the extrapyramidal symptoms and lack of optimal efficacy, risperidone were switched to aripiprazole and tapered to 20 mg/day. In the progress, the patient had almost whole recovery for OCD symptoms in the fourth month of this regimen. Side effects of aripiprazole included “headache, fatigue, and insomnia” and disappeared in the fourth week of treatment. Case 2: 9 year-old female patient had contamination obsessions and cleaning and checking compulsions for the recent one year period. The OCD symptoms did not respond to the fluoxetine 20-60 mg/day and afterwards sertraline 50-150 mg/day therapy. While he was on sertraline 150 mg/day, aripiprazole were added and tapered to 15 mg/day. In the progress, the patient had almost whole recovery for OCD symptoms in the sixth month of this regimen. Side effects of aripiprazole included “dizziness and insomnia” and disappeared in the fourth week of treatment. Case 3: 16 year-old female patient had contamination obsessions and cleaning and checking compulsions. The OCD symptoms did not respond to the sertraline 50-200 mg/day and afterwards paroxetine 20-40 mg/day therapy. While he was on paroxetine 40 mg/day, olanzapine 5 mg/day were added. The patient did not want to use olanzapine because of sedation. Olanzapine were switched to aripiprazole and tapered to 15 mg/day. In the progress, the patient had almost whole recovery for OCD symptoms in the seventh month of this regimen. Discussion: Aripiprazole is a partial dopamine agonist and its prescription for childhood psychiatric disorders, e.g. bipolar disorder, autism, and schizophreina is increasing. Scarce number of case reports are presented on aripiprazole augmentation for child and adolescent OCD (Storch et al. 2008; Ulay 2010, Ozturk 2009, Akay 2011). The results of the previous case reports and these three cases suggest that aripiprazole is an effective and safe alternative as augmentation for childhood OCD treatment. Materials and Methods : Feedback from the students was collected using a questionnaire of 21 statements under 10 headings. Student success was evaluated by theoretical and skill examination results and program success was evaluated by portfolio essays. In the data analysis, descriptive qualitative and quantitative analysis was used. Results : The mean overall score given to the program by the students was 8.53±0.88 out of 10. The students expressed the contribution levels of the block to the competence areas of professionalism as 8.45±1.64, to medical knowledge and understanding as 7.72±1.66, to clinical skills as 7.92±1.99 and to high-level skills as 8.09±1.52. Method: A nation-wide, randomly selected, representative population of 5830 children (6–13 years-old) enrolled as a 2nd,3rd or 4th grade student in 30 cities were evaluated for presence of a psychiatric or mental disorder by a Sociodemographic Form, Kiddie Schedule for Affective Disorders and Schizophrenia for School Age Children-Present and Lifetime Version (K-SADS-PL), and DSM-IV-Based Screening Scale for Disruptive Behavior Disorders in Children and Adolescents scales. Impairment criterion was assessed via a 3 point-Likert scale by the parent and the teacher independently. Results: Overall prevalence of any psychopathology was 37.6% without impairment criterion, and 17.1% with impairment criterion. Attention-deficit hyperactivity disorder was the most frequent diagnosis, followed by anxiety (19.5% and 16.7% without impairment, 12.4% and 5.3% with impairment, respectively). Lower education level and presence of a physical or psychiatric problem of the parents were independent predictors of any psychopathology of the offspring. Conclusion: This is the largest and most comprehensive epidemiological study to determine the prevalence of psychopathologies in children and adolescents in Turkey. Our results partly higher than, and partly comparable to previous national and international studies. It also contributes to the literature by determining the independent predictors of psychopathologies in this age group.

We evaluated preliminary survey data of 1501 patients with PH (females, 69%; age, 44.8±5.45) from 20 adult cardiology centers (AdCCs). The average experience of AdCCs in diagnosing and treating patients with PH was 8.5±3.7 years. Pulmonary arterial hypertension (PAH) was the most frequent group (69%) followed by group 4 PH (19%), group 3 PH (8%), and combined pre- and post-capillary PH (4%). PAH associated with congenital heart disease (APAH-CHD) was the most frequent subgroup (47%) of PAH. Most of the patients' functional class (FC) at the time of diagnosis was III. It is impossible to avoid contact with aluminum because of its existence in food to medications. Therefore, removal of aluminum from the blood or wastewater is urgently important. The cost-effective and easy-to-prepare adsorbents are needed to get efficient aluminum removal. For that purpose, the poly(2-hydroxyethylmethacrylate-co-acrylic acid), poly(HEMA-co-AA), microparticles was synthesized to remove aluminum in a very short interaction time. The achievement of the desired polymeric structure was confirmed via Fourier transform infrared spectroscopy (FTIR), scanning electron microscope (SEM) and transmission electron microscope (TEM), etc. Additionally, particle features such as swelling ratio, size, and surface area were determined. The microparticles synthesized in this study have been determined with very good adsorption capacity even in small aluminum concentrations.

The worldwide prevalence of ADHD in children has been reported at 4-7%. Numerous population-and clinical-based studies have reported that more than half of cases of ADHD have at least one psychiatric comorbidity. The presence of psychiatric comorbidities complicates the diagnosis, treatment, and prognosis of ADHD; thus, diagnosis of comorbidities is of great importance. Possible comorbidities should therefore be investigated in cases diagnosed with ADHD before treatment planning.

We aimed to explore the shared and non-shared perinatal risk factors for autism spectrum disorder... more We aimed to explore the shared and non-shared perinatal risk factors for autism spectrum disorders (ASD) and attention deficit/hyperactivity disorder (ADHD) in a clinical sample. Additionally, we aimed to compare these groups regarding pre/postpartum maternal stress and the duration of breastfeeding. Children aged 3-18 years old with ASD (n = 100) were compared with age and gender matched children with ADHD (n = 100) and age and gender matched healthy controls (n = 80). This study revealed prematurity of the neonate and maternal stress/depressive mood in pregnancy as common risk factors shared by ASD and ADHD. The results suggested that postpartum maternal depressive mood may be more specific to ASD while shorter duration of breastfeeding may be related to ADHD. The results of this study revealed that ASD and ADHD may share some perinatal features in common. Identification of perinatal factors for ASD and ADHD carries clinical implications in terms of primary prevention.

The current study examined the rate and psychiatric correlates of sexual abuse involving the use ... more The current study examined the rate and psychiatric correlates of sexual abuse involving the use of digital technologies by the offender in a wide sample of juvenile victims. Sociodemographic, abuse, and psychiatric characteristics of 662 sexually abused children and adolescents were evaluated. Of these, 93 reported that digital devices were used by the offender in several ways to facilitate the sexual abuse. The offender-victim relationship was initiated through the Internet in 39 victims. Involvement of digital technologies in sexual abuse was significantly associated with penetrative and recurrent form of sexual abuse commited by multiple offenders with coexisting violence. Additionally, victims of sexual abuse with a digital component were 4.21 times more likely to develop any psychopathology, 3.77 times more likely to have depression, and 2.14 times more likely to have post-traumatic stress disorder as a result of sexual abuse. These results indicated that the offender's use of digital technology may aid the initiation and facilitation of the sexual abuse of youths and may relate to more severe outcomes. This study revealed the importance of raising the awareness of professionals and the community about the potential risks associated with digital technologies and sexual abuse. Mental health professionals should consider this additional form of victimization, especially when dealing with sexual abuse victims.
